The Pediatric Hematology/Oncology Clinical Pharmacy Specialist provides advanced, comprehensive pharmaceutical care to children and adolescents with hematologic and oncologic conditions in the ambulatory setting. This pharmacist will practice as an integral member of the multidisciplinary care team, partnering closely with physicians, advanced practice providers, nurses, and the pharmacy staff to ensure safe, effective, and evidence ‑ based medication management.
The specialist will support chemotherapy management, optimize supportive care, participate in clinical research and quality improvement initiatives, and serve as a preceptor for pharmacy students and residents.
Essential Functions:
Review and interpret medication orders; assess for clinical appropriateness, therapeutic goals, adverse effects, contraindications, interactions, and disease ‑ specific considerations.
Evaluate, monitor, and modify drug therapy for pediatric hematology/oncology patients, including chemotherapy and supportive care.
Provide comprehensive medication counseling for patients and families, including oral chemotherapy education and adherence support.
Serve as the clinical liaison between the pediatric oncology practice and the Specialty Pharmacy, collaborating with pharmacy staff and support teams to ensure timely medication access and continuity of care
Advance standardized, evidence‑based care by developing and updating clinical guidelines, pathways, and EMR‑based decision support.
Participate in drug therapy consults and pediatric oncology clinical research protocols as needed.
Support quality assurance and improvement efforts to uphold accreditations and ensure compliance with regulatory and institutional standards.
Provide drug information and clinical recommendations to pediatric oncology providers, nurses, and other healthcare professionals.
Precept pharmacy students and residents; contribute to educational programming within the pharmacy department.
Representing Pharmacy Services on committees and workgroups as assigned.
Qualifications:
PharmD with PGY-1 residency or 5 years pediatric hospital experience required.
PGY-2 specialized residency training in pediatrics and/or Hematology-Oncology preferred.
Experience in ambulatory oncology pharmacy practice preferred.
Hospital pharmacy experience required (pediatric hospital pharmacy experience preferred).
Active Pharmacist License (or legally eligible) in States of Delaware, New Jersey & Pennsylvania required.
Board Certification in Oncology Pharmacy Specialty (BCOP) or BCPPS required within 12 months of eligibility.
American Heart Association BLS and PALS required upon hire. ACLS preferred.
PA Clearances are not required for those associates hired to work in DE.

Hospice Philosophy:
Hospice care is an interdisciplinary approach designed to meet the needs physical, psychological and spiritual of a person at the end of life with the goal of comfort and not cure. * Makes face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and reports clinical findings to the Hospice Medical Director
* Consults with the Hospice Medical Director as needed, informs primary physician of recommended services, and collaborates with other physicians as needed.
* Initiates and coordinates the plan of care. Documents problems, appropriate goals, interventions, and patient/family response to hospice care. Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care.
* Instructs and supervises the patient/family in self-care techniques when appropriate.
* Maintains accurate and relevant clinical notes regarding the patient's condition. Reports patient condition and any identified needs/issues to Administrator and Hospice Medical Director.
License Requirements:
* Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice.
* Current certification by a national body such as ANCC, NBCHPN, or AANP.
* Current driver's license and auto insurance
* Current CPR certification required.

Serving Wilmington community and Northern New Castle County, the Labor and Delivery floor at Saint Francis Hospital is a 9 bed unit. We care for various types of patients including but not limited to high risk pregnancies and complications i.e. Gestational DM, HTN, PTL, multiple births. All Registered Nurses are educated to scrub and circulate for C-sections along with phlebotomy and IV start skills. Additionally, we have 24/7 anesthesia and OB availability. We have ongoing evidence-based initiatives instituted i.e. preterm labor assessment tool, prevention of primary C-Sections, severe HTN in pregnancy.
Position Purpose
The staff nurse is a registered professional nurse who plans and renders nursing care within an assigned nursing unit. He/she gives direct patient care and maintains clinical records reflecting the patient’s condition, medication, treatment, examination, and progress according to the Delaware Nursing Practice Act and St. Francis Hospital’s policies and procedures.
As a Registered Nurse you will:
- Completes a comprehensive assessment of women during the antepartum period experiencing complications of pregnancy including preterm labor, premature rupture of membranes, pregnancy-induced hypertension, diabetes, bleeding and infection.
- Completes a comprehensive perinatal nursing assessment on pregnant women admitted to labor-delivery-recovery unit on admission.
- Assesses the woman’s educational needs during the intrapartum period and provides appropriate information to the woman and her family.
- Plans and delivers care to women who experience labor and birth without complications.
- Plans and delivers care to women with perinatal and/or medical complications including pregnancy-induced hypertension, preterm labor, bleeding in pregnancy, and multiple gestations.
- Provides supportive care to the woman and her family in situations involving fetal loss or fetal abnormalities.
- Plans and delivers care to women who labor without analgesia/anesthesia.
Minimum Qualifications:
- Current Licensure as a Registered Nurse in the State of Delaware
- ASN required, BSN preferred, from an accredited school of nursing
- BLS required; NRP within (6) months of employment
- At least 2-3 years' experience in Labor and Delivery
- RNC preferred.
- Ability to scrub and circulate for C-sections

What the Imaging Tech Assistant will do:
Patient Care & Support

This position provides comprehensive care coordination, supporting a holistic and coordinated approach across the continuum of care. Requires excellent organizational skills and clinical knowledge regarding specialty care services, as well as care coordination of services, legal and financial aspects of diagnostic services and health services in specialty area. Requires effective communication and writing skills, good time management skills and knowledge of word processing and database software applications. Overview

This Navigator should have knowledge or willingness to learn Addiction & Recovery Medicine and Behavioral Health care and will serve as primary patient and caregiver contact for education, symptom management, and care plan adherence, coordinate care across inpatient, outpatient, and community settings, including psychiatry collaboration and specialty consults. manage high-risk patient triage, medication protocols (e.g., buprenorphine inductions, alcohol withdrawal tapers), and neonatal abstinence program assessments, oversee ambulatory detox monitoring, specialty medication administration (Vivitrol), lab management, and medication reconciliation, support efforts to reduce readmissions and ED utilization through proactive follow-up and care coordination.
This is a full time (40 hours/week), salaried, day shift position. The average day starts at 0800 and ends at 1600 and is Monday through Friday. With this role, hours are dependent on patient needs and can start earlier or later on occasion. We believe in a work-life balance and will adjust accordingly if the need arises.
POSITION SUMMARY
This position provides comprehensive care coordination, supporting a holistic and coordinated approach across the continuum of care. This position assists with informed decision-making; collaborating with a multi-disciplinary team to allow for timely screening, diagnosis, treatment and increased supportive care throughout the patient experience. This position provides individual assistance to patients, families and caregivers to help identify and overcome barriers which may hinder quality medical and psychosocial patient care.
CORE FUNCTIONS
1. Assesses the patient’s plan of care and develops, implements, monitors and documents the utilization of resources and progress of the patient through their care, facilitating options and services to meet the patient’s health care needs.
2. Evaluates the medical necessity and appropriateness of care, optimizing patient outcomes. Serves as a liaison for referring physicians and assists with scheduling initial tests and consultations.
3. Assists the patient/family through diagnostic services, treatment and care. Coordinates disease care using an interdisciplinary holistic approach, making appropriate referrals and consultations in coordination with physicians and providers.
4. Ensures that the patient and family understand the diagnostic processes, care strategy and recommended actions, and are provided with appropriate information in coordination with physician and health care providers. Responds to patient requests for information regarding the disease process, expected side effects and community resources.
5. Evaluates patient’s functional abilities and limitations. Determines if intervention is needed. Establishes treatment goals that are functional, measurable, patient related and reflect key limiting factors. Establishes and implements a plan of care to achieve treatment goals. MINIMUM QUALIFICATIONS
