Make a Difference in the Lives of Children and Families
Join our supportive, mission-driven team as an Outpatient Child & Family Therapist and help provide high-quality, community-based mental health services to children, adolescents, and their families.
What You'll Do:
Provide individual, family, and group therapy to children and adolescents in an outpatient setting
Conduct comprehensive assessments, treatment planning, and documentation
Collaborate with schools, primary care providers, and other community stakeholders
Participate in clinical supervision, team meetings, and ongoing training opportunities

POSITION SUMMARY

PRIMARY DUTIES AND RESPONSIBILITIES
Provides comprehensive primary care through diagnosis, clinical judgment, and management to restore, maintain, and improve health status in accordance with practice policies. This includes:
Assessment of patient, emotional development status of individual and family.
Analysis of health behavior related to personality, life style, and culture.

Knowledge/oversight of Health Center operations, including crash cart, pharmaceuticals, lab and x-ray
Participates in clinical nursing research and application of research to nursing practice.
Maintains strict confidence on information gained in the clinical practice of the position.
Works closely with the EMTs in emergencies, using ACLS guidelines, when cases are presented to the Health Center by the EMTs or instances when EMTs have been called by the Nurse Practitioner.
Participates in the prescription drug control policies and prescribes according to state rules and regulations.
Merit increases will be based on random monthly or quarterly outcome reviews to evaluate productivity, and patient satisfaction. Ability to meet PCMH standards/MU criteria and outstanding performance over and above the job description.

*Job Requirements*
* Must have graduated from an ACOTE accredited school of occupational therapy and have valid certification from the National Board for Certification of Occupational Therapy (NBCOT) a valid occupational therapist state license in good standing (based on state specific requirements). It is the responsibility of the employee to maintain active licensing/certifications throughout their time of employment.
* Previous therapy experience is preferred, with some of that experience in a long-term care facility.
* Must provide all necessary documentation in accordance with the company’s new hire process to evidence eligibility for employment and pass a criminal background check.
* Must complete new hire orientation as required by company policy and participate in continuing education such as in-services, staff meetings, Relias training, etc.
* Must possess excellent interpersonal skills, including the ability to communicate clearly and work well with others.

*Summary of Principal Accountabilities*
* Optimize the functionality and skills of residents by providing, directing and/or overseeing occupational therapy screening of residents as appropriate, and evaluation and treatment of residents who may benefit from therapy services. The OT develops the initial treatment plan of care with revisions to the plan of care as needed throughout the resident’s stay.
* Comply with best practice industry standards as well as state and federal laws and regulations regarding accurate and timely billing in each resident’s medical record.
* Maintain completeness of each assigned resident’s medical record by completing all assigned evaluations, progress notes, discharge summaries, clarification orders, etc. according to posted schedules for the shift, making sure entries are timely, informative and accurate.
* Establish and maintain professional and positive working relationships with residents which reflects person-centered care. Honor the resident’s rights and provide care in a manner that promotes resident dignity and privacy.
* Establish and maintain a professional and positive working relationship with Certified Occupational Therapy Assistants (COTA). The OT is responsible to supervise the COTA in skilled tasks during group and individual treatment in a manner that promotes efficient and effective utilization of resources and the ongoing development of clinical skills. The OT will refer patients to the COTAs for treatment as appropriate and be available for consultation and clinical supervision to COTAs as needed. The OT should be familiar with what treatment can appropriately be delegated to a COTA and consult with the Director of Rehabilitation with any questions or concerns.
* Coordinate/collaborate on resident care with other members of the interdisciplinary and therapy teams. This may include attending or providing updates for resident care conferences and utilization review/case management meetings, reporting changes in resident condition or allegations of abuse, and making referrals to other members of the interdisciplinary and therapy teams as appropriate.
* Remain professionally informed and enhance clinical expertise as well as professional and management skills through interaction with managers, therapists and other professional staff, continuing education courses/trainings, and attendance at facility in-services and trainings. Implement new programs/techniques to enhance occupational therapy rehabilitation services.
* Understand relevant payment models related to billing and treatment guidelines as well as clinically appropriate means/modes of delivery. Provide quality care that falls within the guidelines of reasonable duration and type of services.
* Help maintain cleanliness of work areas, break areas, linen closets, supply closets, etc.
* Perform other related duties as may become necessary or as directed by supervisor(s).

RESPONSIBILITIES INCLUDE:
Provide therapeutic counseling to children and parents during supervised visits, using evidence-based and trauma-informed practices.

Support parents with conflict mediation, safety planning, and referrals as needed.
Collaborate with supervisors, caseworkers, attorneys, and other professionals to ensure coordinated care.
Mentor and guide program interns through training, observation, and partnership.
Co-facilitate PREPARE parenting group cohorts as needed.
Requirements:
QUALIFICATIONS:
Master’s degree in social work, mental health counseling, or related field.
At least two years of clinical experience with children and families.
Currently licensed in New York State in an appropriate mental health discipline or hold an active New York State limited permit at the time of hire. Candidates who do not hold an active limited permit at hire must obtain one within six (6) weeks of their start date.
Infant Mental Health Endorsement preferred.
Knowledge of child maltreatment, neglect, trauma, and high-conflict custody.
Experience in Infant-Parent Psychotherapy, Child-Parent Psychotherapy, Play Therapy, or Creative Arts Therapy preferred.

AMG Specialty Hospital - Lafayette is seeking PRN Respiratory Therapist for Day Shifts. Some of the medical conditions that are commonly treated include but are not limited to; Respiratory Failure requiring ventilation, vapotherm, high flow, bipap, cpap, and nasal cannula. Depending on the complexity of these patients, they can require sedation/pain management, pressor management, and cardiac/anti-arrhythmic management. Complicated infections requiring long-term IV antibiotics. Chronic non-healing wounds requiring extensive treatment with wound vacs and frequent dressing changes. RN's will be responsible for direct and indirect patient care in the long-term acute care setting. Ability to communicate with physicians, CCO/co-workers, as appropriate about changes in patient's clinical condition including results of diagnostic studies and symptomatology. Respond quickly and accurately to changes in condition or response to treatment. Additionally, is able to perform general nursing duties in all departments with adequate supervision.
