Join a growing cash-based practice built around one-on-one, evidence-based care, thoughtful clinical reasoning, and helping active people keep doing what matters to them.
As Longevity’s first clinical hire, you will have the support and structure to practice well—and a meaningful voice in how the team grows.
The Opportunity
*Care Model:* One-on-one care with no double-booking
*Caseload:* Approximately 30–35 patient visits per week once established, depending on demand, visit mix, and schedule design
*Schedule:* Full-time Monday–Friday, typically 7 AM–4 PM or 8 AM–5 PM by mutual preference; no routine evenings or weekends; protected admin time
*Compensation:* $80,000–$100,000 base salary plus performance-based bonus opportunity
*Start:* Anticipated Q1 2027, coordinated around notice, licensure, and onboarding
Why Longevity
We are intentionally building a practice where excellent care and strong patient relationships come before visit volume. Our model gives clinicians time to listen, assess thoroughly, explain their reasoning, treat meaningfully, and help patients build long-term independence.
You will join early enough to shape a growing practice, but with established clinical standards and operating systems already taking form. Owner, Dr. Sara Hegemier, PT, DPT, provides clinical mentorship and oversees the role.
What You Will Do
* Perform comprehensive evaluations and manage patients from evaluation through return to activity and beyond.
* Develop individualized plans of care tied to meaningful functional goals rather than protocols, visit quotas, or insurance standards.
* Use movement analysis, strength and conditioning, therapeutic exercise, therapeutic activity, neuromuscular re-education, manual therapy, patient education, and other evidence-informed interventions.
* Provide dry needling when qualified, clinically appropriate, and aligned with patient goals.
* Clearly explain findings, recommendations, expectations, and the value of appropriate care.
* Maintain accurate, timely documentation and take ownership of patient follow-through and outcomes.
* Participate in patient-fit conversations with prospective clients and help them decide whether Longevity is the right clinical fit.
* Build trusted relationships within Charlottesville’s fitness, wellness, and healthcare community.
* Contribute to team meetings, case review, quality improvement, and the continued development of Longevity’s systems and standards.
Who We Are Looking For
Clinical ability is essential, but it is not enough on its own. The right person is someone patients quickly feel comfortable with and confident in—a clinician who listens well, communicates clearly, exercises sound judgment, and wants to help build a small practice the right way.
We’re looking for someone with:

Qualifications
* Graduate of a CAPTE-accredited physical therapy program.
* Active Virginia Physical Therapist license or eligibility to obtain one before employment.
* Three or more years of outpatient orthopedic or musculoskeletal experience preferred. Exceptional newer clinicians with strong clinical judgment, communication skills, and alignment with Longevity’s model will be considered.
* Excellent written and verbal communication skills.
* Current First Aid/BLS certification.
* Advanced training or experience in dry needling, strength and conditioning, running-related injuries, sports rehabilitation, movement assessment, or performance-based physical therapy is strongly preferred.
Compensation & Benefits
* *$80,000–$100,000 base salary*, commensurate with experience, clinical expertise, and anticipated responsibilities.

Over time, the clinician builds a sustainable caseload through excellent care, communication, follow-through, and community reputation without compromising clinical judgment or patient needs—and strengthens the team and systems around them.

*Essential Duties, functions, & Responsibilities *
* Assists chair-side with pediatric, adolescent, adult, and geriatric patients, fully anticipating the Dentist’s need for various instruments, materials, and services.
* Documents examination information and treatment performed.
* Orders supplies and keeps Dental Office properly stocked at all times.
* Keeps all instruments sharpened and properly cleaned or sterilized.
* Takes alginate impressions for study models, pours and trims models, fabricates custom trays and bite rims.
* Applies and removes rubber dams, removes sutures, applies desensitizing agent, places surgical gauze following extractions and applies and removes periodontal dressing.
* Exposes intra-oral and extra-oral radiographs.
* Performs oral prophylaxis and applies topical fluoride treatments.
* Measures and records blood pressure and health history.
* Maintains a variety of reports related to dental activities.
* Follows occupational safety and infection control guidelines for Dental Office personnel.
* Clean, sterilizes and disinfects dental instruments and equipment, which includes running and recording weekly sterilization tests as stated in the QI Infection Control Standards.
* Relays post-treatment instructions to patients, motivates and instructs patients in proper brushing and flossing techniques, care of orthodontic appliances and common causes of tooth decay.
* Rotates with Dental Assistant at Tower dental station.
* Orders MSDS sheets with each new product; reviews them and binds them in the MSDS reference binder.
* Establishes a network of sales representatives, supply companies and dental laboratories.
* Assists in identifying high-risk groups and initiating preventive planning needs; assists in developing and implementing community dental prevention programs, such as National Children’s Dental Health-month and the Health Fair.
* Compiles and reports programmatic data and surveys required by the RTC, IHS and dental program to include annual, monthly, preventive, fluoride, and school reports.
* Answers correspondence to dental suppliers, doctors and patients as instructed by the Dentist.
* Assists with scheduling patient appointments and patient services.
* Participates in Quality Improvement activities; attends general staff meetings, dental staff meetings and Bemidji Area Dental meetings, as well as continuing education seminars to maintain licensure.
* Apply pit and tissue sealants and directed by Dentist.
* Remove excess cement from teeth using rotary instrument.
* Perform Orthodontic treatments as directed by the Dentist.
* As an employee of the Bois Forte Reservation, will follow the Bois Forte Procurement Policy and any other applicable procurement requirements when obtaining goods and/or services for the Bois Forte Band of Chippewa.

*MINIMUM MANDATORY QUALIFICATIONS*
Experience:

Responsibilities:
Oversees all stages of design, fabrication, fitting and maintenance of orthotic/prosthetic devices related to patients’ needs, as indicated by a physician.
Assists physicians with the evaluation and analysis of the patient’s need for orthotic/prosthetic services.
Ensures proper patient management is consistent with established practices for orthotic/prosthetic care.
Identifies and reconciles issues/problems to ensure patient satisfaction with devices provided.
Ensures highest standards of patient care are provided to all patients, including but not limited to adequate follow-up with patients and physicians.
Reviews new techniques and materials to ensure the widest range of flexibility and services offered.
Maintains accurate documentation to clearly define goals, issues and solutions for each patient.
Provides administrative staff with information to conduct reimbursement activities.
Provides information to facility management to ensure proper management of inventory and materials.
Meets with physicians, case managers and other referral sources to market orthotics/prosthetics services and capabilities.
Provides on-call services, as necessary.

Qualifications:
Bachelor’s degree from an accredited college or university from a recognized Orthotics and Prosthetics school.
American Board Certified (ABC) certification in Orthotics and Prosthetics required.
Minimum of three (3) years of experience in providing orthotic/prosthetic services under the supervision of a certified practitioner.

* Sorting and filing designated provider’s incoming faxes and form requests and distributing to the provider appropriately.
