We have an excellent opportunity for a full-time Family Medicine physician with OB and C-section skills at our Chelan location. Our Chelan site is staffed with 4 physicians and 2 Advanced Practice Provider’s. Full time is 4, 8-hour clinic days per week. Inpatient and OB call is shared with the other practice in town and averages every 5th night. Our Chelan site is home to a Family Medicine residency rural-training track and resident teaching is an integral part of the position.
This is a predominantly clinical role that includes protected faculty time, as the Chelan clinic serves as a provider training site.
Chelan is a destination resort town at the foot of a 50-mile mountain lake, surrounded by vineyards and mountains, with hot dry summers and snowy winters. The population of Chelan and the surrounding communities is 8,000, which swells to 20,000 on any given day during the summer. We are a 3-hour drive from Seattle and Spokane with airline service in Wenatchee, 45 minutes away.

Qualifications:
* Graduation from an accredited medical or osteopathic school with a degree of Doctor of Medicine or Doctor of Osteopathy. Must be Board Eligible/Certified.
* Certification/Licensure: Must possess a current, valid unrestricted license to practice medicine in the State of Washington or eligible to obtain licensure. Must have a valid DEA license. Basic Life Support (BLS) required. ACLS or ATLS may be used to substitute for BLS.
* Experience: No prior experience is required. Experience working in a community health center or managed care environment preferred. Experience in delivering services to medically underserved populations preferred.
* Bilingual English/Spanish preferred.
* Surgical obstetrical skills required.

POSITION SUMMARY
The CNA is responsible for providing tenants with the highest quality of direct routine care in all activities of daily living, under the supervision of the licensed nurse or AL Clinical Manager and in accordance with the Service Plan, MDH Nursing Assistant Program standards, the Tenant’s Bill of Rights and the campus Quality Assurance and Performance Improvement (QAPI) program. The Endodontist is full-time, exempt position responsible for providing endodontic services in a community health center setting. The Endodontist delivers root canal therapy, retreatment procedures, and related restorative procedures to underserved patient populations. The Endodontist supervises the dental assistants of the community health center in the absence of the dental director.
*Essential Functions*
§ Performs comprehensive endodontic diagnosis, including clinical examination and interpretation of radiographic and diagnostic data to develop individualized treatment plans.
§ Performs non-surgical root canal therapy (single and multi-visit) on anterior, premolar, and molar teeth, including treatment of complex and calcified canals.
§ Maintains strict aseptic techniques, including comprehensive caries removal and rubber dam isolation, to prevent bacterial contamination of the root canal system.
§ Places temporary restorations following endodontic procedures to protect the tooth pending definitive restoration.
§ Performs core buildups and post-and-core restorations to restore structural integrity of endodontically treated teeth prior to crown placement.
§ Manage endodontic emergencies including acute apical abscesses, irreversible pulpitis, and traumatic dental injuries.
§ Provides retreatment of failed root canal therapy, including removal of separated instruments, posts, and obturation materials as clinically feasible.
§ Consults and coordinates with general dentists and other dental specialists regarding case selection, referral criteria, and treatment sequencing.
§ Examines individuals requesting care, diagnoses their dental/oral conditions, prescribes and carries out, or directs others in carrying out, appropriate dental/oral treatment, or refers individuals for specialty consultation or treatment in conformance with approved clinical protocols and guidelines.
§ Records patient-dentist transactions as they occur in the patient's dental records so that the dental record accurately and completely reflects the nature of the contract, the condition of the patient and the care or treatment provided. In addition, the Endodontist shall complete referrals, data collections instruments and other records or paperwork as shall be required from time to time by the corporation.
§ Educates individuals in oral health related conditions and in the general promotion of oral health related disease prevention.

GENERAL DESCRIPTION
The objective of the staff physician position is to establish a comprehensive, community-based, family-centered, primary care program. The overall program will be designed to: a) improve access to a comprehensive range of high-quality primary care services; and b) improve health outcomes through an integrated delivery model focusing on prevention and health maintenance, health education, and evidence based chronic disease management.
DUTIES AND RESPONSIBILITIES
Develop a plan of care for each patient, including: Complete medical history, physical examination, diagnosis of the causes of injuries and illnesses, appropriate treatment and/or referral
Stress the importance of preventative medicine and utilizes all available resources, such as laboratory and radiologic testing as aids in diagnosing and confirming or denying the presence of disease
Practice medicine by caring for patients without discrimination as regards to age, race, color, national origin, ethnicity, sex, sexual preference, weight, handicap or income
Immunize and vaccinate patients against communicable diseases as recommended and required by the standard of care as it from time to time is updated
Confer with consulting physicians, nurses, patients and patients' relatives concerning treatment and care of patients, using good care and diligence as to confidentiality
Refer those cases which require specialist services, adhering to managed care contracts, principles, guidelines, using established and approved provider networks, and maintain responsibility for assuring that those services are required
Participate in practice development, quality assurance, peer review, and staff meetings/activities as deemed necessary by the CMO and/or the CEO
Cooperate and collaborate with other medical providers and support staff at the practice site to support effective communications and continuous improvement of the site’s clinical operation
Maintain accurate and concise documentation of all patient encounters on a timely basis, in such manner and form as determined by CHC, third party payers, federal funding and national accreditation agencies with which CHC has agreements and/or obligations
Provide services to a full patient panel, with a minimum of 1,500 active patients per provider FTE (Active patients shall be defined as those individuals who have had at least one completed medical encounter in the past two years. If the physician wishes to “close” practice to new patients, the minimum full panel of patients shall be attained, and agreement with the CMO and Chief Executive Officer must be obtained in writing)
Comply with the Federal Standards as outlined in the Bureau’s Provider Productivity Initiative Review (PPIR) as it pertains to the provider aspects of productivity/performance (In particular, for a full-time physician: a minimum of 4,200 encounters per fiscal year [as established by Medicare], cost per medical encounter compliance under the federal threshold as calculated each fiscal year)
Provide coordinated and comprehensive patient care throughout all major life cycles (except in the case of a provider specialty that limits the practice to a specific age, gender, or patient type), in all applicable care settings, including but not limited to: ambulatory outpatient; consultation; assistance to other providers, including mentoring and supervision; community medicine, such as sports physicals or services performed as a “Good Samaritan”; and services as defined/required by hospitals for admitting privileges or by contracts/agreements, i.e., third party payers, in which CHC participates
Perform other duties and services which may benefit patients and members of the community, or who could be patients of CHC, or which facilitates the provision of services to patients of the Corporation
Adhere to and provides services in accordance with the: a) mission of the Corporation; b) established and accepted standards of care; c) any clinical protocols and operational guidelines established by the Corporation; and c) terms and conditions as set forth in the Federal Grant, accrediting and licensing bodies (i.e. credentialing)
Cooperate with the Department of Justice in the defense of claims (including access to all pertinent documents, patient information and records). Furthermore, as the department of Justice requires, cooperate in providing information related to all previous malpractice claims as permitted by law and court action
Collaborate with Administration in the development and implementation of practice-specific strategic and operational plans
REPORTING RELATIONSHIPS

While the staff physician does not directly supervise support staff, he/she will direct the day-to-day activities of assigned support staff and will provide input into the evaluation of support staff performance

Works in cooperation with the all CHC staff, partners, and contractors to ensure the effective delivery of patient care
EVALUATION OF PERFORMANCE
The Director of Family Medicine, in conjunction with the CMO, will provide ongoing feedback as well as formal reports on all aspects of performance including: productivity, efficiency; knowledge and techniques of medical diagnosis and treatment, including knowledge of medications and tests; referral patterns; patient rapport; supervisory ability; record maintenance; adherence to Corporate policies, procedures and directives; and interaction with peer, support and administrative staffs.

As a key member of our Carolina Health Centers, Inc. team, the physician will play a vital role in establishing and overseeing a comprehensive, community-based primary care program.
