*Key Responsibilities*
* Perform daily duties as a staff pharmacist while providing clinical pharmacy services.
* Review patient medication regimens and clinical data to identify drug interactions, incompatibilities, dosing concerns, and other medication-related issues.
* Consult with prescribers, nurses, and other healthcare professionals regarding medication therapy.

* Maintain and support the effective use of drug information resources.
* Assist with the development, review, implementation, and maintenance of pharmacy policies and procedures.
* Monitor and analyze adverse drug reactions (ADRs), including preventability and severity, and communicate findings to pharmacy leadership and the Pharmacy & Therapeutics Committee.
* Report ADRs to the FDA when appropriate and incorporate findings into quality improvement initiatives.
* Conduct drug utilization evaluations and communicate findings and recommendations for process improvement.
* Develop and participate in quality improvement initiatives related to medication management.
* Participate in pharmacy staff education and competency assessments on clinical and medication-related topics.
* Assist with Pharmacy & Therapeutics Committee activities, including new drug reviews and pharmacy newsletters.
* Ensure medications are used safely, accurately, and appropriately for patients of all ages in accordance with professional standards, hospital policies, and pharmacy procedures.
*Minimum Qualifications*
* *Bachelor of Science in Pharmacy or Doctor of Pharmacy (Pharm.D.) degree.*
* *Current, valid Pennsylvania pharmacist license REQUIRED*
* Knowledge of hospital pharmacy practice, medication therapy, drug distribution, and medication control processes.
* Ability to interpret medication orders and clinical information accurately.
* Ability to calculate medication doses and apply pharmacokinetic principles.

*Work Environment & Schedule*
This position works in a contemporary hospital pharmacy environment providing unit-dose medication distribution, IV admixture services, drug information, and clinical pharmacy services using computerized pharmacy systems.
* Pharmacist coverage is provided *7:00 a.m.–5:00 p.m., Monday through Friday*, and *7:00 a.m.–3:30 p.m., Saturday, Sunday, and holidays*.
* Shift hours may vary based on departmental coverage needs.

This position will support a *caseload of fewer than 65 students* and offers the opportunity to work closely with teachers, families, and other special education professionals to support students' communication and academic success.

Hospice Philosophy:
Hospice care is an interdisciplinary approach designed to meet the needs physical, psychological and spiritual of a person at the end of life with the goal of comfort and not cure. Hospice considers the unit of care to extend beyond the patient and include the family and caregivers as defined by the patient.
The Hospice Nurse Practitioner provides consultation, symptom management and supportive care to meet the needs of the hospice patient as requested by the referring physician.
* Makes face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and reports clinical findings to the Hospice Medical Director
* Consults with the Hospice Medical Director as needed, informs primary physician of recommended services, and collaborates with other physicians as needed.
* Initiates and coordinates the plan of care. Documents problems, appropriate goals, interventions, and patient/family response to hospice care. Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care.

* Conduct triage assessments in urgent care or emergency settings to prioritize patient needs efficiently.
* Assist with specialized procedures such as cardiac catheterization, dialysis treatments, or trauma medicine in Level I or Level II trauma center environments.
* Educate patients and their families on health management strategies, medication administration, and post-discharge care instructions.
*Skills*
* Extensive experience working with diverse EMR/EHR platforms such as Epic, Cerner, Athenahealth, or eClinicalWorks.
* Proficiency in advanced clinical skills including IV insertion, phlebotomy, laboratory specimen collection/processing, and sterile processing techniques.
* Knowledge of complex medical terminology, ICD coding (ICD-9/ICD-10), CPT coding for procedures, and medical documentation standards.
* Critical care experience in ICU or emergency room settings with familiarity in airway management, ventilator operation, and acute care protocols.
* Demonstrated expertise in specialized areas such as cardiology (including cardiac catheterization), nephrology (dialysis), infectious disease care, hospice & palliative medicine, or geriatrics including memory care and Alzheimer's/dementia care.
* Strong understanding of infection control practices and OSHA safety standards across hospital or outpatient environments.
* Ability to perform physical examinations accurately while utilizing physiology knowledge for effective patient assessment and diagnosis.
