We are currently seeking Full or Part time Physical Therapists to care for home care patients in the Camden, Gloucester or Burlington County areas. The Physical Therapist provides clinical services to patients with functional limitations that affect their ability to carry out activities of daily living. The Physical Therapist provides direct care, support, and education in the home therapy program under the direction of the attending physician and in collaboration with the family, nursing case manager, home health aide, and any other disciplines. Assignments require considerable independence of professional judgment and are carried out in accordance with best practice standards and established policies, procedures, rules and regulations of the agency.

POSITION SUMMARY: As a key member of the Northpoint team, the Adolescent Therapist/Counselor will oversee the therapeutic services provided to a caseload of clients. This individual will act as the treatment plan coordinator for the multi-disciplinary team in the development and implementation of the master treatment plan and the discharge plan for each client. The Adolescent Therapist/Counselor will provide individual, group and family therapy for assigned clients, as required by contractual and licensure obligations. This person will actively communicate with support staff, treatment teams, and outside agencies on treatment and safety issues pertaining to clients and their family members. The Adolescent Therapist/Counselor will complete all documentation associated with these duties in a professional and timely manner.
ESSENTIAL RESPONSIBILITIES AND DUTIES:
Provide counseling, assessments and group facilitation to individuals and families
Coordinate all aspects of direct clinical care from admission to discharge
Consult with the multidisciplinary treatment team to provide quality client care
Provide orientation and education to clients, families and others to aid in the therapeutic process
Collaborate with assigned clients in the development of written treatment plans; include client and family involvement in assessment, treatment, education and discharge planning
Monitor clients on an ongoing basis to continually assess needs and facilitate treatment and interventions
Document clinical assessment, diagnosis, treatment plan, diagnostic summaries, and progress charting in a timely manner to assure comprehensive and quality client care
Actively participate as a member of the multidisciplinary treatment team and maintain effective working relationships with employees and peers
Maintain professional boundaries with clients at all times
Oversee and participate in the quality assurance and compliance activities of the facility
Provide clinical updates to utilization review and representatives of managed care to best coordinate reimbursement of treatment services

*MAJOR FUNCTIONS:*
1. INTENSIVE LEVEL OF CARE
A. Maintain case management of a caseload of up to 12 participants using active treatment principles.
B. Act as Treatment Team Leader for weekly internal staff meetings.
C. Ensure that each resident receives 75 minutes of therapy and 75 minutes of therapeutic intervention services per week. Individual therapy must be provided at least 50 minutes out of the required 75 minutes per week and family therapy must be provided to the youth and biological, adoptive, or foster family members with whom the youth previously resided or plans to reside with upon discharge. If family therapy is not appropriate based on the particular situation of the youth, the lead clinical staff shall document specific reasons why family therapy cannot be provided.
D. Follow Progress Notes Policy for documentation guidelines. Therapy sessions and therapeutic intervention must address the youth's treatment goals and objectives in the treatment plan and each session must be documented in the case record by the lead clinical staff.
2. Complete clinical assessment within 10 business days of the youth's admission.
3. Develop and complete Individual Case Plans/ Master Treatment Plans on each participant. Master Treatment Plans must be reviewed and updated at least every 30 days. A copy of the treatment plan must be provided to the youth's placing agency and custodial parent or guardian within 10 business days of plans development. Ensure Discharge Summaries are completed within 10 business days, ensuring a continuum of care is in place for participants and family. (See Individual Treatment Plan/ Master Treatment Plan and Discharge Summary Policies.)
4. Develop and submit in a timely manner “Continuation of Stays” for participants meeting the medical criteria for stay at the therapeutic group home level of care.
5. Oversee therapeutic infrastructure of group home as well as maintains organizational structure within the group homes and delegates responsibilities to assigned Program Managers, Lead Youth Care Specialists, and/or Youth Care Specialists. Serves as liaison between administration/business office and the group homes. Report directly to supervisor. Attend clinical meetings.
6. Complete quarterly chart audits under the direction of Quality Assurance or Clinical Director.
7. Other duties as assigned by New Day, Inc. administration.

*Education and/or Experience: *A minimum of a Master’s degree from an accredited college or university with major course work in social work, counseling or psychology, as well as management and extensive experience working with troubled youth, families, and the subsequent development of the emotional problems by displaced/dysfunctional youth. Uphold the highest standards of psychiatric practice, including ensuring informed consent for treatment, respecting patient confidentiality, and adhering to ethical guidelines set forth by psychiatric governing bodies.

* Reviewing documentation and evaluating the efficacy of interventions identified in the FBA/BSP.
* Developing data collection mechanisms and collecting baseline data
* Designing a written therapeutic consultation plan detailing the interventions, environmental adaptations, and support strategies to address the identified issues and desired outcomes, including recommendations related to specific devices, or adaptation of other training programs or activities.
* Demonstrating specialized, therapeutic interventions and individualized supports
