Key Responsibilities
Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.
Collaborate with the interdisciplinary care team to optimize patient outcomes.
Provide education and support to patients and families/caregivers on therapy programs.
Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
Identify patient and family/caregiver needs for additional services and make appropriate referrals.
Confirm daily visit schedules to ensure seamless coordination with other staff.
Communicate patient progress and changes with physicians and care teams.
Document all visits and progress notes on the same day of service.
Requirements:
Qualifications
Must have a current PT license (state-specific).
Graduate of an accredited physical therapy program.
Valid BLS certification.
Minimum one year of PT experience (home health experience preferred).
Excellent communication, organizational, and time management skills.
Must have a valid driver’s license and access to a reliable and insured vehicle.
Ability to visit homes that may not be wheelchair accessible.
Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
Proficiency with electronic medical records is a plus.

Responsibilities:

We're looking for a Nurse Practitioner to provide *on-site primary care to older adults throughout St. Cloud and surrounding communities*, with additional coverage in the Willmar area as needed.
Instead of practicing inside the same clinic every day, *you go to your patients*, caring for seniors where they live and building relationships with the communities surrounding them.
You'll have significant clinical autonomy, a *Monday through Friday schedule with no evenings, weekends, or hospital call*, and *$120,000 - $160,000+ in total compensation* with additional earning potential through our productivity incentive program.
Build Your Practice Across Central Minnesota
This isn't a role where every morning starts with a waiting room full of unfamiliar faces.
You'll develop a consistent patient population across senior living communities and follow those patients over time.

What You'll Be Doing

Experience in *primary care, family practice, internal medicine, geriatrics, long-term care, skilled nursing, post-acute care, hospital medicine, home health, hospice and palliative care*, or other settings serving medically complex adults can translate extremely well.
What matters is whether you have the *clinical judgment, independence, communication skills, and interest in older adults* needed to manage a medically complex patient population.

*Essential Duties, functions, & Responsibilities*
* Secure a health and developmental history from the patient or parent and perform a detailed physical exam; records findings and makes critical evaluations. Performs or requests special screening and developmental tests and diagnostic tests and procedure and interprets the results.
* Discriminate between normal and abnormal findings to recognize early stages of serious physical, emotional or mental problems. Make decisions concerning medical care needs of patients which may involve various treatment modalities, prescribed medications, consultation, and referral when appropriate.
* Provide direct and independent medical care in the presence of illness or disability in order to maintain life, provide comfort, reduce distress, and enhance coping ability.
* Provide surveillance or medical practices applied to stabilize chronically ill persons; adjust treatments within established standing orders recognizing when to refer the patient to the physician or other health team members.
* Provide Medical Services during regular business hours.
* Provide the greatest scope of services possible and refers out the least number of patients as possible.
* Assess healthcare plans and provides comprehensive and therapeutic medical care to patients in a family practice facility.

Experience:

Education:

· Excellent interpersonal skills in handling interactions with hospital staff, other agencies, groups, and patients and families; capable of working with and actively participating with a diverse multi-disciplinary team.
