You finish seeing one of your longtime patients, update the nursing team on her plan of care, grab your things, and head to your next community.

Instead, you're out in the community practicing medicine where your patients actually live.
At Twin Cities Physicians, we're looking for a Nurse Practitioner who wants to build relationships with older adults throughout *Owatonna, Rochester, Albert Lea, and surrounding Southern Minnesota communities*.
You'll have the independence to manage your patients, the variety of practicing across multiple communities, and the backing of an experienced *physician-led clinical team* when you need it.
For an NP who enjoys *autonomy, geriatrics, and getting out into the community*, this can be an incredibly rewarding way to practice.

Your patients shouldn't always have to come to you.
Sometimes better healthcare means *going to them*.
You'll provide primary care directly within *assisted living, memory care, independent living, and skilled nursing communities*, caring for older adults with a wide range of clinical needs.
You'll manage chronic conditions, address acute concerns, follow patients after hospitalizations, and work closely with the nurses and caregivers who know them best.
And because you'll follow many of these patients over time, you'll have something that's becoming harder to find in healthcare:

One day may take you into a memory care community in Owatonna.
Another may include patients around Rochester.
Another may bring you farther south toward Albert Lea.
Wherever you're rounding, your purpose remains the same: *bring proactive medical care directly to seniors who benefit from having a consistent provider where they live*.
You'll work alongside nurses, caregivers, therapists, pharmacists, families, and facility leadership to understand the complete picture surrounding each patient.
That can mean catching a subtle change before it becomes an emergency, helping stabilize a chronic condition, or making sure a hospital discharge plan actually works once the patient returns home.
Your work helps *strengthen continuity of care, reduce unnecessary hospitalizations, and keep older adults healthier where they call home*.
*What You'll Be Doing*
* Provide comprehensive primary care throughout *senior living and long-term care communities*.
* Travel throughout *Owatonna, Rochester, Albert Lea, and surrounding Southern Minnesota communities*.
* Diagnose and manage *acute and chronic medical conditions*.
* Perform comprehensive assessments, preventative care, and routine follow-up visits.
* Develop individualized treatment plans.
* Manage chronic disease and monitor changes in condition.
* Coordinate care following hospitalizations, rehabilitation stays, and specialist visits.
* Collaborate with *physicians, nurses, therapists, pharmacists, caregivers, and senior living teams*.
* Educate patients and families regarding diagnoses, medications, and treatment plans.
* Help improve outcomes through *proactive and preventative medicine*.
* Maintain accurate and timely clinical documentation.

The Hospice RN is responsible for planning, organizing and directing hospice care for Eventide Hospice Senior’s/resident’s/client’s.
Essential Functions
Completes an initial, comprehensive and ongoing comprehensive assessment of Senior/client and family to determine hospice needs. Provides a complete physical assessment and history or current and previous illness(es).
Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act.
Assesses and evaluates Senior’s/client’s status by:
Writing and initiating plan of care that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions
Regularly re-evaluating Senior/client and family/caregiver needs
Participating in revising the plan of care as necessary, including the Senior/client and the family in the planning process
Uses health assessment data to determine nursing diagnosis.
Initiates appropriate preventative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
Provides health care instructions to the Senior/client as appropriate per assessment and plan.
Assists the Senior/client with the activities of daily living and comfort care.
Acts as Case Manager when assigned by Patient Care Coordinator and assumes responsibility to coordinate Senior/client care for assigned caseload.
Completes, maintains and submits accurate and relevant clinical notes regarding Senior’s/client’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
Communicates with the physician regarding the Senior’s/client’s needs and reports changes in the Senior’s/client’s condition; obtains/receives physicians’ orders as required.
Ideal Candidate
AGE REQUIREMENT: At least age 18.
EDUCATION: Successful completion of a Registered Nurse course of study. Bachelor’s degree in Nursing preferred.
EXPERIENCE: Two years of acute care/long term care nursing experience within the last five years preferred. Experience in community health and/or hospice preferred.
CREDENTIALS: RN License in good standing as recorded on the Nebraska State Registry or other Nurse Compact state required. CPR required. License must be kept up to date at the expense of the TEAMember. CPR certification must be kept up to date at the expense of the TEAMember. JOB TITLE: School Psychologist
JOB SUMMARY: Provide direct and indirect (relational) therapy to identified students (individual and group) to foster and track social, emotional, and cognitive development.
Directly responsible for the completion of all adaptive, social / emotional, and intellectual assessments and evaluations (and achievement assessments, as deemed necessary) to identify student profiling (strengths and weaknesses across all contexts) while developing and implementing learning, retention, communicative, and behavioral strategies for students, in collaboration with student’s IEP teams.
Provide consultative services to families, staff, and community members to support and promote students' success.
In collaboration with student’s IEP team, assists with behavior intervention plans and functional behavior assessments, and assistance with classroom management technique and staff development.
EDUCATION: Doctorate or Master’s Degree in School Psychology or related field, appropriate DOE certification, and licensing by NH State Board.
EXPERIENCE: Two years of experience working with children with disabilities in applied settings, developing and implementing behavioral, social, daily-living, and academic interventions and applicable skill-development programs. Knowledge of behavioral mental health, psychopharmacology, psychotherapy, and consultation models appropriate to their respective targeted population and environment.
PHYSICAL REQUIREMENTS: Free of restrictions that would limit the use of the hands as it relates to computer operation, with or without reasonable accommodation and or mechanical devices, and the ability to stand, walk, sit, squat, stoop, lift and carry fifty (50) pounds, and accomplish physical management of students’ safety consistent with CPI.

ACCOUNTABILITIES:

3. Reviews psycho-educational (and related) reports, case management discussion, and procedural guidelines to assess the need for behavioral services, individual counseling, and group counseling. Interviews and/or observes new and existing students that present difficult and complex behavior problems in collaboration with IEP team members. 4. Conducts testing and evaluation / reevaluation of identified students when assessing behavioral, emotional, and intellectual domains as appropriate. Collaborates with parents, districts, students, and professional staff to help develop an individualized comprehensive program of learning, therapy, evaluation, treatment, and goals and objectives (included within the IEP and respective treatment plans). Collaborates with primary care physician(s) and other related clinicians that provide medical / therapeutic / behavioral treatment to students.
5. Targets socially significant behaviors when developing plans for skill acquisition and behavior reduction.
6. Provides training opportunities for staff, as deemed necessary and appropriate. Ensure that child engagement and restraint procedures utilized are consistent with the safest, ethical and legal practices and strategies.
7. Participates in clinical and educational research via data collection, compilation, documentation, and analysis of clinical data and therapy notes. Collaborates in the development of guidelines for the collection of clinical data for student progress monitoring and research.
8. Performs reliability checks on data collection and intervention implementation to ensure consistency and to accurately report on student progress.
9. As a mandated reporter, communicates any educational, behavioral, safety, or family concerns to immediate supervisor (and appropriate reporting agency) in a timely manner.

The Opportunity
The Speech-Language Pathologist will provide evaluation, treatment, consultation, and support to children and adults experiencing communication, language, social communication, cognitive-communication, and/or swallowing needs.
But this position is about more than providing therapy.

An SLP may help a child find a way to communicate a need, help an adult become more independent in the community or workplace, collaborate with a family who has struggled to access services, or help a team discover a new way to support someone successfully.
We are looking for an SLP who sees the person before the diagnosis and understands that sometimes the smallest communication breakthrough can open an entirely new world of possibilities.
Essential Responsibilities
* Conduct comprehensive speech, language, communication, cognitive-communication, social communication, and swallowing evaluations within the SLP's scope of practice.
* Develop individualized, person-centered treatment plans based on assessed needs, personal goals, family priorities, and functional outcomes.
* Provide individual and group speech-language therapy to children and adults with intellectual, developmental, neurological, and other disabilities.
* Support individuals who use or may benefit from Augmentative and Alternative Communication (AAC) and other assistive communication strategies.

Primary Purpose
Provide direct primary medical care to all participants with the highest medical standards and ethics in a caring, nurturing atmosphere while keeping with Senior Care Partners P.A.C.E. mission, vision, and values.
Duties and Responsibilities
Performs an initial comprehensive history and physical assessment on new enrollees, interval assessments on established participants (semi-annually, as condition necessitates or as requested) and documents in the medical record as required.
Functions as a member of the Interdisciplinary Team (IDT), communicates participant changes to team members, and participates in IDT meetings.
Assists in the development, implementation, and evaluation of the participant plan of care as described in Senior Care Partners P.A.C.E. policy.
Evaluates and treats participants with episodic illnesses and documents in the medical record as required by policy.
Provides, promotes, and educates participants and families on preventative health maintenance of their chronic medical conditions and actively participates in family conferences.
Regulates and adjusts participant medications as needed and in a cost-effective manner.
Facilitates consultations with medical specialists as indicated and oversees care rendered.
Collaborates effectively with primary (attending) medical management while an inpatient; Provides updates to the IDT on a regular basis; Judiciously utilizes specialty consultants; Coordinates discharge planning with the IDT and hospital discharge planners.
Manages all of a participants care if admitted to a skilled nursing facility (SNF) to include: Regularly visit the SNF for participant evaluation; Communicates with SNF staff via phone/fax as requested/required.
Participates in committees, work groups as assigned, and Quality Improvement activities as required.
Participates in the coordination of the participant’s 24-hour delivery of care services.
Maintains current accurate documentation of health care services provided, coordinated or contracted.
Must care, support, and collaborate with everyone involved in participant's care including the participant’s family using the patient-family centered care approach.
Supervisory Responsibilities
This position has no direct supervisory responsibilities. The Physician works collaboratively with Nurse Practitioners and other healthcare professionals as part of the interdisciplinary care team to support the delivery of patient-centered care.
Qualifications
For an MD/DO or NP Role
Graduate of an accredited School of Medicine, College of Osteopathic Medicine, or accredited Nurse Practitioner program.
Current licensure as a Physician (MD/DO) or Advanced Practice Registered Nurse (APRN) in the applicable state.
Board certification in Internal Medicine, Family Medicine, Geriatric Medicine, or applicable Nurse Practitioner specialty certification preferred.
Minimum of one year of experience working with frail elderly, long-term care, hospice, palliative care, or medically complex patient populations.
Demonstrated ability to work collaboratively within an interdisciplinary team to manage complex patient care needs.
Specific Requirements
Must possess a current, unencumbered, active license to practice as a MD or DO in the State of Michigan.
Must possess a current, unencumbered, active DEA license.
Current privileges or the ability to obtain and maintain staff privileges at Senior Care Partners P.A.C.E. contracted provider facilities
Possess current CPR certification by AHA or willing and able to obtain as offered by Senior Care Partners P.A.C.E.
Must be able to read, write, speak, and understand the English language and follow written and oral instructions.
