We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. In this role, you will be responsible for evaluating, developing, and implementing personalized treatment plans to improve patients’ mobility, strength, and overall well-being. You will work closely with a multidisciplinary team to deliver high-quality patient care across diverse settings, including outpatient clinics, inpatient facilities, home health environments, and rehabilitation centers. Your expertise will help patients regain independence and achieve their health goals through evidence-based physiotherapy practices.
*Responsibilities*
* Conduct comprehensive assessments of patients’ physical conditions, including mobility, strength, flexibility, and functional abilities.
* Develop individualized treatment plans aligned with patient needs and clinical best practices.
* Implement therapeutic interventions such as manual therapy, trigger point therapy, therapeutic exercises, and modalities to promote recovery.
* Collaborate effectively with healthcare professionals—including physicians, nurses, occupational therapists, and social workers—to coordinate care and optimize patient outcomes.
* Manage discharge planning processes by preparing care plans for patients transitioning from hospital to home or community settings.
* Document all patient interactions accurately in electronic health records (EHR) systems following clinical documentation standards.
* Provide education and guidance to patients and their families on exercises, injury prevention, and home care strategies to support ongoing rehabilitation.
*Requirements*
* Valid state licensure or certification as a licensed Physical Therapist.
* Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health settings.
* Strong knowledge of physiology, anatomy, orthopedics, neurology, geriatrics, pediatrics, and related medical terminology.
* Experience with outpatient rehabilitation center environments and inpatient hospital settings preferred.
* Familiarity with electronic health records (EHR) management systems and EMR (Electronic Medical Records) systems for accurate documentation.
* Ability to develop effective treatment plans tailored to diverse patient populations including geriatric and pediatric clients.
* Excellent client communication skills with the ability to motivate and educate patients throughout their recovery process.
* Knowledge of discharge planning procedures and working collaboratively with healthcare teams on care plans.
* Prior experience working with acute care cases and home & community care environments is highly desirable.

Responsibilities
Complies with facility policies and procedures for appropriately documenting, billing and coding for residents receiving speech language pathology and audiology services.
Provides resident information needed for the resident assessment instrument according to facility policy such as signs and symptoms of swallowing disorders, speech and hearing patterns, cognitive patterns, Brief Interview for Mental Status (BIMS), speech language pathology and audiology minutes and distinct calendar days, etc. during the defined observation period(s).
Provides resident information needed for prior functioning in functional cognition as required by facility policy (RAI User’s Manual) for Medicare Part A, Medicare Advantage residents upon admission.
Required Qualifications:
Master’s Degree, or equivalent, in Speech Language Pathology
A valid, unrestricted license in his/her therapy discipline in the state.
Knowledgeable of skilled nursing home regulations, procedures, laws, regulations and guidelines pertaining to long-term care.

The Clinical Pharmacy Specialist position is responsible for providing and coordinating specialty pharmacy services within the Pharmacy Department at Nemours Children's Hospital, Delaware. The Clinical Pharmacy Specialist will be responsible for clinical and distributive activities in the assigned area of practice. This position works closely with the Medical staff, Nursing staff, and Pharmacy staff to develop and maintain services that utilize the pharmacist's drug knowledge base to improve patient care, reduce cost, reduce medication errors, and improve efficiencies. There is also an expectation to enhance pharmacy students' professional experiences and serve as a mentor for pharmacy interns and externs and pharmacy residents.
Essential Functions:
Evaluates medication orders for accuracy using sound principles, accounting for pharmacology of the drug, pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism, and elimination with responsibility for establishing and continually improving the delivery of pharmaceutical care to patients within areas of direct responsibility and assisting others in the department with the same.
Consults, educates, and works cooperatively with internal customers (e.g., members of the pharmacy staff, medical staff, clinical staff, research staff, nursing staff, and informatics staff) concerning information on medications, therapeutic evaluations, and drug protocols. Develops and provides drug information material. Meets and/or coordinates with medical and/or personnel to resolve problems related to Clinical Pharmacy Services.
Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes including advising physicians and practitioners on issues concerning drug therapy in both the hospital and clinic settings, the inherent toxicity of drugs, drug-drug, drug-food interactions & side effects, as well as assisting in the prescribing of appropriate doses.
Develops quality improvement projects and participates in the quality improvement and medication use review activities of the department. Collects data; monitors processes; maintain logs, records, and other documentation as assigned.
Provides clinical support for multidisciplinary committees, including the Pharmacy and Therapeutics Committee, through the development of materials to support clinical evaluation of drugs, procedures, and processes including the coordination and reporting of adverse drug reaction reporting, pharmacist interventions and other quality initiatives, within areas of direct responsibility. Promotes the use of the NCHD drug formulary by converting non-formulary orders to formulary alternatives when clinically appropriate. Coordinates procurement of non-formulary drugs when necessary.
Provides oversight for the accuracy of the online Lexi-comp drug information resource. Promotes the use of the NCHD drug formulary by converting non-formulary orders to formulary when possible and coordinates procurement of non-formulary dugs when necessary.
Supports clinical staff pharmacist education and training associated with core competencies and performance review.

*Compensation:* $61/hour + daily bonus

* Perform top-notch dental hygiene services: cleanings, scaling, fluoride/sealants, perio maintenance, etc.
* Complete comprehensive assessments and screenings, including oral cancer checks.
* Review and document patient medical/dental histories and vitals.
* Educate and empower patients on oral hygiene and preventative care.
* Collaborate with dentists on treatment plans and patient care.
* Administer anesthetics and cavity-prevention treatments.

The PT - Physical Therapist provides direct rehab care in physical therapy to patients upon physician referral in accordance with all applicable laws, regulations, and Life Care standards.
Education, Experience, and Licensure Requirements:
* Graduate of an accredited program in physical therapy, BSPT, MSPT, or DPT (2003 or later must have MSPT or DPT)
