- Provides biopsychosocial assessments, clinical behavioral health diagnoses, treatment planning, evidence based clinical interventions, information and referrals to community.
- Interviews patient/patient's social support system to complete initial psychosocial evaluation and/or completes psychosocial assessment for the patient.
- Provides brief psychotherapy, supportive counseling, and education to patient/patient's social support system. Assists in coping with stress related to hospitalization, disability, chronic/terminal illness.
- Identifies, plans, and coordinate aspects of care for the patient/family.
- Develops, monitors and revises a plan of care based on assessment of patient/family needs, in collaboration with multidisciplinary team.
- Provides and assesses effectiveness of patient/family education.
- Advocates for the patient and family throughout care continuum.
- Maintains availability to the patient/family as a resource to facilitate communication among providers and to monitor services rendered.
- Provides clinical supervision to provisionally licensed social worker and/or mentors new social workers in the department.
- Serves as a resource to team members and physicians regarding emotional, social and psychosocial components of the patient's illness and its effects on their social support system.
- Intervenes in crisis situations, appropriately and effectively applying crisis intervention theory and skills.
- Participates in the initiation, development, and maintenance of clinical pathways/protocols and other population-based programs, facilitating collaboration of multidisciplinary teams in the process.
- Analyzes and evaluates the effect of social work involvement on quality outcomes, fiscal parameters, customer satisfaction, and system operations and implements strategies to resolve the system, performance, and patient variances.
- Refers patients/family to external agencies/resources for treatment when appropriate.
- Gathers and interprets appropriate data from the medical record, physician reports, patient, and/or family as evidenced by documentation.
- Documents accurately, timely and according to departmental policy.
- Participates, as appropriate, in clinical research and may provide local, national, and/or international presentations within specified field.
Hearing (corrected) adequate for oral/aural communication with patients, team members, family, visitors, etc. Vision (corrected) adequate for reading. Intelligible speech and normal language/cognitive skills. Must be able to push patients in a wheelchair or stretchers. Lifting of patients, equipment or supplies will be required up to 20 pounds frequently and 50 pounds occasionally. Able to laterally transfer patients 100 pounds rarely. Sitting, standing, and walking required throughout the day. Sometimes requires climbing stairs, kneeling, twisting, bending; on occasion, crouching, crawling and reaching overhead. Works is in a fast-paced clinical environment. The work environment is primarily indoors.

- Assesses and monitors nutritional status of patients/clients and adjusts nutritional care as indicated utilizing current scientific literature on methodology and relates this information to medical status, anthropometric data, clinical assessment, and nutrition evaluation of the patient/client.
- Provides medical nutrition therapy (MNT)/counseling including assessment and education, developing, monitoring, and implementing MNT/counseling for patients/clients in assigned areas. Completes consults and referrals as received.
- Communicates with physicians, nurses, and others to coordinate nutrition into a total patient/client therapy program.
- Completes all required documentation including nutritional care plan, assessments, consultations, nutrient intake analyses and patient/client education in all required documentation tools in a timely, professional, and thorough manner.
- Provides and assesses effectiveness of patient/client and family education. Develops, evaluates, and utilizes appropriate education materials. Learning needs are identified using methods appropriate to each learner's developmental and educational level; evaluates and documents learner response to teaching.
- Provides/develops periodic in-services on selected topics to team members and other disciplines as requested. In-service development for residents and attending physicians is supervised by clinical manager or designee.
- Under the guidance of the manager or designee, participates in basic skills orientation, training, and mentoring of team members and dietetic interns as applicable.
- Teaches and disseminates results and findings of nutrition research to medical team members, nursing, pharmacy, patients, and public.
- Communicates with multidisciplinary departments on patient/client care and issues, including performance feedback on personnel when requested or applicable to ensure standards of excellence are met for customer satisfaction.
Physical Requirements
Must be able to give and receive verbal communications on a continuous basis. Must be able to read written communications on a continuous basis both hard copy and on a computer screen. Able to compose written communications frequently. Able to lift and carry objects weighing 25 pounds or less occasionally. Able to perform motor skills as reaching out/up. Frequent periods of walking, standing, sitting. Infrequently squatting, twisting, and bending.

Education

Certification / Registration / License

Major Responsibilities
- Age-Appropriate Patient Care: Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his/her age-specific needs, and to provide the care needed as described in the department’s policies and procedures. - Patient Evaluation and Care Planning: Interprets medical records and provider referrals as appropriate, and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods. - Maintains designated applications in research department databases.
- Creates new databases, prepares and runs data analysis queries, and uploads local database to the national database center.
- Uses existing database to create reports as requested.
- Performs data manipulation and migration.
- Reviews data quality reports for data submissions and errors.
- Ensures data requests are received, have authorizations, are evaluated, and reported.
- Prepares specifications and summary reports for data queries and requests.
- Performs chart abstraction.
- Provides administrative support when needed.
Physical Requirements
Work consistently requires sitting for 5-6 hours. Intact senses of sight, hearing, smell, touch, and manual dexterity. Each employee has the responsibility to work in a safe manner.
Education, Experience and Certifications
Bachelor's Degree in Computer Science or health-related field required but Associates Degree and equivalent work experience will be considered. Training in information systems technologies, desktop application, and databases required. Experience with analytical software and database analysis and reports preferred.

Performs with minimal supervision radiographic examinations under the direction of the Radiologist. Coordinates daily activities including performing examinations and associated clerical duties.
Major Responsibilities
Performs and assists with radiographic and fluoroscopic procedures.
Documents patient history pertaining to specific procedure on requisition submitted by physician.
Confirms patient identification by verbal confirmation and validating proof of identity.
Prepares and administers contrast media as required.
Consults with radiologist and other physicians as necessary.
Selects and operates equipment as dictated by demands of individual procedure and demonstrates correct and adequate anatomic positioning skills.
Maintains appropriate records as required by departmental standards.
Minimum Job Requirements
Education
Graduate of an institution accredited by an agency that ARRT recognizes in Radiologic Technology.
Certification / License
Must be registry eligible with registry completion within 1 year of hire date. CPR certification required.

Knowledge / Skills / Abilities
Demonstrates knowledge of the principles of growth and development and demonstrates the skills and competency appropriate to the ages, culture, developmental stages, and special needs of the patient population served.
Physical Requirements and Working Conditions
Requires strenuous physical effort while assisting patients from wheelchairs and stretchers. Handles equipment and monitoring supplies. Must have good oral and written communication skills to interact with patients and hospital staff. May be required to frequently stretch, bend, twist, squat or kneel in operating radiographic equipment. Must be able to use hands for power gripping, speed work and precision during manipulation of radiographic equipment and in the case of injured and diseased patients. Vision and hearing for distances of up to 10 feet is necessary. May be exposed to various physical conditions throughout the workday such as walking on hard surfaces, climbing stairs, and being near moving equipment. Able to adapt to changes in temperature and humidity.
