Hours: Monday to Friday- Business hours
Overview
Plans and coordinates education and care of patients/persons with diabetes across a continuum in collaboration with other disciplines, community agencies and Sentara subsidiaries. Provides staff education, educates/counsels patients to promote wellness and prevention of complications in persons with diabetes. Serves as a clinical resource person for the clinical staff serving the inpatient and/or outpatient population.
Sentara Martha Jefferson Hospital is a DEAP accredited facility. The Diabetes and Nutrition Educator can expect between 6 and 10 patients per day. A mix of inpatient and outpatient and cross training for coverage for both areas is expected. This role will support the DEAP program accreditation

Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.

Intermediate Care Unit - Cornell 1: The Telemetry/Stroke Unit at Sentara Martha Jefferson Hospital provides care for patients with cardiac conditions including heart failure, cardiac rhythm disturbances, newly implanted permanent pacemakers and defibrillators, post- coronary interventions, post-ablation, myocardial infarctions. Cornell 1 also cares for TIA, acute stroke, and post procedural neuro cases that stepdown from ICU. This 30-bed unit sees a ratio of 1:5 with mixed patient acuity levels and is well supported with additional patient care technicians, educators, and a strong leadership team.

The Registered Nurse Unit Coordinator demonstrates proficiency in nursing practice for assigned specialty area and provides clinical leadership for the delivery of nursing care in a department. About the department:
The noninvasive team at Sentara Martha Jefferson includes RN’s, Cardiovascular Technicians, and Sonographers seeing an average of 7 patients a day both inpatient and outpatient. The Echocardiographer must live within a 30-minute drive of the hospital for call purposes.
Hours: Monday – Friday, day shift, with call every 4th week to include the weekend. Either 7 to 330p or 8a to 430p.
The noninvasive Echocardiographer performs two dimensional echocardiograms, Doppler echocardiograms, transesophageal echocardiograms and stress echocardiograms to evaluate heart and vessel abnormalities for the evaluation of heart disease and blood flow abnormalities. Completes the technologist report in the structured report application.
Education

Hours 7PM to 7AM with weekend rotation.
This role is open to New Graduate candidates with Heavy ICU clinical rotations or Capstone/Preceptorship or those who have worked as a Patient Care Technician or similar in an ICU. Otherwise, 2 years of acute care, critical care experience is highly preferred.

ICU: Intensive Care Unit
This is a mixed Critical Care (ICU) 15 bed and Step down (IMCU) 5 bed unit.
The unit has 15 private rooms with Phillips monitoring and state of the art Hil Rom technology with low air loss, continual lateral rotation, and percussion capabilities. We serve a broad range of patient conditions and provide services for neurology, cardiology, gastroenterology, oncology, nephrology, and surgical patients. We also have a phenomenal palliative care team that helps guide our nurses with that special touch at end of life. We have critical care NP/PA on sight 24/7 and have daily interdisciplinary rounds where the nurses provide valuable input and add insight to their patient’s care. Registered nurses can expect a 1:1 or 1:2 ratio depending on patient needs and a 12-hour shift schedule with rotating weekends.
Sentara Martha Jefferson Hospital is a primary plus stroke center. We provide care based on the American Stroke Association (ASA) treatment guidelines for the early management of adults with ischemic or hemorrhagic stroke. The Registered Nurse (RN) - New Graduate delivers nursing and patient care during the first year of practice while transitioning from academic preparation to independent professional practice. Working under the guidance of preceptors and experienced registered nurses and supported by the organization's nurse residency and onboarding program, this role assesses patients, delivers care, administers medications, and documents in the electronic health record within a growing scope of competency. MINIMUM REQUIRED EDUCATION

The specialist safeguards and protect the patient's right to privacy by ensuring that only authorized individuals have access to the patient's medical information and that all releases of information are in compliance with the request, authorization, company policy and HIPAA regulations.
Major Responsibilities:
- Ensures, safeguards, and protects patients right to privacy by staying current and knowledgeable of all Federal and State Regulations and organization policies regarding privacy, confidentiality, and release of information pertaining to continuing care, insurance, patient, and social security disability requests.
- Receives requests, validates that the authorization meets legal requirements, and identifies requester type. Ensures all releases of information are in compliant with the request, authorization, policies, and State and Federal HIPAA regulations and guidelines to safeguard the organization from legal and government repercussions.
- Logs requests into the Release of Information tracking system utilizing OnBase and EPIC. Scans the authorization into the release module according to documented process.
