Summary
The Pharmacy Care Coordinator acts as an extension of the ambulatory clinic team to support patient access to high touch, complex medication therapies. Works in collaboration with insurance companies, nursing, social workers, and pharmacy to obtain appropriate authorization for medications includingregistration with drug specific manufacturer programs when applicable. Performs tasks related to prescription billing including completing benefits investigations, assisting with the prior authorization process, and locating copay assistance options for patients to ensure affordability of specialty medications.
The Pharmacy Care Coordinator escalates clinical support needs and questions to the pharmacist. Assists with clinic intervention outcomes tracking to monitor value of services. Coordinates patient outreach during clinic visits or via telephonic follow up to ensure continuity of care and optimize patient outcomes. Responsibilities may include working to establish and maintain short- and long-term quality improvement goals.

Baptist Health is searching for a Specialty Pharmacy Care Coordinator to join our Cardiology team at Baptist Health Hardin, Elizabethtown, KY!
Completes benefit investigations to verify patient eligibility for prescribed specialty medications.
Maintains current knowledge of payer formularies and reimbursement.
Engages in personal development to stay updated with pharmacy trends related to role and practice setting.
Supports third party prior authorization process and obtain appropriate overrides to improve speed to therapy for new patients.
Maintains accurate insurance information and monitor for prior authorization renewal needs to avoid gaps in medication therapy
Triages patient or caregiver clinical questions, medication related concerns, and third-party insurance denials to the pharmacist for intervention as appropriate.
Utilizes Electronic Health Record to maintain communication between departments and ensure accurate documentation of patient care activities.
Communicates and represents pharmacy effectively throughout all levels of the organization.
Serves as the primary point of contact with patients related to specialty medication needs and shows empathy and compassion to patients and caregivers.
Serves as a liaison with physicians, nurse practitioners, physician assistants, pharmacists, nurses, and social workers to provide administrative support, communicate changes in medication related coverage and regulatory mandates.
Facilitates enrollment of patients in patient assistance programs to maximize copay savings and assist with gathering appropriate documentation.
Utilizes technology to organize daily outreach tasks to complete prescription refills, delivery coordination, and claim processing on time.
Minimum Qualifications
High school diploma or equivalent.
Registration with Kentucky Board of Pharmacy upon hire date.
Pharmacy technician experience required.
Prior experience with third party billing, financial counseling, or medical terminology and coding preferred.
Prior experience with Epic electronic medical record preferred.

Performs all functions and duties with the patient at the center, ensuring prompt and friendly service in the Community Pharmacy.
Performs all tasks associated with the filling and dispensing of community prescriptions, including computer order entry, updating patient information, filling and labeling prescriptions, and completing transactions at the point of sale.
Delivery of medications to discharging patients per Pharmacist’s requests.
Completes prescriber outreach for prescription refills under direct pharmacist supervision.
Assists with scheduling and coordinating home delivery for prescriptions.
Assists in inventory management processes, including order review, inventory returns, restocking shelves, and physical inventory preparation.
Ability to prioritize workflow by patient impact and ensure that all assigned tasks are completed at the end of the shift.
Performs simple nonsterile compounding per current USP<795> guidelines and as permitted by state law under the direct supervision of a pharmacist.
Assists in training new staff when assigned.
Escalates all identified problems or patient complaints, if they cannot be corrected by the employee, to the Pharmacist.

Minimum Requirements
High school diploma or equivalency is required.
Previous retail pharmacy experience preferred.

Provides patient care within the medical director approved department protocols and state scope of practice.

Performs ambulance transportation services within the scope of practice of a Paramedic.

MINIMUM QUALIFICATIONS
Certificate or diploma from an approved/accredited Radiologic Technology programs. Requires national certification from the American Registry of Radiologic Technologists (ARRT), Nuclear Medicine Technology Certification Board (NMTCB), or American Registry for Diagnostic Medical Sonography (ARDMS) based on modalities in which they will be conducting work and licensure by the state regulatory agency as applicable. As of August 1, 2019, all Multi-Modality Technologist's hired on or after August 1,2019 will be required to have an advance certification by an accrediting body in a specialty if applicable. This position requires clinical knowledge typically achieved with 2 years of Radiologic Technologist experience. Must be actively working in each certified modality, in accordance with the needs of the facility. The Multi-Modality Technologist II will be required to rotate and cover department scheduling as needed and required in areas where obtained certifications apply. Multi-Modality Technol II Requires certification in at least two (2) of the following modalities, or one certification from Level I and one certification from Level II: Ultrasound, Magnetic Resonance Imaging (MRI), Nuclear Medicine (CNMT), Position Emission Tomography (PET).

Estimated Pay Range:
$39.00 - $65.00 / hour
