We also provide nutritional counseling, palliative care, psychological support, and access to support services. Patient Access describes a multi-disciplinary function that is carried out at the point of service areas of the medical office.Patient Access Representativesperform front office duties under the general direction of the Clinical/Practice manager or designee.

Outpatient Imaging Coordinator
The Outpatient Imaging Coordinator provides leadership and operational oversight for multiple outpatient imaging locations, including Middletown Diagnostic Center, Eastpoint Diagnostic Center, PET Center, Dutchmans MRI, and the MRI Department at Baptist Health Louisville. This role is responsible for coordinating daily workflow, optimizing patient scheduling, and ensuring efficient department operations while maintaining high standards of patient care, quality, and safety. The Coordinator supervises technologists, registration, and ancillary staff through coaching, hiring, training, performance evaluations, competency assessments, and staff scheduling. Additional responsibilities include managing timecards, resolving patient and staff concerns, ensuring equipment functionality, maintaining regulatory compliance, overseeing quality initiatives such as ACR and MQSA requirements, monitoring supplies and linens, supporting radiologists during procedures, and participating in service promotion and department improvement efforts. The ideal candidate is a strong leader with excellent communication, organizational, and problem-solving skills who is committed to delivering an exceptional patient experience and fostering a positive team environment.
Requirements:
Graduate from an approved imaging program with licensure in at least one area of Radiology.
Management experience preferred.
Must have current CPR certification within 90 days of hire.

Job Description:
Responsible for the coordination of the care and management of the referrals into advanced organ disease programs and pre-transplant candidate. This includes the initiation and completion of the evaluation, coordination, and timing of testing to determine candidacy for transplant or advanced therapies. Must be able to function with considerable independence in the management of the referrals and pre-transplant patient.
Responsible for promoting the health and safety by ensuring the ongoing communication and collaboration providers and other multidisciplinary team members.
- Coordinates referrals and follow-up evaluation of each patient. Collaborates with the multidisciplinary to assure appropriate evaluation per protocol and policies.Is educated and understands general management protocols for organ specific evaluation. Schedule laboratory tests, X rays, biopsy, etc. according to the established protocols. Monitors laboratory results for each patient and knows normal values for the labs, follows patient trends. If abnormal result or trend noted, coordinator responds per protocol or notifies transplant provider of abnormalities and coordinators follow up as directed.Promptly notifies patient (and consultants and other treaters) when patient care issue notedPhone triage of patient calls and MD calls with ongoing education to patients, shares information regarding protocols and general knowledge with PCP’s or other community treaters.Communicates with other team members problems that are occurring so that continuity of care is maintained.Communicates the results of lab tests and biopsies to the patient and their families.Communicates on an ongoing basis with patients. Documents in Transplant Data Base and appropriate hospital and clinic EMR’s conversations with patient, uses To-Do function.Works with multidisciplinary team members in assuring that the psychosocial and financial needs of the patient are being addressed.
Maintains accurate and current data base on each patient recording all laboratory, diagnostic tests, biopsy results and changes in medications.
- Staffs the outpatient transplant clinic.Schedules all clinic visits according to established protocols and the needs of the patients.Staffs the outpatient transplant clinic with providers and other members of multidisciplinary team, prepares nursing assessment, prepares appropriate documents and reports for clinic and documents clearly each encounter when patient seen in clinic (changes to medications, procedures to be scheduled for follow up on, laboratory schedules changes, etc.).Schedules all clinic visits according to established protocols and the needs of the patients.Discuss findings with the transplant provider and appropriate members of multidisciplinary team and carries out the follow-up that is indicated.
- Maintain compliance with all company policies, procedures and standards of conduct.
- Active involvement in program specific quality and selection meetings, protocol development, and educational programs for community, patients, and staff.

Following physician evaluation, the Clinical Coordinator facilitates implementation of the diagnostic plan by coordinating ancillary studies, consultations, pathology documentation, and case progression through final sign-out. The position serves in a physician support role requiring extensive knowledge of anatomic pathology workflows, pathology information systems, medical terminology, and documentation standards to promote efficient workflow, timely diagnosis, regulatory compliance, and high-quality patient care.
Essential Functions:
- Review daily case assignments and prepare neuropathology, ophthalmic pathology, and heart transplant biopsy cases for physician review.
- Retrieve, organize, and synthesize pertinent clinical history, operative reports, radiologic findings, laboratory results, and prior pathology material from the electronic medical record and laboratory information system to support diagnostic evaluation.
- Verify specimen site, procedure, laterality, and other relevant clinical information to ensure complete case preparation.
- Identify prior pathology specimens requiring comparison and coordinate retrieval of archival slides, tissue blocks, reports, and consultation materials.
- Maintain daily case tracking utilizing departmental databases and workflow management systems, including Vantage and the Neuropathology Service Log, and communicate case readiness and workload status to the pathologist.
- Following physician review, facilitate implementation of the diagnostic plan by preparing diagnosis-specific report templates, coordinating immunohistochemical stains, special stains, molecular studies, and other ancillary testing as directed by the pathologist.
- Coordinate intradepartmental and external pathology consultations, including preparation of consultation materials, tracking pending studies, and incorporation of consultation results into the case record.
- Monitor case progression to ensure timely completion of ancillary studies, pending addenda, consultations, and other outstanding diagnostic activities.
- Prepare documentation necessary for professional pathology reporting, including standardized templates, required documentation elements, and CPT code selection support, under physician direction.
- Coordinate peer review assignments and maintain associated case documentation.
- Prepare pathology summaries, supporting documentation, and case materials for multidisciplinary conferences, including Neuro-Oncology Tumor Board, Pituitary Conference, and Heart Complex Case Review.
- Monitor turnaround times and proactively identify workflow barriers that may delay case completion, communicating issues to the pathologist and laboratory leadership as appropriate.

- Communicate with provider offices and in-network pharmacies regarding patient care and various clinical specialty pharmacy initiatives

What You’ll Do
