What Registered Nurse RN will need:
A Degree from an accredited school/college of nursing is required - BSN or ADN
BLS Provider (accredited by the American Heart Association) required prior to hire
Illinois Board of Nursing license prior to hire
Clinical experience in a PICU setting
Compensation:
BSN Pay Range: $36.05 - $59.92 per hour
ADN Pay Range: $32.97 - $57.06 per hour
Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.

Assesses
the educational needs of patients.
Contacts
the previous educational setting, (providing an adequate release is available),
and/or reviews all available written records.
Assesses,
supervises or approves the educational needs of the patient before the
Diagnostic Conference date.
Prepares
assessment in adherence to established departmental format.

daily lesson plans consistent with evaluations and /or IEP’s and overall

Reports
to work on time prepared for daily work assignments.

in advance for special days off; does not abuse time.

Position Purpose
Functions as a member of the health care team under the direction of the RN. Utilizing a Patient Focused Care approach, provides sitting services to a specific group of patients within the St. Joseph Mercy Oakland System. Actively supports and participates in the activities of the unit based Shared Governance Council. Promotes organizational commitment to customer service.
Duties/Responsibilities
- Performs activities of daily living (ADLs) appropriate to the patient's condition, according to unit routine under direction of the RN. This would include physical care such as bathing, turning, ambulating patients and toileting.
- Provides basic nutritional support to patients such as tray passing, tray setup, assistance with feeding, and communication of intake to RN.
- Demonstrates principles of Relationship-based Care to cultivate and maintain therapeutic relationship as a means to optimal healing and collegial functioning as an interdisciplinary care team member.
- Provides observation following the guidelines as described during department specific orientation. Must ensure they are actively participating in a distraction free environment.
- Reports patient care incidents and changes in the patient's condition to the RN in a timely fashion.
- Creates a neat and clean environment for optimal patient care experience.
- To ensure safe patient care experience, incumbent must remain with patient until hand-off occurs. Seeks and incorporates feedback concerning improvement in one's own practice, including participation in self-appraisal.
- Provides patient care that reflects respect for patient's rights, dignity, values, culture preferences, and expressed needs and attention to providing comfort, relieving pain, and alleviating anxiety.
- Takes personal accountability to seek out experiences in order to maintain skills and competence.

Working Conditions
- Working in the patient care environment may result in exposure to communicable diseases and hazardous substances.
- Position may require frequent lifting and carrying of patients/equipment/other not to exceed 51 pounds. Associates will not lift items/patients exceeding 35 pounds from the floor.
- Associate’s position requires occasional (5-10% of work time) squatting, crawling, crouching, kneeling, and balancing, frequent bending/stooping, and reaching above shoulder level, pushing/pulling.

Minimum Qualifications
- Current license to practice as a Registered Nurse in state of practice. Valid compact licenses must be transferred to the state of practice within 60 days of the start of employment.
- RNs with less than 12 months of experience must obtain their Bachelor of Science in Nursing (BSN) within four (4) years of their hire date.
- Basic Life Support Certification (BLS) for healthcare providers.

Physical Requirements
- Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

MMARY:
Under supervision of the Manager Laboratory Outreach Services or guidance of the Lead Phlebotomist, performs responsibilities associated with obtaining blood and various other patient specimens in accordance with department procedures. Schedules venipunctures or capillary blood punctures according to priority and type of test, such as timed studies. Phlebotomy certificate through National Accrediting Agency for Clinical Laboratory Sciences and/or completion of a Phlebotomy certificate program to include formal internship demonstrating 100 successful venipunctures.
In lieu of internship, 6 months of phlebotomy experience may be considered.
In lieu of phlebotomy certification, 5 plus years direct phlebotomy experience may be considered.
Ability to perform venipuncture techniques and capillary puncture on adults, children and infants.
Interpersonal skills to obtain and provide information to patients and to explain procedures effectively in order to allay fears and anxiety.
Demonstrated knowledge and skills necessary to provide care patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
Clerical skills to prepare and maintain records and reports, including keyboard skills to enter access information.
Familiarity with blood collector equipment, anticoagulants, special precautions and interference.
Ability to distinguish chromatic colors.
A Phlebotomy certificate required.
WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:
Works with in-patient care environment with considerable walking between units and where some physical effort may be required to deal with patients who are uncooperative.
Some exposure to odors when handling specimens.
Exposure to communicable diseases while administering to patients, however chance of injury is minimized by the adherence to established infection control procedures.
Manual dexterity to perform venipunctures.

Eddy Senior Care PACE (Program of All-Inclusive Care for the Elderly) helps older adults remain safely in their homes and communities by providing comprehensive medical, social, and supportive services. As our program continues to grow, we are expanding our team to ensure every prospective participant receives timely, thoughtful, and informed support.
Position Overview
We are seeking a highly organized and personable Inquiry & Intake Support Specialist to serve as the first point of contact for prospective participants and families. This role plays a critical part in ensuring that inquiries are efficiently screened, documented, and routed, allowing our Intake/Enrollment Specialists to focus on the most qualified leads and move candidates smoothly through the enrollment process. In addition, this position provides administrative and marketing support to the Community Liaison team and PACE program as a whole
Key Responsibilities
• Serve as the initial contact for incoming inquiries (phone, message, referral)
• Gather key information to help determine basic eligibility and level of interest
• Document and track all inquiries (qualified and non-qualified) accurately and timely
• Route qualified leads to the appropriate Intake/Enrollment Specialist for follow-up
• Provide clear, professional information about the PACE program to callers and families
• Maintain consistent communication and a positive experience for all prospective participants Administrative & Marketing Support
• Prepare and assemble marketing packets, folders, and outreach materials
• Support Community Liaison staff with event preparation and logistics
• Assist with data entry, tracking logs, and reporting needs
• Provide general administrative support to the intake/enrollment function

- Collects data pertinent to the healthcare consumer's health or the situation from patient assessments in the home setting, telephonic contacts and/or related EMR information available.
- Analyzes the data to determine the issues and available resources to seek effective resolution. Identifies patients at high risk for re-hospitalization, falls and/or high utilization of resources that require additional intervention and case conferencing to ensure appropriate care and frequency of services mitigates risk and ensure high quality outcomes
- Prioritizes multi-issue, situations appropriate to the criticality of patient health and welfare, patient/staff safety and impact to effective, timely services
