Position Purpose
We are hiring Pharmacists to support operations and our expanded emergency pharmacy program! We are expanding our shift coverage to include a decentralized pharmacist. We are searching for per diem help to cover this role as well as centralized distribution. This position is for pharmacists with experience in hospital pharmacy who are able to work directly with patients for medication histories, counseling, and responding to emergencies within the hospital.
As a Pharmacist, you will:
Appropriately assesses patient data to identify medication-related problems through profile reviews, pharmacy consults and interdisciplinary rounding.
Participates in medical emergencies and other formal alerts such as trauma, stroke, sepsis and STEMI as required.
Reviews and interprets providers orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, and comorbidities. Checks for drug incompatibilities and allergies, and takes appropriate action when necessary. Updates medication history for all admitted patients.
As necessary, consult with providers, nurses and other healthcare professionals involved in patient care regarding pharmacotherapy optimization with intent to achieve optimal patient outcomes across the continuum of care.
Provides accurate and timely responses to other healthcare professionals and patients regarding drug information questions and facility medication use policies to assure safe and appropriate use of medications.
Documents patient care activities including therapy plan, monitoring, and follow-up in electronic medical record as per departmental guidelines.
Minimum Qualifications:
Pharmacy degree (Pharm.D.) from an ACPE approved college of pharmacy required.
Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital experience is required.
Registered pharmacist in Pennsylvania required.
Preferred: Experience with Pyxis MedStation, EPIC
BLS and ACLS certification before completion of orientation (will be provided if cannot provide upon hire)

WVUHS and the Department of Emergency Medicine seeks an Advanced Practice Provider (PA or NP) to practice at Potomac Valley Hospital.

Duties: The successful candidate will practice in the areas of Emergency Medicine. In addition to providing comprehensive outpatient clinical care to patients in an emergency medicine capacity and establish preventive health practices for patients.
Qualifications: Candidate must have a PA or NP degree and be eligible for state medical license. All qualifications must be met by the time of appointment.
MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing.
2. National Certification by one of the recognized APRN Certification Centers.
3. Licensure, or eligibility for licensure, as a professional Registered Nurse and Advanced Practice Registered Nurse in Pennsylvania.
4. CNOR Certification required.
5. BLS Certification required with additional Advanced Life Support training at the discretion of the department.
EXPERIENCE:
1.Emergency medicine/ Urgent care experience preferred.
2. Eligible new graduates will be considered.

- Collects, documents and delivers lab specimens; obtains lab results and other reports and enters EMR if needed.
- Notifying patients of test results and follow-up appointments. Refers to RN/MD for patient education.
- Reviews with patient/family medications, treatment and after visit instructions.
- Responds to and screens telephone calls for referral to RN or physician.
- Coordinates pre-visit planning by reviewing the EMR to include patient history and proactively order labs and other tests based on approved clinic protocols.
- Completes necessary forms (i.e. FMLA, DME, Home Health orders) and obtains physician signatures as appropriate.
- Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

POSITION SUMMARY

CORE FUNCTIONS
1. 3. Triages adverse events and communicates with the research physician regarding the occurrence of adverse events. Collects information regarding indicated medical follow-up with the subject and collateral informants as needed.
4. Provides educational information to patients and their families. Serves as a resource and support person for patients and families by being a consistent person they can contact within the clinical research program. Provides information to the patient and family related to the healthcare of the patient such as health maintenance, prevention, and anticipatory guidance relevant to study-related diseases.
5. Ensures uniform implementation of protocols by completing required documentation in a consistent, accurate and timely manner. Assists in preparing appropriate IRB (Institutional Review Board) forms, informed consents, correspondence with sponsor, etc., in an accurate and timely manner. Principal Accountabilities
Provides oversight, problem-solving and support in collaboration with hospital administration.

Collaborates with department managers/directors to give support to the operations of the facility.
Collaborates with the Chief Nursing Officer in meeting the goals, objectives and mission of the Nursing Division.
Supports staffing procedures, patient classification, and supplemental staffing of nursing services.
Initiates conflict resolution among hospital personnel, physicians, patients and/or visitors and refers these problems to the responsible director.
Assumes responsibility as the hospital representative in dealing with patient transfers, the media and acquisition of equipment.
Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
