Nurse Practitioner (NP) | Community-Based Primary Care | Southern MN
Twin Cities Physicians View all jobs
Posted 1 month(s) ago
Reference: 2312316
*Company:* Twin Cities Physicians
*Location:* Southern Minnesota | Owatonna, Rochester, Albert Lea, and Surrounding Communities
*Compensation:* *$120,000 - $160,000+ Total Compensation* (Base Salary + Productivity Incentive Program) + Mileage Reimbursement
*Benefits:* Comprehensive Benefits Package, PTO, Paid Holidays, CME Allowance, Malpractice Coverage, DEA Reimbursement, Clinical Resources, and More
*Schedule:* Full-Time or Part-Time | Community-Based Rounding
*To Stand Out*
Email Tom Lecy at *[email protected]* with your resume and tell us:
* About your clinical background.
* Which patient population you most enjoy serving.
* Why *community-based senior medicine* appeals to you.
*Your Practice Doesn't Have to Be Confined to Four Walls.*
You finish seeing one of your longtime patients, update the nursing team on her plan of care, grab your things, and head to your next community.
There's no waiting room full of patients watching the clock.
No spending the entire day in the same exam room.
Instead, you're out in the community practicing medicine where your patients actually live.
At Twin Cities Physicians, we're looking for a Nurse Practitioner who wants to build relationships with older adults throughout *Owatonna, Rochester, Albert Lea, and surrounding Southern Minnesota communities*.
You'll have the independence to manage your patients, the variety of practicing across multiple communities, and the backing of an experienced *physician-led clinical team* when you need it.
For an NP who enjoys *autonomy, geriatrics, and getting out into the community*, this can be an incredibly rewarding way to practice.
*Trade the Traditional Clinic for Community-Based Medicine*
Your patients shouldn't always have to come to you.
Sometimes better healthcare means *going to them*.
You'll provide primary care directly within *assisted living, memory care, independent living, and skilled nursing communities*, caring for older adults with a wide range of clinical needs.
You'll manage chronic conditions, address acute concerns, follow patients after hospitalizations, and work closely with the nurses and caregivers who know them best.
And because you'll follow many of these patients over time, you'll have something that's becoming harder to find in healthcare:
*Continuity.*
You'll know your patients. They'll know you.
*Why Nurse Practitioners Choose Twin Cities Physicians*
* *$120,000 - $160,000+ total compensation.*
* Productivity incentive program with additional earning potential.
* *Mileage reimbursement.*
* Full-Time or Part-Time opportunities.
* Practice at the *top of your license*.
* High degree of *clinical autonomy*.
* Variety without being confined to the same clinic every day.
* Strong physician collaboration and clinical support.
* Opportunity to build meaningful relationships with *older adults and senior living teams*.
* Comprehensive benefits package.
* Paid Time Off and *Paid Holidays*.
* CME Allowance.
* Malpractice Coverage.
* DEA Reimbursement.
* Clinical resources and administrative support.
*Your Office Is the Community*
One day may take you into a memory care community in Owatonna.
Another may include patients around Rochester.
Another may bring you farther south toward Albert Lea.
Wherever you're rounding, your purpose remains the same: *bring proactive medical care directly to seniors who benefit from having a consistent provider where they live*.
You'll work alongside nurses, caregivers, therapists, pharmacists, families, and facility leadership to understand the complete picture surrounding each patient.
That can mean catching a subtle change before it becomes an emergency, helping stabilize a chronic condition, or making sure a hospital discharge plan actually works once the patient returns home.
Your work helps *strengthen continuity of care, reduce unnecessary hospitalizations, and keep older adults healthier where they call home*.
*What You'll Be Doing*
* Provide comprehensive primary care throughout *senior living and long-term care communities*.
* Travel throughout *Owatonna, Rochester, Albert Lea, and surrounding Southern Minnesota communities*.
* Diagnose and manage *acute and chronic medical conditions*.
* Perform comprehensive assessments, preventative care, and routine follow-up visits.
* Develop individualized treatment plans.
* Manage chronic disease and monitor changes in condition.
* Coordinate care following hospitalizations, rehabilitation stays, and specialist visits.
* Collaborate with *physicians, nurses, therapists, pharmacists, caregivers, and senior living teams*.
* Educate patients and families regarding diagnoses, medications, and treatment plans.
* Help improve outcomes through *proactive and preventative medicine*.
* Maintain accurate and timely clinical documentation.
*Clinical Backgrounds That Translate Well*
Experience in any of the following areas is highly valued:
* Primary Care.
* Family Practice.
* Internal Medicine.
* *Geriatrics.*
* Long-Term Care.
* Skilled Nursing.
* Post-Acute Care.
* Hospital Medicine.
* Home Health.
* Hospice & Palliative Care.
You don't need to come directly from senior living. We're especially interested in NPs who genuinely *enjoy caring for older adults* and want the opportunity to build longer-term patient relationships.
*What We're Looking For*
*Required Qualifications*
* Active Minnesota Nurse Practitioner license.
* Excellent assessment, diagnostic, and *clinical decision-making skills*.
* Ability to practice independently while collaborating with physicians.
* Strong communication and care coordination skills.
* *Passion for serving older adults.*
* Comfortable managing medically complex patients.
* Reliable transportation and willingness to travel throughout *Southern Minnesota*.
*Preferred Qualifications*
* Experience caring for *medically complex older adults*.
* Chronic disease management expertise.
* Community-based or senior care experience.
* Assisted living, memory care, or skilled nursing experience.
* Advanced wound care experience.
* *Wound Care Certification (WCC)* is a plus.
*More Freedom. More Variety. More Connection.*
Some providers thrive inside a traditional clinic.
Others want something different.
They want to *get out into the community*, manage their own patient population, work with different clinical teams, and build relationships with patients beyond a single appointment.
They want independence without practicing alone.
Most importantly, they want to feel connected to the people they're caring for.
If that sounds like the way you want to practice medicine, *Southern Minnesota needs providers like you*.
*About Twin Cities Physicians*
Twin Cities Physicians is a growing, *physician-led medical group* providing proactive primary care services to thousands of seniors throughout Minnesota.
Our providers bring healthcare directly into senior living communities, helping improve *access, continuity of care, quality of life, and patient outcomes while reducing unnecessary hospitalizations*.
If you're ready to *leave the traditional clinic behind, get into the community, and build a practice around the older adults who need you*, we'd love to meet you.
Pay: $120,000.00 - $160,000.00 per year
Benefits:
* 401(k)
* 401(k) matching
* Dental insurance
* Flexible spending account
* Health insurance
* Health savings account

We are seeking a *Sp*eech-Language Pathologist who wants to use their skills where they can make a meaningful difference. This position is based at our Gladwin location and will serve children and adults with disabilities in an underserved community where access to specialized services can be limited.
The position will part-time basis, with the opportunity to grow into a full-time position if desired as services and referrals expand. This is an opportunity for the right professional to help us build something special while maintaining flexibility and balance.
The Opportunity
The Speech-Language Pathologist will provide evaluation, treatment, consultation, and support to children and adults experiencing communication, language, social communication, cognitive-communication, and/or swallowing needs.
But this position is about more than providing therapy.
At the Arnold Center, our work happens in the context of real lives, real goals, and real communities. An SLP may help a child find a way to communicate a need, help an adult become more independent in the community or workplace, collaborate with a family who has struggled to access services, or help a team discover a new way to support someone successfully.
We are looking for an SLP who sees the person before the diagnosis and understands that sometimes the smallest communication breakthrough can open an entirely new world of possibilities.
Essential Responsibilities
* Conduct comprehensive speech, language, communication, cognitive-communication, social communication, and swallowing evaluations within the SLP's scope of practice.
* Develop individualized, person-centered treatment plans based on assessed needs, personal goals, family priorities, and functional outcomes.
* Provide individual and group speech-language therapy to children and adults with intellectual, developmental, neurological, and other disabilities.
* Support individuals who use or may benefit from Augmentative and Alternative Communication (AAC) and other assistive communication strategies.
* Incorporate communication goals into everyday environments and meaningful activities whenever possible.
* Collaborate with families, caregivers, Behavior Analysts, Behavioral Technicians, Direct Care Professionals, Service Coordinators, educators, healthcare professionals, and other members of the individual's support team.
* Provide education, consultation, and practical strategies to staff and caregivers so communication supports continue beyond the therapy session.
* Complete required evaluations, treatment plans, progress notes, reports, and other documentation accurately and within established timelines.
* Monitor progress and modify treatment approaches based on outcomes and individual needs.
* Participate in interdisciplinary meetings, person-centered planning, and care coordination as appropriate.
* Assist the Arnold Center in developing and expanding speech-language services throughout Gladwin County and surrounding underserved communities.
* Maintain services in accordance with professional standards, ethical requirements, payer requirements, Arnold Center policies, and applicable state and federal regulations.
Who We Hope to Find
We are looking for someone who is clinically skilled but also *warm, creative, flexible, and genuinely interested in people*.
The right person will understand that meaningful therapy doesn't always happen sitting across a table. Communication can be developed while preparing something in the kitchen, participating in a community activity, practicing workplace skills, playing, building relationships, or simply finding the right way for someone to express, _"This is what I want."_
We want someone who is excited about bringing high-quality services to a community where families and individuals may have historically faced limited access, long drives, waiting lists, or other barriers to receiving specialized care.
Qualifications
* Master's degree in Speech-Language Pathology from an accredited program.
* Current or eligible for Michigan licensure as a Speech-Language Pathologist.
* Certificate of Clinical Competence (CCC-SLP) through ASHA preferred.
* Clinical Fellows who meet applicable supervision and licensure requirements are encouraged to apply.
* Experience working with children and/or adults with intellectual or developmental disabilities preferred but not required.
* Experience with autism, AAC, social communication, or interdisciplinary services is a plus.
* Ability to successfully complete required background checks and organizational credentialing requirements.
* Strong communication, documentation, collaboration, and organizational skills.
* Valid driver's license and reliable transportation for any required community-based services.
Why the Arnold Center?
The Arnold Center has spent decades creating opportunities for individuals with disabilities to *work, live, learn, participate, and thrive*.
Our organization brings together clinical services, community living supports, employment services, skill development, and social enterprises to create something different from a traditional clinical setting. We can take skills learned in therapy and help individuals use them where they matter most—in their homes, workplaces, relationships, and communities.
And there is something else that is harder to put into a job description.
We call it *the Arnold Center magic*.
It happens when someone accomplishes something others thought they couldn't. It happens when a person finds their voice, earns their first paycheck, makes a friend, learns a new skill, becomes more independent, or simply realizes that they belong.
Our SLP will have the opportunity to be part of those moments.
A Position That Can Grow With You
We recognize that not every professional is looking for the same thing.
This position will begin part-time, allowing the successful candidate flexibility while helping us expand access to speech-language services in the Gladwin community. As the program grows, the position can develop into full-time employment if that is what the SLP desires.
For someone who enjoys building programs, there will also be an opportunity to help shape how speech-language services develop within the Arnold Center and the communities we serve.
Our Commitment
The Arnold Center is committed to creating communities where individuals of all abilities have opportunities to participate, contribute, communicate, and belong.
If you became a Speech-Language Pathologist because you wanted your work to matter—and you are looking for a place where you can know the people you serve, collaborate with a team that believes in possibilities, and help bring needed services to an underserved Michigan community—we would love to meet you.
