The Advanced Practice Professional (APP) provides high-quality, patient-centered care as a member of the interdisciplinary healthcare team. Working collaboratively with physicians, nurses, and other healthcare professionals, the APP assesses, diagnoses, treats, and manages patients within their scope of practice and area of specialty. The APP is responsible for delivering evidence-based care, promoting positive patient outcomes, maintaining professional and regulatory standards, and supporting the clinical, educational, and operational goals of the department and organization.Additional Job Description:
POSITION SUMMARY: TheAdvanced Practice Professional (APP) has coordination and clinical skillsrequiredfor the comprehensive management ofvascular surgerypatients (including biological,social,and psychological assessments). The APP coordinates involvement of medical specialty teams necessary to develop a comprehensive plan for thepatient andreviews the plan of care with the other members of the clinical team,participatesin directing that care, performs diagnostic/therapeutic procedures and coordinates discharge planning (as applicable). The APPparticipatesin the education of residents and medical/nursing students.
Provide healthcare services consistent with state law and the requirements of the professional licensing or certification authority and/or hospital privileges, where applicable. Sucha servicemayinclude butisnot limited to.
MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1.
Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing.
2.
National Certification by one of the recognized APRN Certification Centers.
3.
Licensureor eligibility for licensure as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
4.
Department Specific: APRNswho will beprivileged as First Assistsin the OR (as part of their role)are required to complete
RNFA certification.
5.
American Heart AssociationBLS Certification required with additional Advanced Life Support training at the discretion of the
department.

Manages comprehensive and individualized care forvascular surgerypatients and their families in the inpatient phase of hospitalization immediate post dischargeandin the ambulatory setting.
2.
Completes daily rounds; consults and/or pre-procedure/evaluations.
3.
Completes integrated assessments.
4.
Participates in the perioperative management of vascular surgery patients which mayinclude butis not limited to first assist in the operative suite.
5.
With the collaboration ofvascular surgeryfaculty or by following established clinical Guidelines and Protocols, orders additional tests or consults as deemed appropriate.
6.
Performs and communicates results of diagnostic tests following established clinical guidelines and scope of practice.
7.
Consults and communicates with clinic and hospital staff regarding abnormal findings in the evaluations of patients.
8.
Performs succinct comprehensive clinical evaluations and documents findings in medical record.
9.
Facilitates patient discharge by coordinating appropriate services and tests prior to discharge necessary for outpatient follow-up appointments in the clinic (as applicable).
10.
Develops a comprehensive individualized plan of care forvascular surgerypatients, which includes teaching, discharge planning and necessary referral to other members of the health care team as necessary.
11.
Demonstrates expertise in caring for patients across the age continuum of their educational experience and certification.
12.
Identifies unstable or complexvascular surgerypatient care situations and facilitates appropriate intervention in the clinic and/or hospital setting.
13.
Recognizes emergency situations and initiates effective emergency care.
14.
Evaluatesvascular surgerypatients and serves as a coordinator for follow up procedures and tests in the ambulatory setting (as applicable).
15.
Assists in the collection of outcome measures (as applicable).
16.
Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making by the identification,evaluation,and management of the individual needs of patients/families.
17.
Demonstrates the ability to utilize equipment in a safe manner.
18.
Bases decisions made/actions taken on the relevant scientific principles, established standards of care and/or research findings.

Utilizes teaching/learning theory and appropriate teaching strategies in the provision of education to nursing staff, hospital staff,students, patients,families,and the community.

Completes all identified clinical competencies.

Documents the plan of care,interventions,and evaluations in a clear and concise manner for every patient visit in the patient’s medical record following approved hospital/UHA policies and compliance standards for documentation.

Collaborates with other leaders and staff to effectively achieve patientoutcome-basedimprovements in care.

Provides leadership in the development and implantation of changes in thevascular surgerypractice that positively impact patient outcomes.
29.
Works collaboratively with medical and nursing staff to facilitate creative problem solving and provide professional support.

Participates and initiates inter-disciplinary approaches to patient care, program development and education. Serves as a professional role model.
31.
Promotes and practices innovation of the expanded role in the delivery of care tovascular surgerypatients and families throughout the inpatient hospitalization, and the ambulatory setting.

Documents hours and submits billing reports, as appropriate.

Reviews and interpretsvascular surgerydata from sources such as patient registries, clinical and financial reports, length of stay, access to care, and cost per case reports and recommends/implements changes as necessary.

Performs phlebotomy (the practice of entering a vein to draw blood) services on inpatients and outpatients, as requested by physicians, in support of the hospital's mission of patient care.MINIMUM QUALIFICATIONS:

1. Performs, in a timely manner, blood collection using established laboratory guidelines designed to acquire acceptable specimens used in diagnostic and therapeutic procedures to meet the needs of patients, as requested by the ordering provider.
2. Follows standard operating procedures to perform basic diagnostic procedures to produce accurate results.
3. Operates basic laboratory equipment (e.g., centrifuges, computer system, tube transport system, timers) consistent with standard operating procedures.
4. Restocks, as required, the work area of needed supplies to assist co‑workers in the performance of their duties.
5. Must demonstrate, after receiving instruction and training, knowledge of the patient care requirements for all age groups as required by the laboratory standard operating procedures.
6. Utilizes time, material, and resources in an economical manner, prioritizing and arranging workload to provide for optimal patient care and efficiency.
7. Promptly collects patient specimens, to reduce wastage of time, materials and resources, unnecessary requests for additional specimens, and undue delays in result reporting, in accordance with the standard operating procedure.
8. Completes assigned workload within established time frames by adjusting organization of workload, (e.g., daily monitoring, turnaround time studies).
9. Maintains effective communication with the laboratory sections, administrative office, coworkers, personnel, patients and other departments as necessary to assure identification of problems and provide problem resolution in support of the hospital's mission of patient care.

11. Participates in various activities (i.e., staff meetings, in services, health screens, etc.) and keeps current in all communications (e.g., reading circulated material, staff meetings, mailboxes, e-mail and procedures) incorporating new information into routine use.
12. Effectively communicates in a timely manner any unusual supply usage or critical supply level to the supervisor to expedite ordering of required supplies for the continual needs of patients.

1. Heavy/Hard work:Work requires strength and/or stamina, lifting, moving, stooping, reaching, positioning of patients, standing, walking; carrying of patients, materials and equipment weighing 40+lbs.This is performed on a daily basis.

SKILLS AND ABILITIES:
1. Must have the ability to work successfully under highly stressful conditions, and must be capable of adapting to varying workloads and work assignments on a constant basis.

3. Must be skilled with computer systems used by both physician offices and hospitals to capture data accurately and consistently.
4. Ability to function as a team member in respect to organization, communication, and task prioritization. Must also be able to work weekends, holidays and all shifts.
