JOB SUMMARYAssesses nutrition status of patients, including performance of a nutrition-focused physical exam, and identifies nutrition diagnoses. Develops, implements and evaluates outcomes of nutrition care plans for patients at nutrition risk. Initiates changes in care plans as needed. Provides nutrition education and counseling to patients, families, and the community. Trains dietetic interns and other health care students.

KEY RESPONSIBILITIES
- Assesses nutrition status, including performance of a nutrition-focused physical exam, and implements nutrition care plans to address identified nutrition diagnoses for patients with common medical conditions, using the Nutrition Care Process and standardized nutrition terminology in documentation.
- Identifies patients who meet the criteria for a medical diagnosis of malnutrition and documents criteria in the medical record.
- Writes, accepts, and implements medical nutrition therapy and nutrition orders in eSTAR, including diet and infant feeding orders and enteral nutrition orders, according to the VUMC Standard Operating Procedure and RD/LDN Scope of Practice.
- Monitors nutrition progress and reassesses nutrition status of patients/clients with common medical conditions, adjusting the nutrition care plan when needed.
- Provides nutrition counseling and education to patients, families and caregivers, and the community, using evidence-based nutrition practice guidelines and appropriate instructional methods and tools.
- Conducts nutrition telehealth, real-time HIPAA-compliant interactive audio and video telecommunications with patients, and monitors telehealth technologies for HIPAA compliance.
- Trains dietetic interns and other students by planning, implementing, supervising and evaluating clinical experiences and/or teaching classes.

• Evidence-Based Nutrition Practice (Intermediate): Demonstrates mastery of integrating nutrition evidence from multiple sources to determine if a practice change should occur. Vanderbilt University Medical Center’s Department of Nursing Strategic Programs is dedicated to serving the VUMC Health System. The Vanderbilt Medical Center Staffing Collaborative supports nurse staffing needs as a system-wide resource. The Staffing Collaborative has created a new structure that allows alternative nursing employment at VUMC. Like a travel nurse, the Staffing Collaborative will allow Nursing staff to work 13-week assignments within a specific unit at VUMC. The Department of Nursing Strategic Programs and the Staffing Collaborative provide a new and exciting approach to nursing in a patient-focused, safe, and efficient manner. The assignment comes with premium pay and optional health benefits.
The Staffing Collaborative's primary responsibility is to ensure coverage for the most significant scheduling needs across the organization. Assignments may necessitate working weekends and holidays. Minimum requirements will be consistent with the expectations for full-time staff within each specific unit.

6RW Medicine Cohort 10Job Summary:
The Clinical Staff Leader is a direct supervisory leadership role and a key member of the Nursing leadership team. The CSL leads a cohort of staff members and actively participates in overall management of the unit, development of clinical practice, and provision of patient care with accountabilities for implementation and outcomes for people leadership, quality, service, compliance, finance, throughput and scheduling. The primary focus of the Clinical Staff Leader is to ensure the unit's optimal performance by engaging in effective problem solving for staff, patients and families and to develop/lead the assigned cohort. The Clinical Staff Leader provides feedback and coaching to staff members, recognizing them for their contributions while fostering a high performance culture. The CSL works under the guidance and direction of the manager to develop..
SHIFT: The person in this position will work two 8-hour admin shifts and two 12-hour charge shifts on the night shift for a total of 40 hours per week.
ABOUT THE UNIT:
This is 16 bed unit with a nurse to patient ratio of 1:5-6. We care for adult patients age 18 and older who are being treated for various acute and co-morbid conditions including respiratory, gastrointestinal, renal, neurological, psychiatric, cardiac, or infectious diseases.

Vanderbilt Clarksville Intensive Care Unit (ICU), is a 21 bed mixed medical/surgical adult ICU specialized in providing close monitoring and treatment for patients with a wide-range of diagnosis. We care for patients being treated for Cardiovascular conditions, Acute Respiratory Failure, Acute Respiratory Distress Syndrome (ARDS), Multisystem Organ System Failure, Sepsis, Drug Overdoses, Gastrointestinal Bleeds, Diabetic Ketoacidosis, Hypertensive Crisis, Acute/Chronic Renal Failure, STEMI, cardiogenic shock and post heart catheterization.
Our staff is committed to provide exceptional care to our patients. Our environment is challenging, team oriented, patient and family centered. We work collaboratively to provide opportunities for nurses to grow in critical thinking, professionalism, and participate in ongoing education.
Our team consists of Intensivists, advanced practice nurses, critical care nurses, and respiratory therapists. Daily multidisciplinary rounds include the intensivist, advanced practice nurse, ICU nurse, case manager, respiratory therapy, pharmacy, infectious disease, and a nurse leader. SWAT team consists of intensivist, advance practice nurse and charge nurse available as bedside consult. ICU charge nurse responds to codes, and RRT’s. Each shift begins with a safety huddle to look back and forward 24 hours to review safety and quality concerns.

Orientation: 12 weeks of orientation is offered based on nursing experience for nurses with at least one year of experience.

Performs Phlebotomy with minimal guidance. Responsible for accessioning specimens or patient tissue materials for testing including packaging specimens or tissue materials to be sent out to referring laboratory. Administers oral solutions for glucose tolerance testing according to established training and identifies the best method for retrieving blood samples and other specimens.

Verifies patient identification with two identifiers both before and after venipuncture, labeling tubes immediately following the venipuncture in front of the patient.

Processes specimens to prepare them for laboratory testing, to include centrifuging and aliquoting of blood samples. Packages and stores specimens appropriately for transport using guidance from lab test directories.

Performs Phlebotomy with minimal guidance. Responsible for accessioning specimens or patient tissue materials for testing including packaging specimens or tissue materials to be sent out to referring laboratory. Works closely with all laboratory staff, medical staff, and external customers. Performs customer service functions including telephone etiquette and basic troubleshooting and problem resolution..
Key Responsibilities:
Performs patient check-in procedure and updates patient information as necessary in registration system.
When applicable, places generic manual orders and sends to Specimen Receiving or to the Patient Service Specialist for accessioning.
Instructs patients on proper collection of urine, stool, sputum and saliva specimens, and provides appropriate collection supplies.
Administers oral solutions for glucose tolerance testing according to established training and identifies the best method for retrieving blood samples and other specimens.
Demonstrates Credo behaviors in all interactions including explaining blood draw procedures to patients, answering any questions about the process and assisting patients, co-workers and internal customers as appropriate
Verifies patient identification with two identifiers both before and after venipuncture, labeling tubes immediately following the venipuncture in front of the patient.
Maintains specimen integrity by using aseptic technique, following department procedures and obtains blood specimens by performing venipunctures and capillary sticks according to age specific guidelines.
Processes specimens to prepare them for laboratory testing, to include centrifuging and aliquoting of blood samples. Packages and stores specimens appropriately for transport using guidance from lab test directories.
Takes responsibility for safety checks, supply management and assists with training. preceptor to students and new hires.
