Are you a compassionate Occupational Therapist who values patient-centered care and enjoys working independently? We are seeking a dedicated *Home Health Occupational Therapist* to join our growing team. In this role, you will help patients regain independence, improve safety, and achieve their functional goals in the place they call home.

As a member of our interdisciplinary team, you will have the opportunity to make a meaningful impact on patients' lives by providing skilled therapy services, caregiver education, and personalized treatment plans that support recovery and quality of life.

What You'll Do

* Perform comprehensive occupational therapy evaluations in patients' homes.
* Develop individualized treatment plans based on physician orders and patient goals.
* Provide skilled interventions to improve activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
* Assess home safety and recommend adaptive equipment and environmental modifications.
* Train patients and caregivers in energy conservation, fall prevention, and compensatory strategies.
* Collaborate with nurses, physical therapists, speech therapists, social workers, physicians, and caregivers to coordinate patient care.
* Monitor patient progress and adjust treatment plans as appropriate.
* Complete timely and accurate electronic documentation.
* Participate in interdisciplinary team meetings and case conferences.
* Ensure compliance with all agency policies, payer requirements, and regulatory standards.

What We're Looking ForRequired Qualifications

* Graduate of an accredited Occupational Therapy program.
* Current Occupational Therapist license in the state of practice.
* Current CPR certification.
* Valid driver's license, reliable transportation, and current automobile insurance.
* Strong communication, organizational, and clinical assessment skills.

Preferred Qualifications

* One or more years of experience as an Occupational Therapist.
* Home health experience preferred.
* Experience working with adult and geriatric populations.
* Knowledge of Medicare home health regulations and documentation requirements.

* Frequent travel within assigned territory.
* Ability to safely assist patients with transfers and mobility activities.

Pay: $90,000.00 - $105,000.00 per year
