Major Responsibilities:
Interprets physician referrals and conducts a speech and language evaluation of patients with various speech, language, voice, cognitive and swallowing disorders.
Establishes a written plan of care and implements individual treatment based on evaluation results, functional goals and by utilizing appropriate techniques. Monitors and evaluates outcomes and appropriately adjusts the individualized treatment plan based on the patient's response to treatment and provides status updates to physicians and other teammates. Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs.
Plans for and completes patient discharge including providing written home programs, patient/family teaching, referral for community services, transition planning within the continuum of care and ordering equipment as indicated/appropriate. Communicates and collaborates with appropriate stakeholders to assist in achieving optimum care for the patient.
Completes all required documentation following department policy and accreditation standards with appropriate co- signatures, including patient evaluations, progress notes, team conference reports, and discharge summaries as appropriate. Record patient care and progress of each patient assigned. Completes daily statistics, billing, and scheduling. Applies insurance guidelines regarding authorization and number of visits. Responsible for maintaining site/department established productivity standards. Responsible for understanding and adhering to all billing and documentation policies, procedures, and timelines.
Responsible for cleaning up after patient treatment, putting away equipment, and keeping area neat and orderly. Performs various manual techniques, patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique. Demonstrates skill in use of modalities, exercise equipment and other devices that are available in area worked per department orientation checklist. Demonstrates knowledge of indications and contraindications of modalities. Promotes patient safety by participating in safety initiatives as requested and appropriately follows all safety and infection control measures pertaining to the general working environment.
Participates in performance improvement activities designed to identify and resolve problems affecting patient care outcomes and to improve quality of care. Manages assigned caseload, informing site leadership of coverage needs and ability to cover; assists with coverage, based on department needs and regional needs.
Pursues professional growth through participation in continuing education courses, department in-services, and reading professional literature. Orients new staff to clinical and department policies as requested. Participates in the evaluation process of co-workers by providing verbal or written feedback as requested.
May supervise non-licensed personnel following all applicable state and local compliance requirements. Includes delegating patients to assistive personnel while providing direction, recommendations regarding treatment and ongoing supervision while maintaining responsibility for patient plan of care and goals. Supports the student clinical education program by monitoring, observing, and providing work direction to a minimum of one student per year unless students are unavailable.
Minimum Job Requirements
Education
Master’s degree in Speech Pathology from an accredited school or educational requirements commensurate with the date of graduation; maintains continuing education requirements as mandated by applicable state professional standards.
Certification / Registration / License
Speech Language Pathologist license issued by the state in which the teammate practices. If the teammate supports multiple states, licensure in at least one of those states is required prior to hire.
Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) American Heart Association.

Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs.

This position provides screening, evaluation, and rehabilitation services for patients with speech and language disorders related to speech, language, cognitive-communication, voice, swallowing, and fluency. This position plans and carries out specialized treatment programs, documents treatment, and maintains data regarding patient care. This position demonstrates the clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served.
CORE FUNCTIONS
1. Evaluates patients’ functional abilities and limitations to determine appropriate intervention. Develops a plan of care and treatment goals, as ordered by physician or other referral source.
2. Implements individualized treatment plans that consider patients’ physical, social, spiritual, cultural, educational, and age-specific needs. Ensures that treatment plans are supported by standard practices and/or current literature. Re-evaluates treatment plans, as needed, to ensure maximum patient benefit.
3. Educates patients, family, and other care providers to promote patient participation and attainment of therapeutic program goals. Provides individualized home programs with clear instructions.
4. Documents patient treatment according to professional and regulatory standards.
5. Participates in the provision of training to other staff and students.
6. Follows plan of care to accomplish functions within established procedures and scope of practice. Makes independent judgments based on specialized knowledge. Confers with supervisor on any unusual situations. Internal customers: All levels of nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: Physician offices, patient families, third party payers, and medical supply companies.
MINIMUM QUALIFICATIONS
Masters degree in Speech Language Pathology from an accredited university program.
Current licensure in the state of practice. Certificate of Clinical Competence or documented acceptance in Clinical Fellowship Year. BLS certification required.
Requires the ability to work collaboratively with physicians and other healthcare providers. Must have strong interpersonal communications and human relation skills.
Must have the ability to become proficient with computer software applications commonly used in an automated patient care environment. Employees working for Banner Home Care AZ must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.

Estimated Pay Range:
$39.00 - $65.00 / hour

Minimum Qualifications:
Education: Graduate degree in Speech/Language Pathology from an accredited university-based Speech/Language Pathology program.
Licenses/Certifications: Current license to practice Speech/Language Pathology by the State Board of Examiners for Speech Pathology and Audiology for Texas. Current certification from the American Speech-Language-Hearing Association. Current certification in Basic Life Support.
Experience / Knowledge / Skills:
- Must have at least two (2) years experience.
- The position requires occasional lifting and carrying of up to 30 pounds; prolonged standing; frequent walking, stooping and squatting. Fine hand dexterity, eye-hand coordination, the ability to receive oral and written communication, and to communicate verbally and in writing are also necessary. The visual ability to monitor instruments, computers and equipment, recognize alphanumeric characters, distinguish colors and observe physical characteristics are essential to this position.

Monroe Carell Jr. Children's Hospital at Vanderbilt is seeking a Pediatric Acute Care Speech Language Pathologist (SLP) on a PRN basis to provide feeding therapy, swallowing therapy, dysphagia management, speech-language services, and pediatric rehabilitation for infants, children, and adolescents across inpatient and outpatient settings.
This role supports medically complex pediatric populations, including infants with feeding difficulties, dysphagia, tracheostomies, traumatic brain injuries, developmental and communication disorders, and children requiring Video Fluoroscopic Swallow Studies (VFSS). • Speech Pathology Treatment Implementation (Novice): Demonstrates proficiency providing treatment consistent with physician prescription and established plan of care. • Speech Pathology Patient Evaluation (Novice): Demonstrates proficiency in evaluating patients with speech-language, auditory, and/or swallowing/oral feeding deficits using the appropriate testing methods and equipment for the patient type, diagnosis, and reason for the referral. This position provides screening, evaluation, and rehabilitation services for patients with speech and language disorders related to speech, language, cognitive-communication, voice, swallowing, and fluency. 3. Educates patients, family, and other care providers to promote patient participation and attainment of therapeutic program goals. Provides individualized home programs with clear instructions.
4. Documents patient treatment according to professional and regulatory standards.
5. Participates in the provision of training to other staff and students.
6. Follows plan of care to accomplish functions within established procedures and scope of practice. Makes independent judgments based on specialized knowledge. Confers with supervisor on any unusual situations. Internal customers: All levels of nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: Physician offices, patient families, third party payers, and medical supply companies.
MINIMUM QUALIFICATIONS
Masters degree in Speech Language Pathology from an accredited university program.
Current licensure in the state of practice. Certificate of Clinical Competence or documented acceptance in Clinical Fellowship Year. BLS certification required.
Requires the ability to work collaboratively with physicians and other healthcare providers. Must have strong interpersonal communications and human relation skills.

About the Role
As a Pediatric Speech-Language Pathologist, you will:
- Provide speech-language evaluations and treatment services for infants, children, and adolescents.

Qualifications
Required
- Master's degree in Speech-Language Pathology from an accredited program.
- Current Tennessee Speech-Language Pathologist license or eligibility.
- Certificate of Clinical Competence (CCC-SLP).
- Minimum one year of pediatric Speech-Language Pathology experience.
- Ability to work independently while collaborating effectively within a multidisciplinary environment.

• Speech Pathology Patient & Family Education (Novice): Demonstrates proficiency in communicating functional impact of assessment findings, treatment interventions, treatment planning diagnosis, and disease process. • Discharge Planning (Novice): Demonstrates proficiency in the discharge planning process. Makes recommendations for continued services, follow-up, or referrals to other services. Seeks guidance consistently on the transition and discharge process including transition services and referral resources. May anticipate transition and discharge needs of the patient based upon understanding of pathology/diagnosis and relationship to their present functional status. Will have completed at least one year of experience with transition and discharge planning.
• Speech Pathology Treatment Implementation (Novice): Demonstrates proficiency providing treatment consistent with physician prescription and established plan of care. Identifies treatment objectives based upon assessment results and family input. Writes functional objectives in measurable terms that are consistent with the family's goals and resources. Applies evidence based clinical management practices. Identifies and recommends additional/alternative services when appropriate.
• Speech Pathology Patient Evaluation (Novice): Demonstrates proficiency in evaluating patients with speech-language, auditory, and/or swallowing/oral feeding deficits using the appropriate testing methods and equipment for the patient type, diagnosis, and reason for the referral. Demonstrates the ability to obtain the case history and selects appropriate measures and performs a functional assessment to document patient's status requiring skilled therapy. Interprets the results of the assessment and their significance for the patient, family, referring physician and other members of the health care team.

I. Major Responsibilities:
1. Conducts diagnostic evaluations of various communication disorders in both children and adults. This may include, but is not limited to, language disorders, articulation deficits, auditory and auditory-perceptual deficits, voice disorders, stuttering and neurological disorders such as congential, developmental, or acquired. Assessment may include consideration of the physical status of the oral and vocal mechanisms, breathing patterns, vocabulary, language structure, articulation, voice quality, fluency, auditory processing, language integration, written language and such other parameters as may be deemed appropriate. Analyzes client’s data and interprets test results to physicians.
2. Provides remedial treatment in both individual and group settings for children and adults with the above communication and swallowing disorders, utilizing clinical techniques appropriate to each type of disorder. Uses complex decision making to analyze each patient’s problems and develop treatment interventions to maximize function in daily living activities.
3. Recommends, designs, and/or implements augmentative methods of communication such as communication boards and mechanical devices when oral and/or aural communication is not possible. This is done in conjunction with the patient, family and/or others.
4. Provides information about the communication disorders to patients, families and others.
5. Designs and establishes home programs and other activities which may be used for carryover or reinforcement of the therapy program.
6. Maintains records and reports necessary to the conduct of treatment. This may include, but is not limited to diagnostic evaluation reports, daily treatment notes, monthly physician renewals as appropriate, progress reports as appropriate or requested and losing summaries of treatment.
7. Confers with other individuals and/or agencies involved with the patient’s program. This may include physicians and other related professions, school personnel, social service agencies and the like.

• Graduate of an accredited baccalaureate or post-baccalaureate program in Speech-Language Pathology
• Current Basic Life Support Certification
• Required Valid Texas License or temporary license
• 1 year licensed SLP experience preferred.

Methodist Southwest Medical Center, previously known as Methodist Charlton Medical Center, is a 314-bed, full-service, acute care teaching hospital that serves Cedar Hill, DeSoto, Duncanville, Lancaster, and Glen Heights in southwestern Dallas County.
