Patients depend on our team for the best in evidence-based care and innovative therapies, such as inhaled nitric oxide, high-frequency oscillation, and invasive and non-invasive ventilator support modalities. Opportunities exist for motivated Registered Respiratory Therapists (and Certified Respiratory Therapy Technicians) with the skills to provide a high level of quality service in a variety of settings including the ED, Trauma and Critical Care.
Summary:
Provides care for patients with cardiopulmonary disorders.
Responsibilities:
Educates the patient and family about the patient's illness and provides information about community support groups and other resources.
Delivers and assesses response to ordered therapy per plan of care. Monitors, documents, and communicates patient condition.
Responds to codes and calls for emergent assistance. Assesses patient condition and delivers appropriate treatment.
Evaluates respiratory care policies and procedures based on patient outcomes, current research, and best practices.

Shift varies: 0600-1430 or 0800-1630, will work at any three of our locations: Southern Lakes Surgery Center, Aurora Medical Center Burlington, or Aurora Lakeland Medical Center. Call shifts are for Aurora Medical Center Burlington and would be one call day per week and every 8-10th weekend.
Pay Range:
$38.20 - $57.30Major Responsibilities:

- Utilizes the nursing process to assess, plan, diagnose, implement, and evaluate nursing care, engaging patients and families through the continuum of care.

- Continuously evaluates patient, team, and unit outcomes, taking action as needed.

Minimum Job Requirements
Education
Graduate of a Board of Nursing approved nursing education program.
Certification / Registration / License
- Basic Life Support (BLS). Additional education, training, certifications, or experience may be required based on specialty.
- Active registered nurse (RN) multi-state compact and/or single-state license with privileges to practice in the state(s) where the RN is providing client nursing services
Note: Licensed nurses practicing via telehealth/telenursing/virtual modalities are required to be licensed or hold the privilege to practice in the state(s) where the client(s) is/are located. Licensed nurses are responsible and accountable for knowing, understanding, and practicing in compliance with the laws, rules, regulations, and standards of practice of the state(s) where the client(s) is/are located.
Experience
Typically requires 1 year of experience in clinical nursing.

Physical Requirements and Working Conditions
- Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.
- Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.
- Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.
- Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.
- Maneuver foot pedals on carts or machines.
- Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.
- May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.
- Must be able to respond quickly to changes in patient and/or unit conditions.

- Proficiently operates angiographic and ultrasound equipment used to perform basic to advanced procedures on inpatients and outpatients. Must perform and be proficient with all of the following procedures if performed at assigned facility or by assigned specialty at ASLMC (neuroendovascular, interventional radiology or interventional cardiology), which includes carotid stenting, cerebral intervention including embolization, coiling/stenting, angiograms and/or vertebro, kyphoplasty, chemo therapy, thera sphere embolization, adrenal and petrosal sampling, cryo and Radio Frequency (RF) ablation, and/or aortic aneurysm repair, advanced imaging applications, and complex peripheral interventions.
- Assists with interventional and surgical procedures as a scrub assistant with competent knowledge of equipment and equipment options, while working seamlessly with the attending and resident physicians. Serves as a resource to physicians on procedural equipment options.
- Monitors patients and alerts staff of life threatening changes in patient status. Applies the defibrillator and performs cardiopulmonary resuscitation when required. Provides assistance with emergency procedures, responding quickly and effectively as they occur.
- Identifies and processes images, assuring that images are of diagnostic quality. Assists clinical engineering staff in evaluating digital equipment to maintain consistent image quality.
- Compares requisitions to physician orders to ensure ordered exam accuracy.
- Post-processes images as directed and accurately identifies and labels images, identifying patient name, medical record unit (MRU) number, and location or position of scan on all images.
- Maintains records in accordance with various regulatory agencies, and enters patient and study information into the radiology information system, and documents billing codes of procedures performed to ensure accurate patient charges.
- Serves as a technical resource, role model and/or preceptor for students and staff. Acts as a resource to staff at own facility and throughout the system.
- Facilitates clinical problem solving and collaborates with other departments to provide education on managing patients after procedures, and any other imaging department processes and procedures.
- Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Summary:
Coordinates the successful transfer or bed placement of patients entering Memorial Healthcare System (MHS). Ensures patients are assigned rooms in an expedient manner by communicating effectively with the surgery department, emergency department, nursing units, physician offices, and outlying facilities to ensure appropriate transfer and placement of patients. Gives high priority to customer service and complete customer satisfaction.
Responsibilities:
Follows established documentation protocols for transfer and bed placement in the Epic EMR to ensure adequate patient safety standards are met.
Collaborates across all disciplines to coordinate patient transfers, including disposition, ambulance transportation (internal and external), specialist referrals, etc. Coordinates the successful transfer and placement of patients across MHS.
Assigns initial bed placement and establishes bed assignments based on documented assessment, patient and bed attributes, isolation and identified risks.
Anticipates admission needs and communicates findings with transferring department, nurse or facility and collaborates to obtain optimal patient flow across the system.
Plans, implements, and evaluates patient needs based on reported assessment to optimize outcomes and maximize available resources. Remotely triages using Critical Care or Emergency Department experience to identify patient care needs providing expert and timely recommendation for correct facility transfer based on patients acuity, services required, condition, circumstances and area facility capacity and capability matrixes.
Utilizes the chain of command to advocate for a patient in need of priority admission or transfer to higher or lower level of care that are beyond the scope of practice for the facility, specialists, etc. (e.g., STEMI, stoke, trauma alert).
Accurately completes effective and expedient placement of patients from all nursing units with emphasis on the ED, ICU, Post Anesthesia Care, Cardiac Care, Cath Lab, and external transfers.
Remotely engages with facility staff to monitor and document transfer need; communicates patient condition to sending or receiving providers and facilities. Consults other disciplines based upon patient s needs, e.g., infection control, equipment needs, nutritional, functional, pain, social and skin integrity.
Serves as the expert in Emergency Medical Treatment and Labor Act (EMTALA) and intervenes including escalation to leadership, as needed. Follows established protocols for interfacility transfer acceptance and escalates to direct report, Administrative Officer, or Administrator On Call (AOC).

- Performs basic to complex diagnostic tests and visual screenings, such as: visual field/acuity, tonometry / glaucoma pressures / applanations, topography, color vision testing, lensometry, A-Scans, ocular motility, measurement of the cornea using a keratometer, refraction, corneal topography, retinoscopy, blood pressure, and glare testing.
- Performs refractometry to determine the refractive error of the eye.
- Assists the ophthalmologist with surgical procedures and post-operative care or treatment.
- Performs medical eye photography functions as directed by the physician, which may include taking general optic nerve, retinal and macular photos, and fluorescein angiograms, etc.
- Assists patients with ordering and dispensing glasses and contact lenses. Performs contact lens polishing.
- Maintains and calibrates equipment.
- Sterilizes instruments using appropriate technique and procedures.
- Assists with scheduling patients for post-operative, surgery and specialist appointments.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. - Competently performs basic to advanced radiologic procedures including general diagnostic, general fluoroscopy, and c-arm procedures, if performed at assigned facility.
- Communicates with patients to explain procedures, obtain clinical history, confirm acknowledgment of patients' understanding and address any questions or concerns.
- Positions patients and selects exposure factors for various radiologic examinations and provides patient with radiation protection and safety in accordance with prescribed methods.
- Identifies patient and labels images pertaining to the patient, compares requisitions to physician's orders to ensure exam accuracy, and processes images ensuring quality of radiographs and calls back patients as necessary.
- Assists, communicates and consults with the physician(s). Works independently outside the department with providers.

Schedule: Full-time, M- F 8am-5pm with occasional on-call.
Major Responsibilities:
Responsible for identifying and coordinating patient/family care to support terminally ill patients and families in all hospice settings. Frequency of patient / family contacts will be at the discretion of the Case Manager and his/her assessment of need, but will be a minimum of every 15 days. The Case Manager utilizes teaching, assessment, and intervention skills to provide comfort care at end of life and maximize the quality of life for the patients and families.
Educates patients and families regarding disease processes and trajectory, disease management, hospice philosophies, signs and symptoms of decline and end of life. Updates the primary physician as indicated and according to agency, accreditation, state and federal requirements.
Responsible for continuous review of all aspects of every patient of her/his caseload to include appropriate utilization of services and the appropriate hospice level of care criteria are met. Makes referrals to other team members as appropriate.
Administers medication and treatments as prescribed by the physician.
Leads the interdisciplinary care group (IDG) specific to each patient and family on their caseload. Working within the IDG to develop, revise, and implement the patient’s Plan of Care. Coordinates all services and collaborates with members of the interdisciplinary team to provide consistent patient care. Arranges for and/or recommends equipment, medical supplies and other necessary items and services as indicated.
Enters, completes and updates documentation in the electronic health record as required according to agency/department standards and policies, accreditation, state and federal guidelines.
Documentation accurately records the patient’s experience with a combination of assessments and narrative .
Provides on-going assessment of patient response to treatments, medications and teaching while making appropriate changes in interventions and follow-up to meet patient needs. Updates and revises POC as indicated with supportive documentation to changes. Communicates problems and changes in condition to physicians, supervisor and other members of the IDG. Enters verbal orders from the physician or APN into the Electronic Health Record.
Supervises and performs Home Health Aide and licensed practical nurse supervisory visits according to agency standards. Instructs paraprofessional staff on a timely basis and in accordance with agency, state and federal regulations.
Facilitates the discharge planning process according to agency standards and utilizes interagency/system and community resources to assure continuity of care after discharge.
WI ONLY: May conduct OASIS assessments for Palliative Care patient and establish and update the plan of care. May conduct hospice initial and comprehensive assessments and establish and update the plan of care. Ensures continued skilled nursing/home bound criteria are met. Initiates appropriate preventive and rehabilitative nursing procedures.
Utilizes proper body mechanics, assistive devices and safety techniques when performing various patient positioning/repositioning and transporting duties, which require lifting, and pushing/pulling. Orients and mentors new staff as requested.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration and/or Certification Required:

MAIN DUTIES AND RESPONSIBILITIES:
- Available to cover shifts as assigned at any residential program, to include leaving one program to travel and work at another.
- Facilitate ADL skill-building for residential living and for community integration through supervising and teaching clients daily medication routines, budgeting, self-preservation skills, transportation, etc.
- Provide opportunities for skills practice and collaborative learning.
- Participate on a multi-disciplinary team in the development and implementation of client treatment plans and progress reviews, engaging clients whenever possible in the planning and evaluation process.
- Provide, coordinate and monitor treatment services for all assigned clients.
- Includes but not limited to contact with the client, as prescribed in the treatment plan; provision and coordination of care prescribed in the treatment plan through documented contact and implementation of assigned activities with client, client’s family, other Gateway staff, consultants and treatment plan designated staff from other human service agencies and medically oriented organizations.
- Monitoring the effectiveness of care provided by the program or through other service providers, by comparing service provided with service outcomes, as specified in the treatment plan.
- Provide shift coverage which includes but is not limited to carrying out the many functions of household maintenance, especially cleaning and meal preparation, assist in resolution of medical, psychiatric and behavioral crises, conflict resolution and symptom management.
- Attend supervision and team meetings as required.
- Maintaining current knowledge and updates of agency policies and procedures.

Physical Requirements and Working Conditions

- Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.
- Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.
- Maneuver foot pedals on carts or machines.
- Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.
- May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.
- Must be able to respond quickly to changes in patient and/or unit conditions.
- Physical Abilities Testing may be required.

What You’ll Do:

Collaborates with other clinical team members in assessing and supporting psychosocial needs of the patient/family.

$41.10 - $61.65Major Responsibilities:- Provides for continuity of care by ensuring smooth transitions between care settings according to critical pathways (algorithms). Develops a relationship with patient and their primary care physician to facilitate and/or navigate through a cancer diagnosis, subsequent treatment and follow-up to reflect continuity of care. Personalizes the approach to patients with cancer to maximize service management.
- Completes a nursing assessment with a focus on disease site which has program accountability and identifies the patient's individualized needs. Integrates with other departments and disciplines to develop plan of care. Provides evidence-based practice in the role as cancer care coordinator.
- Guides patients with suspected cancer through the diagnostic phase. Tracks the testing and diagnostic reports through the system to ensure timeliness. Provides biopsy procedure follow-up phone calls as needed.
- Communicates necessary information and collaborates with physicians/office staffs, hospital departments/staff, health care providers, other disciplines and community agencies regarding the patient diagnosed with cancer.
- Refers patients to appropriate resources (i.e., Financial Counseling, cancer counseling center services, clinical trials nurse for research programs). Acts as advocate for patients with cancer. Maintains contact with patients requiring ongoing support as necessary. Identifies patients with compromised coping skills or lack of social support. Facilitates support group for patient with cancer and family members/significant others.
- Collaborates with other caregivers to develop optimal systems, which achieve the most effective patient/family outcomes. Provides in-services and education offerings to staff. Assists leadership in the interview process and with orientation and training of staff, student nurses, etc.
- Counsels and educates patients and their family members on issues of prevention and the early detection of other cancers in which they might be of risk. Evaluates and selects or designs patient education materials on specific disease site programs.
- Actively seeks opportunities to promote awareness of specific cancer disease site program. Provides community education/outreach, e.g., health fairs, phone banks, Cancer Awareness month. Provides workshops, seminars and presentations to audiences in the professional and lay community.
- Partners with leadership staff in cancer services to develop a vision for the care of patients in the specific disease site program. Participates in the development of the annual goals and objectives for the program services and in the development, implementation and maintenance of Quality programs that include evaluation of costs, patient care, and customer satisfaction. Utilizes data to implement "best practices".
- Analyzes variances in standard treatment guidelines (NCCN guidelines algorithms) for initiatives with cancer to improve quality and efficiency, control costs, and provide systematic approach to the patient's total care.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:

- Proficiency in operating computer functions (e.g., E-mail, electronic records, digital platforms etc.) clinical, aviation, maintenance, communications, and administrative departments.
Physical Requirements and Working Conditions
- Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.
- Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.
- Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.
- Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.
- Maneuver foot pedals on carts or machines.
- Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.
- May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.
- Must be able to respond quickly to changes in patient and/or unit conditions.
- Physical Abilities Testing may be required.
- Additional department specific physical requirements may be identified for unique responsibilities within the department by the nurse leader.
