About the Position
Provides skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician, and assigned by the supervising registered nurse.
Monday-Friday, 8a-4:30p, weekend/holiday rotation as required
Minimum Qualifications
​Graduate from an accredited or state approved school of practical nursing
Current licensure in the state of practice
Must have a minimum of one (1) year experience in an acute care setting.
Home healthcare experience preferred.
Must possess interpersonal skills sufficient to interact effectively with clients and their families as well as all interdisciplinary caregivers, peers, subordinates and supervisors.
Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
Must have current Driver’s license and reliable transportation to and from work site.

Shift varies: 0600-1430 or 0800-1630, will work at any three of our locations: Southern Lakes Surgery Center, Aurora Medical Center Burlington, or Aurora Lakeland Medical Center. Call shifts are for Aurora Medical Center Burlington and would be one call day per week and every 8-10th weekend.
Pay Range:
$38.20 - $57.30Major Responsibilities:
- Engages in unit councils, professional governance, and quality initiatives to improve care processes and apply evidence-based practices.
- Utilizes the nursing process to assess, plan, diagnose, implement, and evaluate nursing care, engaging patients and families through the continuum of care.
- Monitors patient conditions, adjusts care plans, mobilizes resources, and collaborates with the care team to influence care outcomes.
- Upholds and promotes a culture of safety.
- Continuously evaluates patient, team, and unit outcomes, taking action as needed.
- May administer medications, treatments, and therapies safely and according to clinical protocols and procedures.
- Demonstrates effective communication, feedback, and conflict resolution, fostering team collaboration and appropriate delegation.
- Pursues professional development, completes required education, and maintains certifications.
- Adhere to the ANA Code of Ethics and practices ethical decision-making, respects interdisciplinary roles, and contributes to integrated, unbiased patient care.
- Appropriate delegation to other Registered Nurses, Licensed Practical Nurses, and unlicensed assistive personnel (i.e., nurse aides, credentialed medical assistants, etc.).
- Maintains accurate, timely EHR documentation.
- May be required to float to other units, departments, care areas, or facilities within the designated service area to meet patient care needs.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Minimum Job RequirementsÂ
Education
Graduate of a Board of Nursing approved nursing education program.Â
Certification / Registration / LicenseÂ
- Basic Life Support (BLS). Additional education, training, certifications, or experience may be required based on specialty.Â
- Active registered nurse (RN) multi-state compact and/or single-state license with privileges to practice in the state(s) where the RN is providing client nursing servicesÂ
Note: Licensed nurses practicing via telehealth/telenursing/virtual modalities are required to be licensed or hold the privilege to practice in the state(s) where the client(s) is/are located. Physical Requirements and Working Conditions
- Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.Â
- Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.Â
- Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.Â
- Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.Â
- Maneuver foot pedals on carts or machines.Â
- Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.Â
- May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.Â
- Must be able to respond quickly to changes in patient and/or unit conditions.Â

Job Profile:
Performs duties relating to film and image management as well as various clerical support duties pertinent to Radiology.
Major Responsibilities:
- Answers telephone, screens and directs calls, and pages for calls within the department as necessary, and responds to requests for information. Verifies validity of telephone requests from physicians and/or patients for the release of radiology reports.
- Provides radiology reports and/or radiographic images to staff, physicians, patients and/or families upon request. Files, retrieves, and releases health information records and films.
- Retrieves images to print onto film or save onto compact disc read-only memory (CD-ROM) format. Digitizes outside films, pulls images from advanced workstation (AW) and loads into the picture archive and communication system (PACS). Downloads CD's onto PACS and retrieves images from PACS from other facilities. Performs all data entry, bar coding, and printing of bar code labels related to film management.
- Responsible for retrieving, logging out, and delivering film jackets to all areas as required. Prepares Radiology film release/copy forms and is responsible for obtaining appropriate legal authorization and verification.
- Explains film copy and sign out procedures to patients and assists them with required paperwork and routes them accordingly.
- Retrieves all films from specialty departments for timely interpretation on a regular basis. Ensures film jackets reflect pertinent information and required paperwork is processed before returning them to the department.
- Responds independently to problems, complaints, and questions from staff, patients, and visitors, and is responsible for handling or referring them as needed.
- Prioritizes x-ray requests when specific medical conditions are being questioned by the ordering physicians, i.e. line placements, pneumothorax, and free air.
- Maintains inventory of file room supplies and distribution. Schedules courier service to clinics and other facilities as required.
- Assists in the administration of the PACS system training program which includes, assisting in identifying and supporting user training, and troubleshooting training problems/issues.

- Prepares and rooms the patient for exam by obtaining vital signs and documenting/updating pertinent health information (i.e. chief complaint, allergies, medications).

- Provides basic patient education on the performance of tasks, skills, treatment and self-care. Follows through with necessary procedure or test requests, prescription refills, new appointment times and referrals to other facilities or services. Refers questions to registered nurses and physicians/providers per scope of practice guidelines and relays information back to patient as directed.
- Communicates with physicians and other members of the health care team. Documents and communicates accurate and timely information regarding patient conditions.
- Performs advanced clinical tasks or skills such as removal of sutures and staples, catheterization, audiograms, laboratory procedures (throat/nose culture, drug screen, phlebotomy - butterfly, venous access, heel stick), EKG, spirometry, Holter monitor application, casting, complex wound care, etc., with demonstrated competency and under clinical supervision.
- Organizes and prepares exam rooms, lab and work areas including: cleaning, stocking, ordering supplies, and maintaining equipment. Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
- Completes routine maintenance and quality monitoring of clinic equipment using appropriate technique when cleaning, sterilizing and caring for instruments and other equipment.
- Participates in patient centered initiatives, care management, performance and quality improvement activities.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Our progressive care unit is a 16-bed unit caring for our cardiothoracic surgery (CTS) patient population-lobe resections, esophagectomy, etc. Cardiac overflow patients, stroke, increase monitoring capabilities, cardiac drips, insulin drips, overflow patients needing higher acuity of care. This unit offers an excellent learning environment for both newly licensed and experienced RNs.

Ensures patients are assigned rooms in an expedient manner by communicating effectively with the surgery department, emergency department, nursing units, physician offices, and outlying facilities to ensure appropriate transfer and placement of patients. Gives high priority to customer service and complete customer satisfaction.
Responsibilities:
Follows established documentation protocols for transfer and bed placement in the Epic EMR to ensure adequate patient safety standards are met.
Collaborates across all disciplines to coordinate patient transfers, including disposition, ambulance transportation (internal and external), specialist referrals, etc. Coordinates the successful transfer and placement of patients across MHS.
Assigns initial bed placement and establishes bed assignments based on documented assessment, patient and bed attributes, isolation and identified risks.
Anticipates admission needs and communicates findings with transferring department, nurse or facility and collaborates to obtain optimal patient flow across the system.
Plans, implements, and evaluates patient needs based on reported assessment to optimize outcomes and maximize available resources. Remotely triages using Critical Care or Emergency Department experience to identify patient care needs providing expert and timely recommendation for correct facility transfer based on patients acuity, services required, condition, circumstances and area facility capacity and capability matrixes.
Utilizes the chain of command to advocate for a patient in need of priority admission or transfer to higher or lower level of care that are beyond the scope of practice for the facility, specialists, etc. (e.g., STEMI, stoke, trauma alert).
Accurately completes effective and expedient placement of patients from all nursing units with emphasis on the ED, ICU, Post Anesthesia Care, Cardiac Care, Cath Lab, and external transfers.
Remotely engages with facility staff to monitor and document transfer need; communicates patient condition to sending or receiving providers and facilities. Consults other disciplines based upon patient s needs, e.g., infection control, equipment needs, nutritional, functional, pain, social and skin integrity.
Serves as the expert in Emergency Medical Treatment and Labor Act (EMTALA) and intervenes including escalation to leadership, as needed. Follows established protocols for interfacility transfer acceptance and escalates to direct report, Administrative Officer, or Administrator On Call (AOC).

Provides crisis services and intervention to client’s potential crisis through telephone and face to face evaluation.

MAIN DUTIES AND RESPONSIBILITIES:
- Worker is directly accessed via an answering service and is to respond by phone within 15 minutes during the entirety of shift.
- Provides face-to-face mental health evaluations in hospital emergency rooms, police stations and at people’s home as needed.
- Provides consultation with other service providers regarding referrals and other resources.
- Provides consultation with other clinical staff relevant to clinical issues with clients.
- Consults with the administrator-on-call, and psychiatrist-on-call, to discuss concerns/difficult clinical situations, issues with system/providers, incident report level issues, state contract bed and children’s admissions and exploration of options, alternatives, and for support.
- Has knowledge base and can access all resources for hospitalization and crisis stabilization/hospital diversion.
- Will complete documentation including incident reports of every contact that occurs as well as results from consult with the administrator or psychiatrist on-call.
- Will be responsible for obtaining shifts or information about clients in crisis ½ hour prior to the beginning of their shift.
- Will provide report after every shift by 8 a.m. on Monday through Friday and by 8 a.m. on Monday from every weekend shift.
- Will complete all documentation and forward to a regional mental health center 24 hours after the completion of their shift.
- Refer clients to other community resources as needed.
- Attend all necessary meetings as assigned.
- Will provide supervision and consultation to assigned on-call clinicians who require training in emergency services protocol.
- Other duties and responsibilities as assigned or needed.
MINIMUM QUALIFICATIONS:
- All workers must have an R.N. and/or Master’s Degree for above in a related mental health program to be eligible for on-call.
NECESSARY SPECIAL REQUIREMENTS:
- All R.N’s are required to have a nursing license.
- Any other licenses will be forwarded to personnel and also the business office for billing purposes.
- Must be certified or certifiable by Department of MHRH as an Emergency Services clinician.
- Ability to work independently as well as part of a multi-disciplinary team, self-motivated.

Job Description:
The Supervisor is responsible for supervising operational activities and providing clinical management of rehab services to the patient populations served by the facility. This shall include the direction, supervision, instruction, and evaluation of the professional staff, supportive staff, and students providing rehab services to patients. The Supervisor will also perform the duties of a clinical therapist.
The Supervisor will promote smooth operations, prompt and effective response to problems/concerns, continued department development, and program expansion. This position is also responsible for coordination of interdepartmental functions and activities to ensure smooth and effective operations consistent with the objectives of the department, the facility, and of the Frazier Rehab Institute.
Essential Functions:
- Communicates patient status by providing timely written or verbal communication to physicians and therapy customers
- Consistently establishes and maintains effective working relations with, patients and family members, internal customers, and external customers
- Demonstrates an understanding of the financial implications of treatment costs and resources on the department/facility by consistently working to provide cost effective treatment programming to served populations
- Ensures that patient charges and corresponding clinical documentation are in compliance with regulatory standards and department policies and procedures
- Supervises department personnel for one - two locations in a manner that ensures the highest quality of services and according to organization and department policies and procedures
- Develops a department personnel schedule to maintain adequate staffing to provide services to the patients at the facility. Works with other facility managers to ensure effective use of staff throughout the organization
- Monitors and reviews work of personnel to ensure that productivity, standards of practice, and regulatory requirements are consistently met
- Effectively coaches and counsels department personnel to facilitate optimum team member performance
- Encourages staff participation in program development, performance improvement activities, and community events sponsored by the organization
- Responds to team members’ concerns by demonstrating effective listening skills, problem solving skills, and participatory management when situations require intervention. Communicates issues with the Manager/Director or Administration as appropriate
- Manages department operations in one – two locations a manner that ensures the highest quality of services and according to organization and department policies and procedures

- Performs the clinical duties of a supervising therapist, exhibiting an advanced level of clinical competence. Evaluates and implements competent therapy treatment and provides regular re-assessments for the purpose of treatment modification, to maximize patients’ potential for functional independence, all in accordance with acceptable practice guidelines and department policies and procedures
- Assists with recruitment, interviewing, and selection of new personnel
