Join Our Team! The Medical Telemetry Unit (4th Floor) at Atrium Health University City is a 31-bed acute care unit dedicated to delivering exceptional patient-centered care. With a typical 4:1 to 5:1 RN-to-patient ratio based on acuity, our nurses are empowered to practice at the top of their license while providing high-quality, evidence-based care.

Major Responsibilities:
- Engages in unit councils, professional governance, and quality initiatives to improve care processes and apply evidence-based practices.
- Utilizes the nursing process to assess, plan, diagnose, implement, and evaluate nursing care, engaging patients and families through the continuum of care.
- Monitors patient conditions, adjusts care plans, mobilizes resources, and collaborates with the care team to influence care outcomes.
- Upholds and promotes a culture of safety.
- Continuously evaluates patient, team, and unit outcomes, taking action as needed.
- May administer medications, treatments, and therapies safely and according to clinical protocols and procedures.
- Demonstrates effective communication, feedback, and conflict resolution, fostering team collaboration and appropriate delegation.
- Pursues professional development, completes required education, and maintains certifications.
- Adhere to the ANA Code of Ethics and practices ethical decision-making, respects interdisciplinary roles, and contributes to integrated, unbiased patient care.
- Appropriate delegation to other Registered Nurses, Licensed Practical Nurses, and unlicensed assistive personnel (i.e., nurse aides, credentialed medical assistants, etc.).
- Maintains accurate, timely EHR documentation.
- May be required to float to other units, departments, care areas, or facilities within the designated service area to meet patient care needs.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Minimum Job RequirementsÂ
Education
Graduate of a Board of Nursing approved nursing education program.Â
Certification / Registration / LicenseÂ
- Basic Life Support (BLS). Additional education, training, certifications, or experience may be required based on specialty.Â
- Active registered nurse (RN) multi-state compact and/or single-state license with privileges to practice in the state(s) where the RN is providing client nursing servicesÂ

Experience

Physical Requirements and Working Conditions
- Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.Â
- Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.Â
- Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.Â
- Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.Â
- Maneuver foot pedals on carts or machines.Â
- Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.Â
- May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.Â
- Must be able to respond quickly to changes in patient and/or unit conditions.Â
- Physical Abilities Testing may be required.Â

Position Title
Position Summary / Career Interest:
The Inpatient Social Work Case Manager has responsibility to provide care/service safely and efficiently for a full range of services to patients of all ages and their families. Primary role is to collaborate, communicate and facilitate coordination of services post-hospitalization as established by the healthcare team and executed by the case manager. Responsible for the psychosocial assessment of patients, for post hospital discharge needs including home care, nursing home placement, durable medical equipment, financial assistance, counseling, and other community resources. Ensure appropriate decision makers are informed of and involved in treatment planning. Apply pertinent state and federal regulations regarding discharge planning and reporting requirements.The Inpatient Social Work Case Manager identifies, monitors, and reports opportunities for quality and performance improvement to the appropriate department. The Social Work Case Manager takes an active role in performance improvement activities as it relates to their area of assignment.
Responsibilities and Essential Job Functions
- Accepts responsibility and accountability for achievement of optimal outcomes within their scope of practice. Follows policies, procedures, and standards; complies with Corporate Compliance program. Assumes responsibility for risk and safety issues associated with the position. Takes call as required by the department expectations. Performs job specific responsibilities and demonstrates accountability for own actions and decisions.
- Acquires and maintains knowledge and competence related to the expectations of their position and practices within their scope. Brings ideas and concerns to supervisor, participates in department decision making. Maintains current licensure.
- Completes psychosocial assessments of patient/family situations including social, psychological, emotional, financial and other related factors to facilitate patients return to the community.
- Identifies and utilizes all relevant information (medical/nursing needs, social work knowledge base, disease process, knowledge of community resources) to accurately and thoroughly assess the patient’s psychosocial situation.
- Evaluates psychosocial and medical/nursing information to determine an appropriate social service action/discharge plan.
- Utilizes social work assessment and input from other team members to formulate realistic recommendations for social work action plan/discharge plan.
- Participates in interdisciplinary team meetings as needed.
- Initiates and participates in family conference to determine psychosocial and discharge planning needs.
- Facilitates and participates in daily Interdisciplinary Care Coordination (ICC) Huddles by managing the daily meeting and providing relevant and discipline specific information to the entire healthcare team.
- Advocates on behalf of patients and caregivers for identification and access to services. Advocates for the protection of the patient's health, safety and rights. Ensures patient choice and consistently supports a patient centered environment.
- Provides supportive counseling to assist patients/families in adjusting to disability and illness, and for realistic planning for post hospital care.
- Demonstrates a caring, positive regard for others by clarity of speech, use of understandable terminology and utilizing active listening skills.
- Assures prudent utilization of all resources (fiscal, staff resources, environmental, equipment and services) by evaluating the options available. Demonstrates ability to balance cost and quality to assure the optimal clinical and financial outcomes.
- Documents appropriate information in the patient’s medical record to ensure communication of patients’ psychosocial needs for post hospital care.
- Documentation includes initial contact note and follow-up action plan.

Job Summary:
Responsible for developing, implementing and leading evidence-based, quality educational programs and activities which promote individual development and competency among nursing department personnel. Develops, coordinates and implements activities for orientation/onboarding, nursing application (practice, policy, products) and continuing education/professional development related to system, site and/or specialty. Provides clinical and educational expertise that directly or indirectly influences nurses and healthcare personnel, systems, and community. Designs, implements, and evaluates educational programs that reflect organizational vision and values with awareness of current healthcare trends. Uses the roles of advisor, change agent, collaborator, communicator, consultant, facilitator, leader, mentor, researcher to impact patient safety and quality outcomes through professional development initiatives. Promotes the generation and dissemination of new knowledge and use of evidence to guide clinical practice and improve patient care while advancing professional development. The role may be based at the system, at the site reporting to the system or at the site reporting to the respective site and may teach, develop or support NEPD programs at the site, region or system level.
Major Responsibilities:
- Through application of ANPD principles and acting in the roles of advisor, change agent, collaborator, communicator, consultant, facilitator, leader, mentor, researcher the team member applies advanced knowledge of nursing in the delivery of evidenced-based nursing and education interventions using the educational design process to bridge the knowledge, skills, and/or practice gaps identified through a needs assessment.
- Collects data and information to validate an identified gap in professional practice and to determine the specific knowledge, skill, or practice deficit or opportunity needed. Investigates issues, trends, and supporting data to determine the needs of individuals, organizations, and communities (prioritizing macrosystem needs).
- Analyzes cost and anticipated return on investment for learning activities, programs, and projects. Transforms processes at multiple levels through inspiration, initiation, adoption, and sustainment of and adaptation to change using project management and improvement processes.
- Develops context-specific outcomes based on organizational stakeholders’ and learners’ values, goals, current evidence, and regulations. Establishes a plan that prescribes strategies, alternatives, and resources to achieve expected outcomes. Incorporates theories pertaining to learning and behavioral change while designing educational materials and programs.
- Promotes the generation and dissemination of new knowledge and use of evidence to guide clinical practice, improves patient care and foster life long learning. Role models practices for creating and sustaining positive learning and practice environments.
- Assesses learner characteristics, including learning styles and learning needs for program development. Creates an effective learning environment, demonstrates proficiency in use of technology. Evaluates the outcomes of staff education, functions in the roles of both teacher and facilitator. Implements a variety of teaching strategies tailored to the learner’s characteristics, learning needs, cultural perspectives, and outcome objectives. Plans programs that are culturally relevant and that incorporate concepts of multicultural and multigenerational education.
- Advises in the design, development, implementation, and evaluation of materials, learning environments (i.e. virtual), and teaching strategies appropriate to the situation and learner’s developmental level, learning needs, readiness, ability to learn, and culture.
- Exhibits system thinking throughout the planning process. Develops partnerships to enhance health care through interprofessional initiatives (i.e. quality improvement, organizational excellence initiatives) and coordinates integrated educational services.
- Supports the transition of nurses and other healthcare team members across learning and practice environments, roles, and professional stages. Mentors, guides, and supports NEPD generalist as applicable.
Licensure, Registration, and/or Certification Required:
- Registered Nurse license issued by the state in which the team member practices, and
- Clinical Nurse Specialist (CNS) certification issued by the American Nurses Credentialing Center (ANCC), or
- Nursing Professional Development (RN-BC) certification issued by the American Nurses Credentialing Center (ANCC) needs to be obtained within 6 months.
Education Required:
- Master's Degree in Nursing, or
- Bachelor's Degree in Nursing, and Master's Degree in Education or related field.
Experience Required:
- Typically requires 3 years of experience in a clinical nursing role that includes experiences in adult teaching or training, and/or developing educational programs
Knowledge, Skills & Abilities Required:

Major Responsibilities:
- Prepares and rooms the patient for exam by obtaining vital signs and documenting/updating pertinent health information (i.e. chief complaint, allergies, medications).
- Performs laboratory procedures (i.e. strep test, wound culture, specimen collection, etc.) using principles of aseptic technique and standard precautions / infection control guidelines.
- Assists the physician/provider with procedures and surgeries such as pelvic exams, allergy scratch testing, EMG, cautery, colposcopy, etc.

Job Description:

Essential Functions
- Directs and supervises activities of OT assistants, OT technicians, students, and volunteers involved in administering care to patients and assists with orienting new and rotating staff.
- Evaluates patients and establishes appropriate patient care plan to include necessary modalities, procedures, exercise programs, and patient education as warranted by the patient's diagnosis and condition.

MINIMUM QUALIFICATIONS
Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.

- Prepares and rooms the patient for exam by obtaining vital signs and documenting/updating pertinent health information (i.e. chief complaint, allergies, medications).
- Performs laboratory procedures (i.e. strep test, wound culture, specimen collection, etc.) using principles of aseptic technique and standard precautions / infection control guidelines.
- Assists the physician/provider with procedures and surgeries such as pelvic exams, allergy scratch testing, EMG, cautery, colposcopy, etc.
- Administers medications, under physician/provider orders, including: immunizations, antibiotics, pain medications, vitamins, topical agents, and insulin. With limitations, initiates, maintains and discontinues IV therapy; administering selected fluids with pre-mixed medications.
- Provides basic patient education on the performance of tasks, skills, treatment and self-care. Follows through with necessary procedure or test requests, prescription refills, new appointment times and referrals to other facilities or services. Refers questions to registered nurses and physicians/providers per scope of practice guidelines and relays information back to patient as directed.
- Communicates with physicians and other members of the health care team. Documents and communicates accurate and timely information regarding patient conditions.
- Performs advanced clinical tasks or skills such as removal of sutures and staples, catheterization, audiograms, laboratory procedures (throat/nose culture, drug screen, phlebotomy - butterfly, venous access, heel stick), EKG, spirometry, Holter monitor application, casting, complex wound care, etc., with demonstrated competency and under clinical supervision.
- Organizes and prepares exam rooms, lab and work areas including: cleaning, stocking, ordering supplies, and maintaining equipment. Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
- Completes routine maintenance and quality monitoring of clinic equipment using appropriate technique when cleaning, sterilizing and caring for instruments and other equipment.
- Participates in patient centered initiatives, care management, performance and quality improvement activities.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:

Summary:
The Bid Manager leads the competitive solicitation process from initiation through contract award, ensuring full compliance with Procurement Code, policies, and procedures.
