Summary:
The Patient Financial Services Representative (PFSR) serves as the first point of contact in greeting patients and guarantors in the hospital, ambulatory or medical office setting. The PFSR engages with the patient or guarantor to obtain pertinent information and answer any questions in an effort to ensure that all required demographic, financial, and insurance eligibility information is gathered and verified. Ensures all required notices and consent forms are signed accordingly.
Responsibilities:
May schedule walk-in appointments for services offered.Verifies insurance benefits including obtaining insurance card(s) and confirms coverage is active. Determines correct insurance filing order, if multiple insurance coverages are effective for that service.Collects patient out-of-pocket responsibility per collection guidelines. Provides patient estimates as requested. Prepares and balances a daily deposit of all payment collections.Explains polices including all regulatory and financial consent forms; secures all required signatures.May perform patient discharge functions including, but not limited to, review of after visit summary (AVS), future appointment scheduling, and referrals.Interviews patients and guarantors at the workstation or bedside to obtain all necessary information, including a copy of the patient or guarantor identification card.Provides exceptional customer service and ensures all questions and concerns are addressed in a timely and courteous manner. May guide the patient to appropriate destination for services.May confirm physician and prescription orders ensuring accuracy.Obtains pre-certification and authorization.

One (1) year of related hospital, medical office, or customer service experience preferred. Other Information: In Memorial Physician Group (specialty practices), additional responsibilities include: (1) obtain specialty authorizations (2) authorization denial and peer to peer process (3) patient care navigation ex: surgical and procedural coordination and scheduling for patient specific populations (4) handle all incoming calls and physician and hospital back line (5) obtain and confirm referralsIn the Hospital, additional responsibilities include: (1) Upon validation of patient identity, place identification band on patient (2) obtain signatures for hospital specific regulatory forms not required in an ambulatory or office setting (3) obtain authorizations for walk-in appointments (4) determine when financial assistance is needed.In Memorial Primary Care, additional responsibilities include: (1) MIH-MPC program patient referral, payment collection and eligibility scheduling (2) process referral work-ques and same day access requests (3) work with Patient Access Center on real time patient requests (4) address prescription refill requests, patient advice requests through MyChart, and provider scheduling template.

The position requires a commitment of three days per week, with 12-hour shifts from 6:45 PM to 7:15 AM.

Summary:
The Utilization Management RN uses evidence-based medical necessity screening tools to complete initial and continued stay reviews in determining appropriate level of patient care, appropriateness of tests/procedures, and an estimation of the patient’s expected length of stay. Proactively identifies and resolves barriers to timely discharge/transition and documents avoidable delays information in accordance with health system protocols.Conducts an in-depth case management assessment of a patient’s needs at the time of admission and throughout the patient’s stay. Where You Will Work:Â
26 bed post op surgical unit, with 16 bed of telemetry available. FAST PACED post op surgical unit, caring for patients with Gastroenterology, Urology, Gynecology, and Plastic Surgery populations, abdominal drains, wound VACS’s, PCA’s, epidurals, tube feedings etc. 75% of patients are admitted from PACU or as stepdown from Surgical ICU.
Manages the care of 4-5 patients on day and PM shift and 5-6 patients on the night. 3EF can manage patients on the following drips: insulin, ketamine, dobutamine, heparin, angiomax, etc.  This is a great unit for experienced Med-Surg nurses seeking to advance their skills and grow in their practice. It is also a great foundational unit for new graduate nurses who seek to eventually develop into more advanced clinical settings, such as ICU, ED, OR, and NP.  We are a top performing inpatient unit as measured by their quality outcomes. With one of the highest rates for certified nurses in the hospital, 3EF nurses demonstrate their lifelong commitment to excellent clinical practice and patient outcomes. Shared Governance is engrained within the culture on the unit, where nurses and nursing assistants have a strong voice in the daily operations. 3EF received top team member engagement scores in 2021.

Minimum Qualifications Â
Master’s degree in divinity, Theology, or a related field from an accredited institution.

This is a remote position. Â Acts as the first point of contact for customers reaching out to Advocate Aurora through established Patient Access & Care Team (PACT) services for both internal and external partnerships. Supports base PACT initiatives and services accurately documenting each interaction using the electronic medical record. This position begins with three weeks of MANDATORY training (you must be available for the entirety of training). Training hours are from 8am to 5pm CST / 9am to 6pm EST for Weeks one and two; 8am to 4:30pm CST / 9am to 5:30pm EST for Week three.

Uses protocols, facility/provider information, and established policies and procedures to seamlessly link the patient experience between the Patient Access & Care Team (PACT) and the practice site.
Uses resources and critical thinking skills to assist inbound callers. Makes outbound calls when required or when follow up is deemed necessary.
Has the ability to recognize complex non-clinical problems and questions and escalates for resolution when needed.
Responsibilities include scheduling patient appointments and coordinating cancellations, reschedules, and additions to schedules. Major Responsibilities:
- Evaluates, diagnoses, treats, and manages acute, critical and chronic patients illness and diseases within both an outpatient clinic and inpatient hospital setting.

The Ambulatory Social Work Case Manager has responsibility to provide care/service safely and efficiently for a full range of services to patients of all ages and their families. Primary role is to collaborate, communicate and facilitate coordination of services as established by the healthcare team and executed by the case manager. Responsible for the psychosocial assessment of patients, coordination of care and linkage to community resources and providing emotional support to patients and their families. Ensure appropriate decision makers are informed of and involved in treatment planning. Apply pertinent state and federal regulations regarding documentation and reporting requirements.
The Social Work Case Manager identifies, monitors, and reports opportunities for quality and performance improvement to the appropriate department. The Social Work Case Manager takes an active role in performance improvement activities as it relates to their area of assignment.
Responsibilities and Essential Job Functions
- Accepts responsibility and accountability for achievement of optimal outcomes within their scope of practice. Follows policies, procedures, and standards; complies with Corporate Compliance program. Assumes responsibility for risk and safety issues associated with the position. Takes call as required by the department expectations. Performs job specific responsibilities and demonstrates accountability for own actions and decisions.
- Acquires and maintains knowledge and competence related to the expectations of their position and practices within their scope. Brings ideas and concerns to supervisor, participates in department decision making. Maintains current licensure.
- Completes psychosocial assessments of patient/family situations including social, psychological, emotional, financial and other related factors to facilitate patients’ linkage to resources to support care in the community.Identifies and utilizes all relevant information (medical/nursing needs, social work knowledge base, disease process, knowledge of community resources) to accurately and thoroughly assess the patient’s psychosocial situation.
- Evaluates psychosocial and medical/nursing information to determine an appropriate social service action/ plan of care.Utilizes social work assessment and input from other team members to formulate realistic recommendations for social work action plan and linkage to resources
- Participates in interdisciplinary team meetings as needed.
- Initiates and participates in family conference to determine psychosocial and community resource needs.
- Participates in Care Team meetings by providing relevant and discipline specific information to the entire healthcare team and coordinating any linkage to services on identified areas.
- Advocates on behalf of patients and caregivers for identification and access to services. Advocates for the protection of the patient's health, safety and rights. Ensures patient choice and consistently supports a patient centered environment.Provides supportive counseling to assist patients/families in adjusting to disability and illness, and for realistic planning for care in the community.
- Demonstrates a caring, positive regard for others by clarity of speech, use of understandable terminology and utilizing active listening skills.
- Assures prudent utilization of all resources (fiscal, staff resources, environmental, equipment and services) by evaluating the options available. Demonstrates ability to balance cost and quality to assure the optimal clinical and financial outcomes.

Graduate of a Board of Nursing approved nursing education program.Â

Summary:
The Patient Financial Services Representative (PFSR) serves as the first point of contact in greeting patients and guarantors in the hospital, ambulatory or medical office setting. The PFSR engages with the patient or guarantor to obtain pertinent information and answer any questions in an effort to ensure that all required demographic, financial, and insurance eligibility information is gathered and verified. Ensures all required notices and consent forms are signed accordingly.
Responsibilities:
May confirm physician and prescription orders ensuring accuracy.May schedule walk-in appointments for services offered.Provides exceptional customer service and ensures all questions and concerns are addressed in a timely and courteous manner. May guide the patient to appropriate destination for services.Collects patient out-of-pocket responsibility per collection guidelines. Provides patient estimates as requested. Prepares and balances a daily deposit of all payment collections.Obtains pre-certification and authorization.Verifies insurance benefits including obtaining insurance card(s) and confirms coverage is active. Determines correct insurance filing order, if multiple insurance coverages are effective for that service.Explains polices including all regulatory and financial consent forms; secures all required signatures.May perform patient discharge functions including, but not limited to, review of after visit summary (AVS), future appointment scheduling, and referrals.Interviews patients and guarantors at the workstation or bedside to obtain all necessary information, including a copy of the patient or guarantor identification card.
