Responsibilities
Complies with facility policies and procedures for appropriately documenting, billing and coding for residents receiving speech language pathology and audiology services.
Provides resident information needed for the resident assessment instrument according to facility policy such as signs and symptoms of swallowing disorders, speech and hearing patterns, cognitive patterns, Brief Interview for Mental Status (BIMS), speech language pathology and audiology minutes and distinct calendar days, etc. during the defined observation period(s).
Provides resident information needed for prior functioning in functional cognition as required by facility policy (RAI User’s Manual) for Medicare Part A, Medicare Advantage residents upon admission.
Required Qualifications:
Master’s Degree, or equivalent, in Speech Language Pathology
A valid, unrestricted license in his/her therapy discipline in the state.
Knowledgeable of skilled nursing home regulations, procedures, laws, regulations and guidelines pertaining to long-term care.

*Position Overview*
We are seeking a passionate and motivated Pediatric Speech-Language Pathologist (SLP) to join our team. The SLP will work with children under the age of 8, including those on the Autism spectrum, to develop and implement individualized speech and language goals in a play-based preschool setting.

*Job Summary*
We are a community-focused early childhood non-profit—not a high-volume private practice or corporate clinic. The Speech-Language Pathologist (ST) provides evaluation, assessment, and therapeutic intervention services to children from birth to 12 years old within Early Childhood Education, at our Center, in the community, and in home-based settings. This role involves assessing communication needs, developing and implementing individualized treatment plans, delivering direct services, and collaborating with families and interdisciplinary team members.
The ST is responsible for maintaining accurate clinical documentation and communicating progress to caregivers and relevant providers. With manageable caseloads, flexible scheduling, and a supportive interdisciplinary team, this role offers the chance to make a lasting and measurable impact in a mission-driven organization committed to equity, inclusion, and whole‑child development.
*Essential Duties & Responsibilities*
* Conduct speech-language screenings and comprehensive evaluations using standardized assessments, informal measures, and clinical observation.
* Identify speech, language, social communication, voice, fluency, and early feeding concerns across developmental stages, with a primary focus on early childhood.
* Develop and implement individualized Plans of Care aligned with IFSPs, IEPs, and ECE developmental standards.
* Utilize play-based, developmentally appropriate, and neurodiversity-affirming intervention strategies.
* Monitor progress toward goals and adjust treatment plans as needed.
* Participate as an active member of multidisciplinary teams, including IFSP meetings, parent-teacher conferences, and transition meetings.
* Collaborate with teachers and caregivers by offering guidance, strategies, and ongoing communication to support communication-rich learning environments and promote skill carryover at home and in the community.
* Maintain accurate, timely documentation including evaluations, treatment plans, progress notes, and discharge summaries.
* Meet productivity expectations and documentation timelines in accordance with internal policies, payer requirements, and program procedures.
* Utilize Fusion electronic health record system for scheduling and therapy documentation.
* Ensure compliance with HIPAA regulations, ASHA ethical standards, and state licensure requirements.
* Follow established office routines, including managing voicemails, emails, letters of medical necessity, and maintaining clean and organized therapy spaces.
* Attend required meetings, including Therapeutic Services meetings, staff meetings, Educational Services team meetings, and other meetings as assigned.
* Participate in required training and ongoing professional development aligned with TLC’s philosophies, policies, and procedures.
*Minimum Qualifications*
* Master’s Degree in Speech-Language Pathology from an accredited program.
* Current Colorado Speech-Language Pathologist license verified through DORA, or eligibility for licensure.
* ASHA Certificate of Clinical Competence (CCC-SLP), or in the process of obtaining (Clinical Fellows considered based on supervision availability). Strong Clinical Fellows (CFYs) may be considered depending on supervision availability.
* CPR and First Aid certification (or ability to obtain upon hire).
* Proficient with computers and digital tools, including electronic health records, documentation software, and basic office programs, and able to effectively use technology to communicate with families, educators, and team members.
*Preferred Qualifications*
* Working with IEPs, assessments, and collaborating with teachers/staff.
* Experience working with pediatric and school-age populations with a range of developmental, communication, and learning needs.
* Training or experience in play-based, routine-based, and caregiver-coaching models of intervention.
* Knowledge of early childhood development and use of developmental frameworks (milestones, functional communication, regulation, social-emotional development).
* Spanish language proficiency and/or knowledge of American Sign Language (ASL).
* Experience collaborating within an interdisciplinary team (OT/PT/ECSE/BCBA/mental health, teachers, etc.).
*Physical Requirements*

DEPARTMENTAL SUMMARY: The Rehabilitation Department provides therapy and nursing services to patients, families and patient’s caregivers including: Rehabilitation Nursing Care, Physical Therapy, Occupational Therapy and Speech Language Pathology. Our department operates as a system department and provides services to Acute Care, Inpatient Rehabilitation Facility, Home Health and Outpatient settings across four campuses. Our Rehabilitation departments offer a variety of specialty services across the various services delivery areas.
POSITION OVERVIEW: The Occupational Therapistevaluates, plans, and provides treatment and rehabilitation as prescribed by a referring physician. Works with family and team members regarding patient’s abilities and goals.
This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES: RESPONSIBILITIES:
Adheres to Oregon State Occupational Therapy Licensing Board.
Provides occupational therapy evaluation and treatment to achieve desired outcomes.
Provides staff consultation and training.
Provides community resource knowledge and referral services.
Provides occupational therapy services to other hospital service areas.
May be required to drive Rehabilitation van and meet all related SCHS vehicle policy requirements.
Functions as a member of the Interdisciplinary Care Team to assure patient care is accomplished effectively and efficiently in a cost effective manner.
Participates in clinical care conferencing.
Gathers data and accurately documents information in a timely manner demonstrating compliance with departmental and professional regulations.
Participates with team members in facilitating patient’s and family’s learning throughout the continuum of care. Reinforces patient’s continued health care through teaching and/or referral to community agency follow-up.

EDUCATION:
Required: Bachelors, Masters, or Doctorate degree in Occupational Therapy from a college or university accredited by the American Occupational Therapy Association

LICENSURE/CERTIFICATION/REGISTRATION:
Required: Must have necessary requisites for licensure as an Occupational Therapist by the Oregon State Board of Health. Valid Oregon drivers license and ability to meet St. Charles Health System driving requirements. Ability to travel to business functions/trainings/meetings and all St. Charles Health System worksites. Current AHA Basic Life Support for Healthcare Provider certification.

EXPERIENCE:
Required: Must have one year of experience with therapy procedures, current orthopedic and neurological follow-up procedures, and rehabilitation practices.
Preferred: Previous occupational therapy experience in a hospital based setting.

Responsibilities
● Conduct comprehensive assessments of residents' functional abilities, including activities of daily living (ADLs), cognitive status, upper extremity function, and safety awareness, within The Victorian.
● Develop and execute tailored care plans that incorporate evidence-based occupational therapy techniques, such as ADL retraining, therapeutic exercise, cognitive-perceptual retraining, and adaptive equipment training, specifically adapted for geriatric populations and those with various age-related conditions.
● Provide hands-on therapy sessions to improve resident outcomes in areas like geriatrics, orthopedic rehabilitation, neurological rehabilitation, low vision, and post-surgical recovery.
● Collaborate with an interdisciplinary team including nurses, physical therapists, speech therapists, and facility staff to coordinate care, optimize resident well-being, and facilitate seamless transitions on and off therapy.
● Educate residents and their families on treatment strategies, fall prevention, adaptive equipment and assistive device use, energy conservation techniques, and home exercise programs to support ongoing recovery and promote independence within their living environment.
● Maintain accurate and timely documentation within electronic medical record (EMR) systems, including initial evaluations, progress notes, treatment plans, and discharge summaries, adhering to all regulatory guidelines.

Skills
