$54.90 - $82.35Major Responsibilities:
Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens
Uses effective patient education techniques to provide counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration
Designs, recommends, monitors, and evaluates patient specific pharmacotherapeutic regimens based on relevant biomedical literature. Implements recommendations effectively and utilizes policies and procedures and documents in the appropriate format.
Reviews and accurately interprets medication orders and prescriptions
Participates in the management of medical emergencies when applicable
Actively supports department medication safety and quality improvement activities, including the reporting of events through appropriate channels
Performs quality assurance and performance improvement of the medication use system as appropriate
Collaborates with the healthcare team to manage formulary adherence and the medication-use process. Assists in the acquisition of non-formulary, compassionate use, and investigational medications.
Practices in compliance with organization’s policies and procedures, as well as local, state, and federal regulations
Prepares, dispenses, and oversees the proper storage and handling of medications in accordance with best practice, policies and state and federal regulations
Serves as preceptor for pharmacy students, residents, and healthcare learners as assigned
Mentors, orients, trains, and assists with the performance evaluation of pharmacy teammates, residents, and students as assigned
Directs and checks the accuracy of the work of pharmacy support team members in accordance with the organization’s policies and procedures to ensure safe and efficient workflows, service standards, and regulatory compliance
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
Licensed in the state(s) of practice or eligible for reciprocity required
Immunization and non-vaccine injectable certification required based on area of practice
Credentialing and privileging required based on area of practice
Education Required:
Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP

Physical Requirements and Working Conditions:
Requires considerable walking, standing, pushing mobile cart of 50 pounds maximum and lifting 35 pounds of material without assistance
Exposed to various conditions throughout the workday such as walking on hard surfaces, climbing stairs and changes in temperature and humidity
May be required to frequently stretch, bend, squat, or kneel to perform job
Utilizes appropriate personal protective equipment (PPE) to minimize exposure to hazardous agents
Takes necessary precautions to avoid inadvertent needle sticks, minor cuts or bruises
Must be able to perform manipulative skills such as writing, typing, and data entry into the computer and automated dispensing machines
Must have use of both hands, be able to see clearly and read small font such as that on unit-dose medication packages and syringes
Must have functional speech, vision, and hearing
Operates all equipment necessary to perform the job
Exposed to normal office environment

- The FNE/SANE nurse will conduct a forensic examination of the patient, cataloging evidence of physical or sexual abuse that may be used in a court case or police proceedings. Additionally, the FNE/SANE nurse will conduct a forensic interview, asking the victim about the incident(s) of abuse.
- During the forensic examination, the FNE/SANE nurse may be required to collect specimens and DNA samples, take photographs of bruises and other injuries, and conduct a detailed physical and genital examination based on patient preference.

- Bariatric surgery (Gastric Sleeve, Gastric Bypass – not all patients are bariatric)
- Vascular surgery (CEA, AAA repair, revascularizations, thrombectomies, amputations)

$54.90 - $82.35This teammate will be responsible for assisting the multidisciplinary team with medication therapy selection and management in areas including but not limited to trauma, medical resuscitations, and medical emergencies in STEMI, stroke, PE, and sepsis. The candidate will work with the clinical team leader with protocol development and education to the medical staff as necessary. The candidate will be expected to perform limited preparation and verification of medications in high acuity situations.
Major Responsibilities:
Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens
Uses effective patient education techniques to provide counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration
Designs, recommends, monitors, and evaluates patient specific pharmacotherapeutic regimens based on relevant biomedical literature. Implements recommendations effectively and utilizes policies and procedures and documents in the appropriate format.
Reviews and accurately interprets medication orders and prescriptions
Participates in the management of medical emergencies when applicable
Actively supports department medication safety and quality improvement activities, including the reporting of events through appropriate channels

Collaborates with the healthcare team to manage formulary adherence and the medication-use process. Assists in the acquisition of non-formulary, compassionate use, and investigational medications.

Position Summary and Purpose
The Rehab Technician is a non- licensed caregiver who works under the direction of a Physical Therapist, Occupational Therapist, Speech Therapist, and/or Assistant to aid in the provision of patient care. The technician will be involved with patient preparation, components of certain interventions, and post-treatment procedures. Specific tasks will be determined by the specific therapy provider, Physical Therapy (PT,) Occupational Therapy (OT,) or Speech Language Pathology (SLP). The Technician attends to the daily tasks of departmental organization and upkeep. The Technician will be expected to perform various clerical, regulatory, and environment of care duties on a regular basis. The rehab technician may serve as an office coordinator or liaison when requested by the department leader.
Essential Functions:
• Administers components of interventions (therapeutic exercise, therapeutic activity, gait training, activities of daily living); specific functions and tasks will be determined by the therapy provider (PT, OT, or SLP)
• Performs setup for exercise, evaluation, or modality equipment
• Assists in transfer and/or transport of patients, as needed
• Take vital signs, as needed
• Assists therapist with initiation and monitoring of patients on available therapy and exercise equipment within the rehab department
• Monitors departmental supply level
• Performs clerical duties (answers phones, scheduling, data entry, filing)
• Reports any and all status changes regarding patient to supervising Therapist or Assistant
• Maintains organization and cleanliness of all patient care equipment and treatment areas
Other Functions:
• Initiates orders for repairs and routes non-functioning equipment for repair

Summary:
The RN Case Manager coordinates the care and service of patient populations from admission through discharge. The RN Case Manager, collaboratively with inter-disciplinary teams, works to build a comprehensive case management plan through effective care coordination and utilization of healthcare resources to achieve desired clinical and financial goals. Key responsibilities are to partner with the healthcare team to ensure all aspects of the patient’s needs, clinical, psychosocial, and financial are adequately addressed in the transition of the care plan and to manage the patient’s timely progression of care and safe transition to the next most appropriate level of care.

Monitors patient and family satisfaction. Responds to questions and complaints from patients, family members, and payers regarding care.Performs duties in a manner that promotes quality patient care/satisfaction, while promoting safety, cost efficiency, and a commitment to the continuous quality improvement process.Addresses system-level issues impeding diagnostic or treatment progress with the healthcare team and reports unresolved opportunities for improvement through the organizational defined escalation process (chain-of-command structure). Proactively identifies and resolves barriers to timely discharge/transition and documents avoidable delays information in accordance with health system protocols.Conducts an in-depth case management assessment of a patient’s needs at the time of admission and throughout the patient’s stay. Obtains and confirms information necessary for the development of a comprehensive discharge/transition plan of care.Clinically assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual’s health needs to facilitate a safe and timely discharge to the next appropriate post acute level of care.Collaborates with the Social Worker to proactively identify the need for team and/or patient and family conferences to facilitate discussion of the patient’s condition, discuss prognosis and determine an agreed upon transition of care plan.Consults with Physicians and multidisciplinary teams regularly to evaluate the patient's status and appropriateness of medical care, including admission, length of stay (LOS), transfer, and discharge.Performs ongoing chart review to identify actual or potential issues, which may include service delivery, patient outcomes, satisfaction, compliance, cost, and reimbursement.Actively coordinates progress and the patient’s care by monitoring the length of stay (LOS) of the patient’s hospitalization, leads and facilitates rounds, and proactively works to meet expected length of stay and clinical targets/indicators.

The Patient Financial Services Representative (PFSR) serves as the first point of contact in greeting patients and guarantors in the hospital, ambulatory or medical office setting. This department also provide cares to OB patients with conditions such as hypertensive crisis, pulmonary hypertension and metabolic exacerbations.

This is a 16 beds, Medical Respiratory ICU providing state of the art care to patients with diagnosis such as: COPD, CHF, metabolic exacerbations and sepsis. This department also provide cares to OB patients with conditions such as hypertensive crisis, pulmonary hypertension and metabolic exacerbations.

Aurora Medical Center Summit NICU has 6 semi-private rooms and 1 isolation room for newborn patients born at 32 weeks gestation and above. Provide care for infants at 32 weeks of gestation or later weighing 1500g or more who have physiologic immaturity or who are moderately ill with problems that are expected to resolve rapidly and are not anticipated to need subspecialty services on an urgent basis. Provide mechanical ventilation for brief duration (less than 24 hours) or continuous positive airway pressure or both.

Focuses on the care of the critically ill patient with medical diagnoses and complications which may include respiratory disease, diabetic complications, renal disease and other complex medical conditions.

Essential Functions
- Evaluates patients, selects and administers assessment techniques, synthesizes information to establish appropriate patient care plan to include necessary modalities, procedures, exercise programs, therapeutic groups and patient education as warranted by a holistic view of the patient's diagnosis, condition, age, and discharge needs.
- Gathers and interprets appropriate data from the medical record, physician reports, patient, and/or family as evidenced by documentation.
- Establishes functional goals in conjunction with the patient and provides evidenced based intervention to work towards meeting these goals with ongoing assessment to adjust the plan of care as needed .
- Documents evaluations, tests, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, discharge notes, and charges as needed.
- Communicates patient's evaluation and subsequent progress to physician, referral sources, and other clinical staff, coordinates patient's schedule and treatment programs, and discharge planning.
- Provides supervision of PTA's and Rehab Tech/Aide as required for licensure.
Physical Requirements
Hearing (corrected) adequate for oral/aural communication. Vision (corrected) adequate for reading. Intelligible speech and adequate language/cognitive skills to perform job duties. Sitting, standing, and walking required throughout the day. Job duties sometimes require climbing stairs, kneeling, twisting, bending; on occasion, crouching, crawling and reaching overhead. Lifting of patients, equipment or supplies will be required up to 20 pounds frequently and 50 pounds occasionally. Able to laterally transfer patients 100 pounds rarely. Must be able to demonstrate any appropriate exercise and activities to patients/caregivers. Personal Protective Equipment such as gloves, goggles, gowns, and masks are sometimes required due to possible exposure to hazardous chemicals or blood and body fluids.
