Memorial Rehabilitation Institute provides highly specialized care ranging from complex multiple trauma, stroke and brain injury to physical and speech therapy. Inpatient care is offered at Memorial Regional Hospital South, with outpatient care available in five different locations. We also have a home health team and resources for long-term care at Memorial Manor, while also offering therapy for pediatric developmental diagnosis through Joe DiMaggio Children’s Hospital.
Summary:
Evaluates and treats patient populations for physical therapy needs.
Responsibilities:
Supervises activities of physical therapy assistants, students and other support personnel.
Designs and implements a plan of care which is appropriate to issues identified, required frequency of treatments, and involves the patient and family.
Educates and counsels patients and families regarding treatment plans and progress. Provides information regarding appropriate selection and use of adaptive equipment and support resources.
Performs and documents initial and ongoing assessments of patient's condition.
Plans or assists with patient discharge from therapy services.

Summary:
Performs administrative/clerical duties in support of an assigned nursing area.
The position requires a commitment of three days per week, with 12-hour shifts from 11:00 AM to 11:00 PM.
Responsibilities:
Greets patients and visitors. Monitors, answers, screens, and routes all telephone calls. Emergency alerts, pages, and overhead announcements as requested.Performs needed clerical tasks, including copying, faxing, filing, and ordering/obtaining supplies.Assists with processing patient admissions, transfers and discharges. Responsible for identifying and coordinating patient/family care to support terminally ill patients and families in all hospice settings. Frequency of patient / family contacts will be at the discretion of the Case Manager and his/her assessment of need, but will be a minimum of every 15 days. The Case Manager utilizes teaching, assessment, and intervention skills to provide comfort care at end of life and maximize the quality of life for the patients and families.
Educates patients and families regarding disease processes and trajectory, disease management, hospice philosophies, signs and symptoms of decline and end of life. Updates the primary physician as indicated and according to agency, accreditation, state and federal requirements.
Responsible for continuous review of all aspects of every patient of her/his caseload to include appropriate utilization of services and the appropriate hospice level of care criteria are met. Makes referrals to other team members as appropriate.
Administers medication and treatments as prescribed by the physician.
 Leads the interdisciplinary care group (IDG) specific to each patient and family on their caseload. Working within the IDG to develop, revise, and implement the patient’s Plan of Care. Coordinates all services and collaborates with members of the interdisciplinary team to provide consistent patient care. Arranges for and/or recommends equipment, medical supplies and other necessary items and services as indicated.
Enters, completes and updates documentation in the electronic health record as required according to agency/department standards and policies, accreditation, state and federal guidelines.
Documentation accurately records the patient’s experience with a combination of assessments and narrative  .
Provides on-going assessment of patient response to treatments, medications and teaching while making appropriate changes in interventions and follow-up to meet patient needs. Updates and revises POC as indicated with supportive documentation to changes. Communicates problems and changes in condition to physicians, supervisor and other members of the IDG. Enters verbal orders from the physician or APN into the Electronic Health Record.
Supervises and performs Home Health Aide and licensed practical nurse supervisory visits according to agency standards. Instructs paraprofessional staff on a timely basis and in accordance with agency, state and federal regulations.
Facilitates the discharge planning process according to agency standards and utilizes interagency/system and community resources to assure continuity of care after discharge.
WI ONLY: May conduct OASIS assessments for Palliative Care patient and establish and update the plan of care. Summary:
The Patient Financial Services Representative (PFSR) serves as the first point of contact in greeting patients and guarantors in the hospital, ambulatory or medical office setting. The PFSR engages with the patient or guarantor to obtain pertinent information and answer any questions in an effort to ensure that all required demographic, financial, and insurance eligibility information is gathered and verified. Summary:
The Surgical Technologist Cardiovascular Operating Room (CVOR) Trainee I facilitates the safe and efficient performance of surgical procedures. May operate independently towards the end of training on certain cardiovascular procedures.
The CVOR Surgical Technologist Training Program at Memorial Regional Hospital prepares you for a specialized, high‑impact role in cardiovascular surgery. Performs periodic PAR level replenishment duties using various resupply procedures to maintain stock levels. Major Responsibilities:
- Evaluates, diagnoses, treats, and manages acute, critical and chronic patients illness and diseases within both an outpatient clinic and inpatient hospital setting.
- Obtains medical histories, conducts physical examinations and assesses and determines differential diagnosis. Prescribes medication, therapy and treatments accordingly.
- Interprets and evaluates patient data for the purpose of determining management and treatment of patients. Performs minor surgeries and routine procedures. Consults with physician as appropriate. SUMMARY:

MAIN DUTIES AND RESPONSIBILITIES:
- Available to cover shifts as assigned at any residential program, to include leaving one program to travel and work at another.
- Facilitate ADL skill-building for residential living and for community integration through supervising and teaching clients daily medication routines, budgeting, self-preservation skills, transportation, etc.
- Provide opportunities for skills practice and collaborative learning.
- Participate on a multi-disciplinary team in the development and implementation of client treatment plans and progress reviews, engaging clients whenever possible in the planning and evaluation process.
- Provide, coordinate and monitor treatment services for all assigned clients. Includes but not limited to contact with the client, as prescribed in the treatment plan; provision and coordination of care prescribed in the treatment plan through documented contact and implementation of assigned activities with client, client’s family, other Gateway staff, consultants and treatment plan designated staff from other human service agencies and medically oriented organizations.
- Monitoring the effectiveness of care provided by the program or through other service providers, by comparing service provided with service outcomes, as specified in the treatment plan.
- Provide shift coverage which includes but is not limited to carrying out the many functions of household maintenance, especially cleaning and meal preparation, assist in resolution of medical, psychiatric and behavioral crises, conflict resolution and symptom management.
- Attend supervision and team meetings as required.
- Maintaining current knowledge and updates of agency policies and procedures.
- Available to work first and second shift, holidays and weekends.
- Ensure Gateway office areas, residential programs are safe and secure via checking door and window locks and by knowing the whereabouts of all residence at all times during course of work shift.
- Schedule treatment plan meetings, facilitate treatment plan meetings, utilize assessment information, diagnostic results, medical record, client and family participation and participation of other human service providers to develop and document assessments and treatment plans/reviews in accordance with agency policy and procedure.
- Ability to screen for substance abuse and provide assessment with indicated treatment planning, counseling/education and referral.
- Performs other duties as assigned or needed.
- Ability to remain in shift until appropriately relieved by another staff member. (This could result in working two consecutive shifts as designated by supervisor.)
MINIMUM QUALIFICATIONS:
- High school diploma or GED required.

Summary:
The Patient Financial Services Representative (PFSR) serves as the first point of contact in greeting patients and guarantors in the hospital, ambulatory or medical office setting. The PFSR engages with the patient or guarantor to obtain pertinent information and answer any questions in an effort to ensure that all required demographic, financial, and insurance eligibility information is gathered and verified. Ensures all required notices and consent forms are signed accordingly.
Responsibilities:
Provides exceptional customer service and ensures all questions and concerns are addressed in a timely and courteous manner. May guide the patient to appropriate destination for services.May confirm physician and prescription orders ensuring accuracy.Obtains pre-certification and authorization.May schedule walk-in appointments for services offered.Verifies insurance benefits including obtaining insurance card(s) and confirms coverage is active. Determines correct insurance filing order, if multiple insurance coverages are effective for that service.Collects patient out-of-pocket responsibility per collection guidelines. Provides patient estimates as requested.
