The APP will be expected to manage medical and surgical ICU patients and is encouraged to practice at the top of their license. Essential functions: obtain a medical history, physical exam, document findings, and prepare a plan of care in a timely manner including ordering medications and appropriate tests/procedures. Attend, participate in and contribute to group/practice meetings, development of policies and procedures, and quality improvement initiatives.

- Current team consists of 50 physicians, 30 critical care RNs, 6 Health-care assistants.
- Our virtual critical care program provides 24/7 coverage via a shift-based model to 18 different facilities across Illinois and Wisconsin. APPs will cover WI units only.
- Critical care services provided by using two-way audio video communication, real-time bedside waveform monitors, and integrated EMR for review of documentation, labs, vitals, and diagnostics.
Schedule:
- Shifts are available in 5-hour and 8-hour durations and include evenings, overnight coverage, weekends, and holidays.
- Advanced Practice Providers (APPs) must be Acute Care Board Certified.
- Candidates must be a Nurse Practitioner (NP) or Physician Assistant (PA) with a minimum of three years of recent critical care experience practicing as an NP or PA.
- Applicants must be actively working in a critical care setting and demonstrate strong clinical expertise in the management of critically ill patients.
- Will require WI & IL licensure
Pay Range:
$54.90 - $82.35MAJOR RESPONSIBILITIES
Clinical responsibilities:
Assesses, diagnoses, and determines/alters treatment and management plans appropriate for age, acuity and clinical condition. This includes ordering, performing, and interpreting appropriate diagnostic studies and prescription of pharmacologic and non-pharmacologic interventions and therapies.
Manages conditions based on clinical indication, evidence-based care, cost effectiveness, and assessment of risks/benefits and alternatives.
Provides health promotion, disease prevention and disease management counseling and education of patients and families.
Manages patients as part of an interdisciplinary team and within scope of practice. Seeks physician or other healthcare team member consultation or referrals as appropriate. Escalates need for more emergent or specialized care when necessary.
Maintains accurate, complete, concise, and timely documentation in the electronic medical record. Substantiates and submits professional services consistent with compliant coding and billing practices.
Facilitates consistent, coordinated care and clear communication among all members of the healthcare team and/or health or community agencies.
Performs office or hospital procedures in accordance with specialty practice, competency and granted privileges (where applicable).
Other responsibilities:
Participates in quality, safety, and peer review initiatives/performance activities, organizational and/or departmental meetings and committees, peer review, and workgroups as necessary.
Participates in education and/or onboarding of new team members, students, and other health care professionals.
Seeks experiences to maintain and develop clinical and professional skills and advance the profession within and outside of the organization.
Maintains standards of productivity, access, face-to-face time, and quality metrics to ensure optimal, safe and timely patient care delivery.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
MINIMUM EDUCATION AND EXPERIENCE REQUIRED
License/Registration/Certification Required:
Active RN, and APRN/APRN-FPA/APNP or other APRN license(s) in state(s) of practice, AND
Active national board certification in area of clinical practice and populations served, AND
Active DEA registration prior to hire, AND
If Illinois practice: active Illinois Controlled Substance License prior to hire, AND
Active BLS and/or ACLS, PALS, NRP as required by clinical practice prior to or within 6 months of hire
Education Required: Master’s Degree in Nursing

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

Must be able to:

Lift up to 20 lbs over the head.

$19.80 - $29.70Major Responsibilities:- Conducts regular interior and exterior patrols to ensure the safety and security of the property. Inspects exit doors, offices, storerooms, and departments for security and the absence of fire or other hazards. Monitors and reports suspicious activity on hospital property and surrounding area.
- May be assigned to a stationary post location within the facility to offer a visible deterrent in a designated area for visitor control.
- Assists patients and visitors by providing information and directions in the facility.
- Performs routine lost and found pick-ups as needed.
- Performs foot patrol of various parking lots ensuring adherence to parking policy. May assist in the coordination and monitoring of Contracted Security, specific to parking.
- Facilitates the issuance of police citations and/or towing services as needed. Issues necessary parking violations notices as required. Maintains database of violations on a 12-month sliding scale.
- Ensures traffic control, monitors conduct of visitors on premises and reports safety hazards and security vulnerabilities to LPS Sergeant.
- In extreme cases of need, the security aide may be required to perform a medical standby in cases that do not rise to the level of needing a security officer.
Licensure, Registration, and/or Certification Required:
- A valid drivers license issued by the Division of Motor Vehicles.
Education Required:
- High School Graduate.
Experience Required:
- Typically requires 1 year of experience in public contact.

Summary:
The Patient Navigator is responsible for guiding patients through the complexities of the healthcare system. Working collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care continuum for all assigned patients, the Patient Navigator ensures that safety, best practice, and high quality standards of care are maintained across the continuum. The Radiation Chief Therapist supervises staff, staff development, technical procedure development and administrative duties. Has a broad knowledge of radiation oncology which ensures high clinical standards for excellent patient care in accordance with applicable scope and standards of practice in conjunction with policies, values, and mission of the organization.
Responsibilities:
Performs Administrative Duties including radiation therapist team meetings by scheduling meetings, developing agendas, taking notes as needed to facilitate post-meeting review of pertinent items with leader. Manages patient Activity Capture for appropriate patient procedure charges and as needed facilitates review of issues/errors with leader. Meets with leader to review department charges are up to date with CMS. Is assigned projects throughout the year to manage and report to leader for review.Runs and monitors Environment of Care to ensure all simulation and treatment units have a safe environment for patients and staff. Ensure staff is educated on par level stocking of patient equipment and supplies in the technical areas. Reviews and discusses patient treatment issues with physician(s) and/ or physics. Stays abreast of new technical techniques and technology to make recommendations to Department Leadership. Troubleshoots, resolves, and/or reports equipment malfunctions, clinical workflow issues, and staff issues, as necessary. Ensures regulatory safety and radiation safety compliance with the therapist team and contributes to The Joint Commission (TJC) survey, American College of Radiology (ACR) survey and any other assigned to maintain department ready for accreditation.Creates the radiation therapists work schedule to coordinate with patient simulation and treatment units to ensure adequate staffing for efficiency and overtime. Acts as a liaison between the different work groups in the department (Physician/APP, Physics/Dosimetry, RTT, RN/MA, Leadership, etc.) to ensure prompt response to all clinical issues.As needed fills in when a therapist is out-of-clinic to simulate and/or treat patients. Also, as needed, will work with therapists directly to assess clinical operations, competencies, and any clinical issues. For delivering therapy, the Chief Therapist reviews physician orders and radiation therapy treatment plan. Educates patient regarding procedures, equipment and exam to ensure patient's understanding and cooperation. Positions patient and performs imaging/treatment procedure(s). - Assists with organizational marketing efforts by providing associated information and referral to customer, while maintaining appropriate records for documentation. Conducts regular reporting and updating of the provider and marketing databases.

This individual works closely with physical, occupational, and speech therapists and assistants, utilizing treatment modalities, techniques and equipment to provide patient care as directed. Provides on-going services to infant, child, adolescent, adult and geriatric patients and their families based on assignment; service provision is in accordance with discipline specific licensure laws or is provided as a function of a restorative plan that has been developed by a licensed physical, occupational, and speech therapist.
ESSENTIAL FUNCTIONS:
- Under indirect supervision of licensed therapy staff provides restorative treatment plans designed to maintain patient level of functioning in place of/ or is adjunct to other skilled rehabilitation services; carries out, monitors and/or adjusts restorative treatments utilizing therapy techniques, modalities, and equipment for which competency has been demonstrated; assumes responsibility for patient’s care, safety and environment throughout treatment; observes, reports and records all pertinent patient information according to department policy.
- Prepares patient, equipment, supplies (including topical medications used in burn/wound care; does NOT apply medicated supply to patient), and medical record for transport; verifies Rehabilitation Services order in chart; selects technique for transfer and equipment for transport according to established policies and procedures and therapist guidance; assumes responsibility of safe, timely transport of Rehabilitation Services patients and medical records to and from department clinics.
- Works in a collaborative effort with others: answers department telephone, taking complete message or answering questions related to patient schedule; assists in filing and record keeping; monitors patient during exercise.
- Assists in general maintenance of department: Cleans and stores equipment, restocks linens and supplies, changes linens and cleans treatment areas, maintains transport equipment and arranges for necessary repairs, empties soiled linen/waste containers
- Demonstrates appropriate and effective written, verbal and non-verbal communication with patients and their families, colleagues, medical staff, and the public; participates in staff meetings and inservice education; protects patient confidentiality in regard to medical, emotional, and social status.

Functional Role (not inclusive of titles or advancement career progression)
Supports the development, implementation & standardization of system-wide colleague & labor relations strategy, policies, processes & programs throughout the Regional Health Ministries (RHM). In partnership with Colleague & Labor Relations Center of Expertise (COE) leadership, Human Resources Business Partners (HRBPs) & ministry leadership, implements & manages the day-to-day activities supporting colleague & labor relations strategic plans, policies & procedures to achieve Trinity Health's Strategic Aims to realize a person-centered, mission-focused colleague culture. Locally drives & implements the colleague & labor relations strategy through systemic policy alignment & program management to support key stakeholders to proactively cultivate positive colleague & labor relations throughout the RHM, in addition to performing & fulfilling essential colleague relations activities & accountabilities. Works in conjunction Colleague & Labor Relations COE leadership & HRBPs, to provide local direction, operational management & integration of long term & short-term colleague & labor relations initiatives.
Reporting to COE Director & HRBP/CHRO - CR/CLR team metrics & projects for assigned Ministries
Management of CLR team members & daily work
Partners with Ministry HRBPs & CHROs on Ministry initiatives
Triage & assignment of compliance cases
Audits compliance & employee relations entries into case management system
Ensures consistency of disciplinary outcomes
Responsible for assigned Departments & caseload for local Ministry
Participates in local Ministry Executive Team Meetings
Directs local Ministry Executive Team members & partners in sensitive employment transactions
Creates job flows & processes for local projects
Implements System Policies & procedures from Director of CLR
Coordinates with COE Peer Leaders within assigned Ministries
Labor Functions:
Oversees the administration of Collective Bargaining Agreements (CBAs) in alignment with business operations. Manages negation preparation, contract negotiations including Memorandum/Letters of Understanding (MOU/LOU), & arbitration management. Directs Unfair Labor Practice (ULP) & National Labor Relations Board (NLRB) charges in conjunction with direction from Trinity Health Labor Strategy & outside counsel. Manages strike response planning & workforce continuity ensuring effective replacement strategy, mitigation plans, & resources. Facilitates Mutual Interest meetings to strengthen labor-management relationships, promote transparent communication, & proactively address issues before they escalate. Drives Effects Bargaining Meetings through negotiation between Trinity Health management & union leadership. Champions manager education regarding contract terms to ensure consistent application of the CBA. Collaborates with Labor Strategy Team to coordinate & participate in local response strategies & education training of union impact with unionization campaigns, layoff’s, closure of locations, etc. for both leaders & colleagues. Oversees petition response & all aspects of the union election vote management in compliance with labor law.
pay grade 17 range $122,855.2166-$202,711.1074 Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
Minimum Qualifications
Bachelor's degree in Human Resources Management, Business Administration, Labor & Industrial Relations or a related field. Five (5) years of progressive colleague & labor relations experience, including collective bargaining & program development; or an equivalent combination of education & experience. Previous people management or team leadership experience is required.
