Mon - Fri 830-5pm - clinical admin on call rotation 2-4 times a year.

Pay Range:
$44.15 - $66.25Major Responsibilities:
- Promotes an environment conducive to the delivery of quality patient care. Assist with chart audits and assure action is taken. Identifies trends in audits and communicates results to Manager and team.
- Assures field supervisory visits are accurately completed according to agency policy as necessary.
- Provides direction to the staff with clear responsibility for the coordination of resources to efficiently support day to day operations.
- Monitors and assures staff productivity according to established targets on a daily basis. Assures timely correction of charting deficiencies that impact revenue cycle and clinical operations by field team members.
- Participates in the orientation of staff through on-going direct teaching, coaching, observation, review and precepting of staff. Assesses educational needs of clinical staff and assists in development and facilitation of in services for staff. Participates in Field Team Interview/Hiring and Selection. Hires, coaches and terminates direct reporting staff.
- Coordinates and ensures 24 hour availability of field staff services and rotates on call coverage with Manager. In collaboration with the Manager, conducts ongoing process improvement. Conducts tracers/joint visits and provides on-going feedback to staff.
- Ensures accurate, timely and thorough OASIS or Hospice Item set data cycle time.
- Works collaboratively with operational staff and other leadership to ensure growth and maintenance of established patient and referral relationships.
- Establishes and maintains quality standards, continually evaluating workflow and processes.
- Develops collaborative and cooperative relationships with customers, physicians, community organizations and entities of Advocate Aurora Health at Home.
- Performs human resources responsibilities for staff which includes coaching on performance, completes performance reviews and overall staff morale. Recommends hiring, compensation changes, promotions, corrective action decisions, and terminations.
- Responsible for understanding and adhering to the organization's Code of Ethical Conduct and for ensuring that personal actions, and the actions of employees supervised, comply with the policies, regulations and laws applicable to the organization's business.
Licensure, Registration, and/or Certification Required:
One of the following licenses issued by the state in which the team member practices:
• Registered Nurse license
• Physical Therapist license
• Occupational Therapist license
• Speech-Language Pathology license
And:
• A valid driver's license issued by the Division of Motor Vehicles
Education Required:
- Bachelor's Degree in Nursing, or
- Bachelor's Degree in Physical Therapy, or
- Bachelor's Degree in Occupational Therapy, or
- Bachelor's Degree in Speech Pathology.
Experience Required:
- Typically requires 3 years of experience in clinical nursing or rehabilitation.
Knowledge, Skills & Abilities Required:
- Must have thorough understanding of home health or hospice regulations, reimbursement and care requirements.
- Strong communication skills.
- Demonstrates an ability to organize, plan and delegate work effectively an efficiently.
- Demonstrates an ability to resolve conflict, motivate staff and interact with superiors, peers and subordinates in a direct and professional manner.
- Demonstrates an ability to solve problems and make independent judgments and decisions based on experience and good judgment.
Physical Requirements and Working Conditions:
- Exposed to a normal office environment.
- Position requires travel therefore may be exposed to road and weather hazards.
- Operates all equipment necessary to perform the job.

Summary:
Under the general direction of the Chief Therapist, the Radiation Therapist contributes to the organizational plan of the Radiation Oncology Unit, to the quality of patient care and supports the institutional philosophy. The Radiation Therapist is technically competent at the entry level and interacts with other areas of the department. Applies ionizing radiation to the patient, as instructed and prescribed by the physician according to departmental standards, state guidelines, and Memorial Healthcare System’s policy and procedures. As designated, acts as a Charge Therapist and coordinates technical activities and acts as the point person in the absence of the Chief Therapist.
Responsibilities:
Manages patient documentation in Aria for team communication, charge capture and billing reconciliation. Prepares the treatment record information for setup and treatment. Accurately performs weekly chart checks and QA audits at end of treatment through Encounters in Aria EMR. Accurately captures and reviews charges for treatment and simulation procedures.Reviews physician's orders and radiation therapy treatment plan. Educates patient regarding procedures to ensure patient's understanding. Assists patient and family with any problems or concerns and to refers to proper resource personnel. Identifies and reports potential liabilities and risks management to Chief Therapist.Prepares room and equipment properly for each treatment. Performs imaging (CBCT, KV, MV, respiratory gating) as ordered, to position patient. Analyzes results, identifies issues and takes appropriate action including repositioning patient and repeating procedure to deliver therapy according to treatment plan. Performs special procedure treatments (HDR, TBI, SRS, SBRT) and completes treatment documentation.Participates in scheduled rotations in all department equipment, serves as emergency on call therapist, and as needed, rotates between both Radiation Oncology departments. Maintains flexibility and assumes all other additional responsibilities as assigned.Performs CT-Sim procedures and documents accurately. Executes simulations in accordance with department policies and procedures. Prepares room and equipment properly for each simulation and monitors patient. Performs special procedures (4D CT Respiratory Gating, HDR, TBI, SRS, SBRT, Oral and IV Contrast, Urethrogram) and monitors patient for adverse reactions and quickly seeks assistance of a nurse or physician. Completes treatment documentation.Manages patient scheduling in a technical area including simulations, field verifications, daily treatments, and cone-downs to avoid delays and or interruptions.Serves as resource to radiation therapy students by role modeling professional behaviors, instructing, assessing, and evaluating. Stocks and stores appropriate supplies in technical areas. Informs Chief Therapists when supplies are needed. Completes monthly department data logs as assigned. Performs other duties as assigned.Performs appropriate quality control warm-up procedures on equipment. 3CD is a 23 bed post-op surgical unit with 15 beds for Tele Patients. 75% of patients are admitted from PACU or as stepdown from Surgical ICU. This is a fast-paced, high acuity surgical unit specializing in Gastroenterology, Urology, Gynecology, and Plastic Surgery populations. This is a great unit for experienced Med-Surg nurses seeking to advance their skills and grow in their practice. It is also a great foundational unit for new graduate nurses who seek to eventually develop into more advanced clinical settings, such as ICU, ED, OR, and NP. We are a strong performing inpatient unit as measured by their Nurse Sensitive Indicators and Patient Experience scores. 3CD team members are committed to excellent clinical practice and patient outcomes. The Radiation Therapist II contributes to the Radiation Oncology Unit’s operational goals and quality of patient care while supporting organizational standards. Performs imaging (CBCT, KV, MV, Motion Management) as ordered, to position patient. Analyzes results, identifies issues and takes appropriate action including repositioning patient and repeating procedure to deliver therapy according to treatment plan. Performs special procedure treatments (HDR, TBI, SRS, SBRT) and completes treatment documentation. Stocks and stores supplies in technical areas. Advises when supplies are needed.Collaborates with Physicians, Physicists, Dosimetrist, and Chief Radiation Therapist in simulation or clinical setup process. Presents set up questions and solutions while utilizing Crew Resource Management Principles and Practices. Observes established radiation safety measures (ALARA).Performs and documents CT-Sim procedures. Executes simulations in accordance with department policies and procedures. - Patient Evaluation and Care Planning: Interprets medical records and provider referrals as appropriate, and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations utilizing appropriate methods. Summary:
Provides clinical assessment, treatment planning, group, family, and individual counseling, and education services for patients.
Responsibilities:

Join the MercyOne Family! We are looking to hire a Registered Nurse!
As a Registered Nurse at MercyOne, you will provide and direct safe, effective, and culturally competent care for patients with acute medical and surgical conditions. Through evidence-based practice, utilizes critical thinking to assess, plan, implement, and evaluate outcome-based care. The Acute Care RN manages a diverse patient population with varying levels of acuity, including post-operative, medical, telemetry, and neurologic patients. Supervises and coordinates care provided by LPNs, Patient Care Assistants, and other nursing team members. Facilitates communication with physicians, patients and families, and interdisciplinary team members. Adheres to MercyOne’s performance expectations and upholds the Mission, Vision, and Values of MercyOne.
Essential Functions:
1. Conducts an initial assessment to gather data.
- Makes an initial assessment of new admissions within 30 minutes of the patient’s arrival to the unit.
- Completes all areas of the admission assessment database within 24 hours of admission.
2. Identifies patient problems and develops them into an outcome/goal-based plan of care.
- Initiates the patient problem list and incorporates relevant medical history into the plan of care.
- Analyzes and interprets the assessment data to identify desired patient outcomes/goals.
- Determines nursing interventions to achieve those outcomes/goals.
- Prioritizes patient care needs/ activities.
- Identifies and makes appropriate referrals (i.e., Patient/Family Services, Nutritional Services, Spiritual Care, Diabetic Educator, etc.), and education needs.
- Collaborates with other health care disciplines to plan interventions/ patient outcomes/goals.
3. Implements the outcome/goal- based plan of care.
- Assigns and coordinates patient care incorporating patient acuity and the skills, knowledge and abilities of the team members.
- Organizes and prioritizes care for the patients within the team.
- Delegates tasks appropriately to other team members.
- Implements the plan of care including procedures, treatments, physicians orders, nursing orders, education plan and medications.
- Coordinates implementation of plan of care with the Case Manager, Social Worker and other health care disciplines.
- Assists to decrease the patient’s length of stay by helping the patient and family identify and secure appropriate services to satisfy health-related needs upon discharge.
- Uses nursing judgment to determine the appropriate frequency and components of reassessment.
- Reassesses at regular intervals to determine patient response(s) to interventions (minimum RN reassessment every 24 hours).
- Focuses ongoing reassessment on actual and potential problems/needs of the patient including education and discharge needs.
- Participates in develop discharge plan.
- Documents discharge instructions for patient and/or significant other and/or other care provider.
4. Evaluates patient progress and revises the outcome/goal-based plan as needed.
- Evaluates patient response(s) to interventions.
- Review/modifies/revises the outcome-based plan of care based on ongoing reassessments.
- Updates the problem list and outcomes/goals.
5. Communicates relevant clinical information to the team to optimize patient outcomes/goals.
- Communicates relevant clinical information to physicians regarding the patient’s condition.
- Reports progress toward outcomes/goals and/or changes in condition to other health team members.
- Maintains ongoing communication with interdisciplinary team members throughout the shift, at shift change, and transfer of care.
- Communicates with other team leaders and members throughout the shift to reassess and prioritize the needs of the unit.
- Documents completely, comprehensively and legibly and in accordance with policy.
6. Demonstrates clinical competence.
- Utilizes Patient Care Policies and Procedure as references to guide nursing practice.
- Assures medication safety by following the “5 Rights” of medication administration.
- Prioritizes and implements intervention according to patient assessment.
- Demonstrates competency in computer systems and applications required for role.
- Receives, transcribes and processes and implements physician orders per policy including dating, timing and verifying accuracy.
- Maintains communication with the manager/house supervisor and staffing office regarding admissions, unit needs, acuity numbers and significant events.
- Completes assigned shift duties and assures resolution of discrepancies.
7. Manages telemetry and cardiac monitoring when applicable.
- Demonstrates clinical competency and technical knowledge in cardiac rhythm interpretation and telemetry monitoring.
- Recognizes and responds appropriately to cardiac arrhythmias and other changes in patient condition.
- Must be capable of managing multiple patients requiring continuous cardiac monitoring.
- Provides appropriate interventions for patients experiencing cardiac events or changes in rhythm.
- May be required to assist with data collection and quality improvement initiatives.
- Must be willing to serve as a resource to other hospital services (nursing, pharmacy, laboratory, radiology, respiratory, etc.).
- May be asked to participate in unit-based committees and provide education to nursing staff as requested.
8. Based on principles of growth and development, provides care appropriate to the age-related needs of the patients served on his/her assigned unit.
9. Collects, analyzes, interprets and applies age-specific data.
- Performs tasks/procedures, uses equipment in an age-appropriate manner.
- Communicates and works with others in an age-appropriate manner.
10. Adheres to all safety, infection control and colleague health policies and procedures.

- This unit self-schedules. Schedules are 6-week blocks
Minimum Qualifications:
- Associate degree in nursing required
- Bachelor of Science in Nursing (BSN) preferred.
- Current Iowa RN license.
- American Heart Association Basic Life Support (BLS) required.
- American Heart Association Advanced Cardiac Life Support (ACLS) required only if the unit has telemetry monitoring capabilities.
- Proof of completion of Mandatory Reporter – Child and/or Dependent Adult Abuse training within three (3) months of hire.
- High language, intermediate to high math, high critical thinking ability.
- Basic computer skills required.
