Summary:
The Patient Financial Services Representative (PFSR) serves as the first point of contact in greeting patients and guarantors in the hospital, ambulatory or medical office setting. The PFSR engages with the patient or guarantor to obtain pertinent information and answer any questions in an effort to ensure that all required demographic, financial, and insurance eligibility information is gathered and verified. Ensures all required notices and consent forms are signed accordingly.
Responsibilities:
Interviews patients and guarantors at the workstation or bedside to obtain all necessary information, including a copy of the patient or guarantor identification card.May perform patient discharge functions including, but not limited to, review of after visit summary (AVS), future appointment scheduling, and referrals.Explains polices including all regulatory and financial consent forms; secures all required signatures.Collects patient out-of-pocket responsibility per collection guidelines. Provides patient estimates as requested. Prepares and balances a daily deposit of all payment collections.Verifies insurance benefits including obtaining insurance card(s) and confirms coverage is active. Determines correct insurance filing order, if multiple insurance coverages are effective for that service.May schedule walk-in appointments for services offered.Obtains pre-certification and authorization.May confirm physician and prescription orders ensuring accuracy.Provides exceptional customer service and ensures all questions and concerns are addressed in a timely and courteous manner. May guide the patient to appropriate destination for services.

One (1) year of related hospital, medical office, or customer service experience preferred. Other Information: In Memorial Physician Group (specialty practices), additional responsibilities include: (1) obtain specialty authorizations (2) authorization denial and peer to peer process (3) patient care navigation ex: surgical and procedural coordination and scheduling for patient specific populations (4) handle all incoming calls and physician and hospital back line (5) obtain and confirm referralsIn the Hospital, additional responsibilities include: (1) Upon validation of patient identity, place identification band on patient (2) obtain signatures for hospital specific regulatory forms not required in an ambulatory or office setting (3) obtain authorizations for walk-in appointments (4) determine when financial assistance is needed.In Memorial Primary Care, additional responsibilities include: (1) MIH-MPC program patient referral, payment collection and eligibility scheduling (2) process referral work-ques and same day access requests (3) work with Patient Access Center on real time patient requests (4) address prescription refill requests, patient advice requests through MyChart, and provider scheduling template.

Job Description:

EDUCATION/EXPERIENCE: Bachelor's degree in a Health/Fitness field required; prefer Exercise Physiology degree. Master's Degree preferred.
LICENSURE, CERTIFICATION, and/or REGISTRATION: Exercise Specialist certified by the American College of Sports Medicine or eligible to test for the same. Responsible for identifying and coordinating patient/family care to support terminally ill patients and families in all hospice settings. Frequency of patient / family contacts will be at the discretion of the Case Manager and his/her assessment of need, but will be a minimum of every 15 days. The Case Manager utilizes teaching, assessment, and intervention skills to provide comfort care at end of life and maximize the quality of life for the patients and families.
Educates patients and families regarding disease processes and trajectory, disease management, hospice philosophies, signs and symptoms of decline and end of life. Updates the primary physician as indicated and according to agency, accreditation, state and federal requirements.
Responsible for continuous review of all aspects of every patient of her/his caseload to include appropriate utilization of services and the appropriate hospice level of care criteria are met. Makes referrals to other team members as appropriate.
Administers medication and treatments as prescribed by the physician.
Leads the interdisciplinary care group (IDG) specific to each patient and family on their caseload. Working within the IDG to develop, revise, and implement the patient’s Plan of Care. Coordinates all services and collaborates with members of the interdisciplinary team to provide consistent patient care. Arranges for and/or recommends equipment, medical supplies and other necessary items and services as indicated.
Enters, completes and updates documentation in the electronic health record as required according to agency/department standards and policies, accreditation, state and federal guidelines.
Documentation accurately records the patient’s experience with a combination of assessments and narrative .
Provides on-going assessment of patient response to treatments, medications and teaching while making appropriate changes in interventions and follow-up to meet patient needs. Updates and revises POC as indicated with supportive documentation to changes. Communicates problems and changes in condition to physicians, supervisor and other members of the IDG. Enters verbal orders from the physician or APN into the Electronic Health Record.
Supervises and performs Home Health Aide and licensed practical nurse supervisory visits according to agency standards. Instructs paraprofessional staff on a timely basis and in accordance with agency, state and federal regulations.

Clinical responsibilities:

Where You Will Work:
4CD is a 26 bed Medical/Telemetry unit comprised of both private and semi-private patient rooms. Most patients are admitted by the Emergency Department, making it a great learning experience for new graduates. You will care for a very diverse population with multiple acute and chronic illnesses including COPD, CHF, chest pain, diabetes, wounds, cellulitis, stroke, pneumonia, and minor surgical patients (appendectomy, cholecystectomy, etc).
We are a fast-paced medical unit which is the perfect place to learn new skills and have the opportunity to work as a partner alongside our multidisciplinary team. Education
Graduate of a Board of Nursing approved nursing education program.
Certification / Registration / License

7. Turn and position bedfast patient every 2 hours and as needed. Assist patients with bowel and/or bladder functions. Assist patients in Bladder maintenance and bladder or bowel retraining programs.
8. Answer call bells promptly per policy /Procedure.
9. Assist with the care of the dying patient. Provide postmortem care per Policy/Procedure.
10. Provide perineal and indwelling catheter care on your shift and as necessary per Policy/Procedure.
11. Assist patients with (ADL) activities of daily living skills with and without adaptive equipment as per instructions of therapist or nurse.
12. Serve food trays per standard. Assist with feeding as indicated. Record food and fluid intake. Assist patients with identifying food arrangements with sight problem of foods or his/her tray.
13. Serve between meal and bedtime snacks.
14. Assist with lifting, turning, moving, positioning, and transporting patients into and out of bed, chairs, bath, wheelchairs, etc. utilizing proper body mechanics.
15. Report all allegations of patient abuse and/or misappropriation of patient property as per federal requirements and per policy/procedure.
16. Report all accidents and incidents you observe on the shift that they occur per Policy/Procedure.
17. Assists patient with specialized feeding techniques as per recommendations of the speech therapist in accordance with policy and procedure.
18. Assist with ambulation, gait training and CPM administration as instructed by the physical therapist.
19. Apply orthopedic devices such as AF orthosis, abduction braces, hinge joint immobilizer per instruction of the physical therapist.
20. Use safety equipment and supplies (mechanical lifts, steady’s, etc.) when lifting or moving patients who requires more than one person assistance.
