Summary:
Creates a sanitary, safe environment and enhances the appearance of the facility.
Responsibilities:
Performs other environmental services duties as needed.Cleans patient rooms and other assigned areas daily, including collecting and removing trash, changing bed linens and sanitizing all surfaces. Ensures proper sanitation of medical equipment as required.Replenish supplies and/or linen in patient rooms as necessary or designated.Maintains cart and equipment in clean, working condition and stores in appropriate storage area when finished with shift.May be responsible for maintaining a consistent floor care program according to outlined policies and procedures.

Summary:
Responsible for assisting pharmacy leadership in day-to-day supervision and managing the operations of technician personnel and pharmacy systems. Supports the continual site operations for the pharmacy department in a supervisory capacity to achieve departmental goals and objectives and ensures regulatory compliance.

Responsibilities:
Supervises, trains, directs and serves as an internal resource for Pharmacy Technicians to support department operations and goals. Provides feedback to the team regarding daily assignments and responsibilities to ensure work is being followed up accordingly.Responsible for the intra- and interdisciplinary day-to-day management and auditing of operational pharmacy processes. Responsible for providing pharmacy leadership with end-of-month operational metric summary. Contributes to departmental fiscal planning, cost-reduction and containment, and revenue recovery.Monitors quality and assurance processes and ensures compliance and adherence to established programs, regulations, policies and procedures, and applicable institutional licensure.Serves as a role-model and provides mentorship and coaching to technician personnel. Involvement in the recruitment and hiring process. Assists pharmacy leadership with time and attendance, scheduling, performance evaluations, and development of corrective actions for technician personnel. Leads the handling and resolution of complex issues.Supervises the professional growth and development of Pharmacy Technicians by managing the quality of training and services performed by technician personnel. Acts as a department leader to support personnel engagement and subsequent development of department action plan. Acts as a liaison to the Pharmacy Technician Training Program.Performs all Pharmacy Technician roles and functions including coverage including, but not limited to: inventory procurement management, auditing, and pharmacy automation systems.Assists pharmacy leadership in department project management including the identification, implementation, and on-going monitoring and maintenance.

Summary:
The Patient Financial Services Representative (PFSR) serves as the first point of contact in greeting patients and guarantors in the hospital, ambulatory or medical office setting. The PFSR engages with the patient or guarantor to obtain pertinent information and answer any questions in an effort to ensure that all required demographic, financial, and insurance eligibility information is gathered and verified. Ensures all required notices and consent forms are signed accordingly.
Responsibilities:
May perform patient discharge functions including, but not limited to, review of after visit summary (AVS), future appointment scheduling, and referrals.Explains polices including all regulatory and financial consent forms; secures all required signatures.Collects patient out-of-pocket responsibility per collection guidelines. ·        Help Facilitate Internal Aurora Transfers telephonically
·        Help Facilitate Non-Aurora Transfers telephonically, assessing documentation, MD triage, taking and entering orders
·        Will perform Tele Intake (telephonic or Video) assessments for patients in the ER that have a Behavioral Health Consult. Triage with appropriate on Call MD, along with taking and entering orders.

Summary:
The Accounts Payable department is responsible for paying all suppliers for products and services provided to the healthcare system. Job Description:
Assess, plan and direct patient mobilization and exercise programs in an inpatient setting. Works closely with other disciplines and specialties within the department and facility as well as patients and families to assure a cohesive approach to patient care.
EDUCATION/EXPERIENCE: Bachelor's degree in a Health/Fitness field required; prefer Exercise Physiology degree. Master's Degree preferred.
LICENSURE, CERTIFICATION, and/or REGISTRATION: Exercise Specialist certified by the American College of Sports Medicine or eligible to test for the same. ACSM-CEP (Clinical Exercise Physiologist) or ACSM EP-C certification preferred. Basic Life Support for Healthcare Provider (BLS HCP) from AHA required.

- Develops, implements, and performs patient mobilization and exercise programs
- Assesses the patient for contraindications and readiness for mobilization and exercise by reviewing the medical record

The Case Manager utilizes teaching, assessment, and intervention skills to provide comfort care at end of life and maximize the quality of life for the patients and families.
Educates patients and families regarding disease processes and trajectory, disease management, hospice philosophies, signs and symptoms of decline and end of life. Updates the primary physician as indicated and according to agency, accreditation, state and federal requirements.
Responsible for continuous review of all aspects of every patient of her/his caseload to include appropriate utilization of services and the appropriate hospice level of care criteria are met. Makes referrals to other team members as appropriate.
Administers medication and treatments as prescribed by the physician.
 Leads the interdisciplinary care group (IDG) specific to each patient and family on their caseload. Working within the IDG to develop, revise, and implement the patient’s Plan of Care. Coordinates all services and collaborates with members of the interdisciplinary team to provide consistent patient care. Arranges for and/or recommends equipment, medical supplies and other necessary items and services as indicated.
Enters, completes and updates documentation in the electronic health record as required according to agency/department standards and policies, accreditation, state and federal guidelines.
Documentation accurately records the patient’s experience with a combination of assessments and narrative  .
Provides on-going assessment of patient response to treatments, medications and teaching while making appropriate changes in interventions and follow-up to meet patient needs. Updates and revises POC as indicated with supportive documentation to changes. Communicates problems and changes in condition to physicians, supervisor and other members of the IDG. Enters verbal orders from the physician or APN into the Electronic Health Record.
Supervises and performs Home Health Aide and licensed practical nurse supervisory visits according to agency standards. Instructs paraprofessional staff on a timely basis and in accordance with agency, state and federal regulations.
Facilitates the discharge planning process according to agency standards and utilizes interagency/system and community resources to assure continuity of care after discharge.
WI ONLY: May conduct OASIS assessments for Palliative Care patient and establish and update the plan of care. May conduct hospice initial and comprehensive assessments and establish and update the plan of care. Ensures continued skilled nursing/home bound criteria are met. Initiates appropriate preventive and rehabilitative nursing procedures.
Utilizes proper body mechanics, assistive devices and safety techniques when performing various patient positioning/repositioning and transporting duties, which require lifting, and pushing/pulling. Orients and mentors new staff as requested.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration and/or Certification Required:
Registered Nurse license issued by the state in which the team member practices, and
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained upon hire unless department leader has determined it is not required, and
Driver license issued by the state of Wisconsin, or
Driver license issued by the state of Illinois.
Education Required:
