Summary:
Transports patients, and equipment throughout the facility.
Responsibilities:
Assists staff with lift assists and repositioning of patients when needed.Assists patients with entering and exiting vehicles, including transfer from or to wheelchairs or stretchers.Transports patients via wheelchair, stretcher, bed or neuro chair to designated sites as requested, including patients with oxygen tanks, intravenous pumps, drips, monitors, and other equipment.Greets and provides information and direction to patients and visitors.Delivers equipment to requesting staff or department. Assists staff with removal of deceased bodies from units and transport to morgue.

Minimum Qualifications
HS Diploma or equivalency required.
Minimum of three years progressive experience in the field of healthcare foodservice management or an equivalent combination of education and work experience such as Certified Dietary Manager certification (CDM) and at least 5 years progressive supervisory/ management experience.
Additional Qualifications (Nice to have):
BA or BS degree preferred in institutional management, dietetics or equivalent degree

$21.85 - $32.80Major Responsibilities:
- Uses facility/provider information and established policies and procedures to seamlessly link the patient experience between PACT and the practice site.
- Uses resources and critical thinking skills to assist inbound callers. Makes outbound calls when required or when follow up is deemed necessary.
- Has the ability to recognize complex problems and questions and escalates for resolution when needed. Performs basic technical troubleshooting in connection to online applications, systems, or access as requested to resolve issues.
- Responsibilities include scheduling patient appointments and coordinating cancellations, reschedules, and additions to schedules. Obtains demographic and insurance information and verifies insurance coverage. Ensures insurance and patient information obtained is complete and accurate, updating information if necessary, applying acquired knowledge of government and third-party payer requirements.
- Completes all essential forms, obtains necessary information, such as patient demographic and insurance information. Verifies and updates the medical record with patient information.
- Identifies emergent calls based on information provided by caller and department procedures. Follows the process for immediate transfer to Registered Nurse for triaging. Responsible for competency in and adherence to guidelines for emergency situations and critical call handling.
- Determines the needs of patients calling the call center which results in routing patients for triage, scheduling, rescheduling, and canceling appointments, submitting medication refill requests for evaluation, and paging providers and facilities as appropriate.
- Provides customer service per established departmental standards as measured by patients on post call survey. Asks clarifying questions, presents options or solutions, and understands the level of complexity of the call, escalating only those situations necessary for resolution.
- Assists with organizational marketing efforts by providing associated information and referral to customer, while maintaining appropriate records for documentation. Conducts regular reporting and updating of the provider and marketing databases.
- Performs additional duties based on department needs.
- Maintains knowledge and efficient utilization of all information systems utilized by the department.

Summary:
The RN Case Manager coordinates the care and service of patient populations from admission through discharge. $25.30 - $37.95Major Responsibilities:- Interprets physiological data from stress tests for determination of individualized exercise prescription. Plans and implements treatment programs based upon patient evaluation. Modifies exercise prescription based on cardiac symptomatology, signs and symptoms of exercise intolerance, or musculoskeletal, orthopedic, and/or accessory medical problems.
- Works with physicians to determine program eligibility, progress, and timing of progression to unmonitored home program or Phase III participation.
- Establishes and presents a coordinated health education program for patients on exercise and risk factor reduction. Participates in family and patient conferences as necessary and works closely with cardiologists, cardiac surgeons, physician assistants, and medical staff regarding patient physical status and rehabilitation program.
- Promotes referral to rehabilitation program by developing a sound working relationship with referral sources - physicians and nurses.
- Places electrodes on patient for various cardiac monitors and tests.
- Counsels patient in home activity and exercise programs and purchase of exercise equipment.
- Assists in the resolution of cardiac emergencies or symptoms of exercise intolerance. Assists the in-program planning and implementation.
- May serve as a mentor for staff working for certification by American College of Sports Medicine in Exercise Technologist or Specialist category. Serves as a hospital consultant in area of exercise physiology. Participates in appropriate professional organizations for personal development.
- Assists in monitoring outpatient cardiac rehabilitation exercise groups.
- Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.

- Supports imaging technologists with preparation of patient and examination room prior to, during, and following imaging procedures in accordance with established infection control practices. Organizes and prepares exam rooms and work areas, including cleaning, stocking, and maintaining equipment.

The RN is responsible for completing the Workday CBL –“Swing beds”. This is an annual CBL that describes the difference in a swing bed patient versus an in-patient or observation patient. The RN will understand the documentation differences and the specific care required for each individual swing bed patient.
Bronson South Haven Hospital and Bronson Lakeview Hospital employees who are approved to work in a Swing Bed Facility only: Employees may be expected to cover shifts at Bronson Lakeview Hospital and/or Bronson South Haven Hospital depending on patient acuity and staffing needs.
Virtual Registered Nurse (VRN) Role: The virtual registered nurse leverages technology to deliver patient care remotely, collaborating with bedside staff and the broader care team to ensure quality outcomes and timely discharge. Core responsibilities include, but not limited to:
