$38.20 - $57.30Major Responsibilities:
Facilitates communications among patient/family, multidisciplinary team, medical management team, community resources and other disciplines to anticipate, identify, evaluate, and act to resolve any potential barriers and constraints to delivery of care in a timely manner. Understands and interprets multiple contracts and contractual obligations in order to enable the care management team to achieve maximum clinical and financial outcomes.
Collaborates with the patient/family and inter-professional team including the primary care team, hospital care team, post acute care managers, and other care partners to provide a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services. May work embedded within a provider office or telephonically working with a care team.
Uses evidenced-based approaches to increase patient and family activation and engagement in their own care. As appropriate to the population, partners with patient and family to develop SMART (specific, measurable, attainable, relevant, time-bound) goals. Assists in the development, procurement, and adoption of patient self-management educational resources.
Identifies potential barriers to learning and/or to the optimal delivery of care. Reports abnormal findings to the responsible provider/care team, and collaborates to develop a plan.
Independently manages CM caseload according to department expectations. Ensures timely completion of tasks and documentation related to MCO, regulatory and contractual requirements.
Partners with identified at-risk patients throughout the diagnosis, treatment and follow-up in order to deliver continuity of care. Anticipates the needs of the patient, recognizes and responds to changes in a patient’s status and determines priorities of patient care based on essential patient needs.
Coordinates patient information and communication between and among the patient/family, the referring/accepting facilities and physicians, community caregivers (as applicable) and other members of ACM to ensure smooth transitions of care.
Coordinates referrals to other internal AAH departments and/or external community resources as necessary.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
- Registered Nurse license issued by the state in which the team member practices.
Education Required:
- Bachelor's Degree in Nursing or related field.
Experience Required:
- Typically requires 5 years of experience in clinical nursing or 1-2 years of care management experience.
Knowledge, Skills & Abilities Required:
- Applicable certification is encouraged.
- Must be self-directed with the ability to work well independently and within a team environment while recognizing and meeting the individual needs of external and internal partners/customers.
- Ability to demonstrate excellent oral, written and interpersonal skills.
- Ability to demonstrate critical thinking, problem solving and excellent organizational skills.
- Ability to work productively and effectively in a complex environment that includes multiple changing priorities.
- Demonstrated ability to work well with physicians and other healthcare professionals in a direct and positive manner.
- Proficient computer/Microsoft-suite skills and previous Epic EMR experience.
- Ability to handle multiple responsibilities.
Physical Requirements and Working Conditions:
- Position may require travel between clinic sites so there may be exposure to road and weather conditions.
- Manual dexterity required for operation computer and calculator.
- Visual acuity required to facilitate review of written documents/computer screens, medical records, and to record information accurately.
- Clear oral communications and hearing acuity required for receiving instructions and converse on standard telephone.
- Functional speech and hearing to allow for effective communication of instructions and conversation over the telephone.
- Exposed to normal office environment; including usual hazards related to operating electrical equipment.
- Operates all equipment necessary to perform the job.

Complexity of Work: This role requires critical thinking skills, effective communication, decisive judgment, and the ability to build and foster positive relationships. Required to work throughout the South Broward Hospital District in various nursing units treating a diverse range of patients under changing circumstances and challenges in different settings. CVICU 7T is a 30 bed ICU caring for patients post open heart surgery, transcatheter aortic valve replacement, aortic interventions (surgical and endovascular), vascular procedures and end stage heart failure treatment (clinical work-up, left ventricular assist device implantation and heart transplantation).

POSITION PURPOSE
Under general supervision, the electronic technician is responsible for the safe and effective operation of electronic equipment. MINIMUM QUALIFICATIONS

Our patient population commonly includes pneumonia, COPD exacerbations, heart failure, chest pain, stroke, acute kidney injury, diabetes-related complications, electrolyte imbalances, infections, and other complex medical diagnoses. Join a Specialized Neuro-Rehabilitation Team
Vanderbilt Health's Pi Beta Phi Rehabilitation Institute (PBPRI) is seeking an Occupational Therapist to join our nationally recognized outpatient neuro-rehabilitation program.
This position offers the opportunity to work alongside a highly specialized interdisciplinary team dedicated to helping adults maximize independence, function, participation, and quality of life following neurologic injury or disease. More than 80% of the caseload is focused on adult neurorehabilitation, with opportunities to develop expertise in cognitive rehabilitation, vestibular and balance rehabilitation, vision rehabilitation, seating and mobility, and driving rehabilitation.
About the Role
As part of Vanderbilt's collaborative outpatient neuro-rehabilitation team, you will evaluate and treat adults with a variety of neurologic conditions, including:
- Stroke
- Traumatic Brain Injury (TBI)
- Parkinson's Disease
- Multiple Sclerosis (MS)
- Amyotrophic Lateral Sclerosis (ALS)
- Spinal Cord Injury
- Concussion
- Complex neurologic disorders requiring comprehensive rehabilitation
- Driving rehabilitation evaluations and services (based on experience and training)
Working closely with physical therapists, speech-language pathologists, physicians, and caregivers, you will play a vital role in helping patients return to meaningful activities at home, in the community, at work, and in leisure pursuits.
Key Responsibilities
- Evaluate and treat adults with neurologic diagnoses in an outpatient setting.
- Develop individualized treatment plans focused on independence, safety, participation, and quality of life.
- Deliver evidence-based neurorehabilitation interventions addressing upper extremity recovery, motor control, coordination, and functional performance.
- Assess and treat cognitive and executive functioning deficits affecting daily life, work, and community participation.
- Evaluate and train activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
- Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
- Educate patients and caregivers regarding diagnosis, prognosis, adaptive strategies, home programs, and community resources.
- Complete timely clinical documentation, evaluations, progress reports, and payer-required documentation.
- Participate in specialty programs including cognitive rehabilitation, vestibular and balance rehabilitation, vision rehabilitation, seating and mobility services, and driving rehabilitation based on experience and clinical interests.
- Contribute to program development, quality improvement initiatives, mentoring, and ongoing professional development.

POSITION PURPOSE
The Pharmacy Technician II – Advanced (certified and licensed) provides safe, quality technical and non-technical functions which assist in provision of pharmaceutical care under the supervision and direction of a registered pharmacist in accordance with level of experience, education, policies and procedures.
Provides expanded knowledge and experience of the Pharmacy Technician I – General role. Work includes complex sterile and non-sterile compounding, program and quality assurance activities and / or billing. May mentor and/or precept lower-level colleagues.
ESSENTIAL FUNCTIONS
Pharmacy Practice - Acts independently and appropriately within license, scope of knowledge and experience in practice area. Exhibits agility and willingness to take on new and additional responsibilities for role and competence.
Quality and Compliance - Monitors quality assurance activities and issues, and identifies opportunities (e.g., patient care area inspections, sterile and non-sterile compounding, drug diversion) for improvements. Supports auditing for standard work.

MINIMUM QUALIFICATIONS

Licensure: Current pharmacy technician state licensure where employed.

Vanderbilt Clarksville Hospital is seeking an Occupational Therapist to join our collaborative inpatient rehabilitation team. This role is approximately 90% Acute Care and 10% Inpatient Rehabilitation, serving adult patients across a variety of hospital settings.
As an Occupational Therapist, you will help patients regain independence, improve function, and prepare for safe discharge through evidence-based, patient-centered care. You'll work closely with physicians, nurses, physical therapists, speech-language pathologists, and case managers to support optimal outcomes.
Patient Population
Occupational Therapists provide care throughout the hospital, including:

- Rehabilitation Therapist Patient Evaluation (Intermediate): Demonstrates mastery in evaluating patients with physical deficits including impairments, developmental disabilities, functional limitations, disabilities, or changes in physical function by obtaining information pertinent to the assessment and administering and scoring standardized tests and objective measures of deficit areas. Selects appropriate measures and performs a functional assessment to document patient's status requiring skilled therapy. Interprets the results of the assessment and their significance to the patient, family, referring physician and other members of the health care team. Demonstrates experience in clinical evaluation skills and may serve as a mentor for other clinical staff to develop their skills in evaluation techniques. Contributes to the development of best practices for the organization. May consult with team members to implement new procedures.
- Treatment Implementation (Intermediate): Demonstrates mastery in providing treatment consistent with physician prescription and established plan of care. Able to modify treatment plan as needed to demonstrate provision of skilled therapy services including progression of treatment throughout course of care, and revision of treatment interventions of goals not met. Supervises and directs certified athletic trainers (PT only), physical therapist / occupational therapist assistants, technicians and volunteers and students. Selects and follows correct protocol for care, as established, and actively seek out, utilize and mentor/teach others on current evidence based treatment strategies. May consult with team members to offer suggestions for treatment strategies. May serve as a clinical content expert for practice area, initiates consultation with team members for advanced treatment of patients, develop forum for staff members to utilize clinical expertise.
