- Works at Cardinal Bernardin Cancer Center in Maywood, with potential to rotate to other off sites
- Majority of time is spent IV compounding and inventory management

What you’ll do:
- Responsible for the accurate and safe compounding of medication admixtures in an environment set forth by USP 797 chapter and organization policies and procedures
- Handle hazardous drugs according to USP 800 and NIOSH recommendations
- Maintains clean room for sterile compounding, performing routine quality assurance monitoring, complying with regulatory and institutional guidelines for the disposal of hazardous drug waste
- Performs other miscellaneous job related tasks/activities as assigned by the supervising pharmacist

What you’ll need for this job:
- High School Diploma
- Preferred: Completion of a Pharmacy Technician Training Program
- 1-2 years of previous experience as a Pharmacy Technician
- Student enrolled in a college of Pharmacy preferred
- Experience in a customer service pharmacy environment or related field preferred
- Current IL licensure as a Pharmacy Tech
- Certified Pharmacy Technician (CPhT) certification validated by the Pharmacy Technician Certification Board (PTCB) or ExCPT validated by the National Healthcareer Association (NHA) required for all technicians licensed after December 31st, 2007. Newly licensed technicians have 2 years from license effective date to obtain their CPhT and become licensed as a registered certified pharmacy technician.

Pay Range: $20.02 - $31.03 per hour
Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.

Position Summary:
Contributes to the care of the patient by developing pharmacotherapeutic regimens and counseling utilizing pharmacology, pathophysiology, pharmacokinetics, and interpersonal communication skills. Provide clinical pharmacy and drug information services to serve hospital staff and collaborate with physicians. Engages in quality improvement and professional growth to meet hospital, departmental, and personal goals. Compound, package, and dispense medications and IV solutions and preparations as ordered. Assist in the supervision of pharmacy technicians and interns in the operations of the pharmacy. Acts as a resource to students of various health disciplines.
What you will do:
1. Works towards achievement of department-wide pillar goals.

2. Assist in the training of pharmacy residents and new pharmacists.
3. Supervise Pharmacy Interns and Technicians to ensure proper training and job performance according to established policy, procedure, and protocols.
4. Work collaboratively with physicians, nurses, and other staff in both central pharmacy and unit-based practice to ensure effective pharmacotherapy.
Service
5. Ensure patients are knowledgeable about new medications and the side effects of all medications that they are taking.
6. Perform provider and patient education in a manner that is service-oriented and consistent with MGHP values.
7. Participate in community outreach as a member of MGHP and the Department of Pharmacy; Appropriately documents community service
8. Carry out specific clinical protocols, such as IV-to-PO conversions and therapeutic substitutions, as defined by Pharmacy and Therapeutics.
9. Demonstrates through leadership and support a "servant" attitude that exemplifies the philosophy, mission, values and vision of MGHP and Trinity-Health

10. Compound and dispense prescription orders from providers, ensuring patient safety, accuracy, and preventing drug incompatibilities for oral and parental medications.

12. Maintain required records and automated dispensing cabinet inventories for controlled substances. Helps investigate controlled substance discrepancies.
What you will need:
1. BS degree in Pharmacy. Licensed to practice in the State of Michigan. Advanced degrees (i.e., Doctor of Pharmacy) and residency training preferred.
2. Demonstrated ability to interact appropriately and effectively with patients, family members, healthcare team members, ancillary departmental staff, medical staff, visitors and the public at large.

Manage patients on anticoagulants by interpreting treatment goal and adjusting anticoagulants as necessary in Mercy Health's anticoagulation clinic.
Collaborate with other healthcare providers as well as providing direct patient care.
Meet with patients individually to provide extensive drug and disease state education targeting complex disease states such as diabetes, hypertension, dyslipidemia, asthma, COPD, tobacco dependence, and osteoporosis.
Work with patients to improve adherence and evaluate medication regimens.

Position Purpose
A Clinical Pharmacist provides medication preparation, distribution services and is prospectively involved with provision of pharmaceutical care and the promotion of high-quality, cost-effective, patient-focused care for populations across the age continuum (from neonatal to geriatric patients depending on hospital populations served) in conjunction with other members of the healthcare team according to professional standards and statutory requirements. The clinical pharmacist demonstrates pharmacotherapy expertise, participates in interdisciplinary education programs, and participates in problem recognition and resolution related to medication usage, actively participates in CQI initiative development and completion and provides clinical education to pharmacy department staff, students and pharmacy residents if part of the department.

Education:
- Required Bachelor's degree in Pharmacy. Doctor of Pharmacy degree or equivalent clinical degree in pharmacy practice is preferred.
Experience:
- Previous related hospital experience or hospital residency strongly preferred.
Licensure / Certification:
- Registered pharmacist possessing or eligible for a Michigan Pharmacist and Controlled Substance license is required. Maintenance of minimum hours of State Board of Pharmacy continuing education credits every two years as required by the State of Michigan.
- Per specific hospital policy, Current Basic Life Support (BLS) and / or Advanced Life Support (ACLS) certification as required.
REQUIRED SKILLS AND ABILITIES:
- Interpersonal skills necessary to effectively communicate drug therapy information to all health team members, patients, and administrators / managers. Interpersonal skills necessary to interact with patients, physicians, and other healthcare providers.
- Broad knowledge of medications compounding and dispensing. Thorough knowledge of pathophysiology, pharmacokinetics and therapeutics.
- Analytic skills necessary to organize and prioritize multiple job duties.
- General understanding of computers, including general operations and data retrieval.

What you will do:
Interprets and evaluates medication orders to ensure appropriateness of therapy; including, but not limited to dosage form, dose, and route of administration, schedule, starting time and duration.
Monitors patient medication regimens for possible allergies, drug-drug interactions, nutrient-drug interactions, contraindications, therapeutic duplication and potential adverse drug reactions.
Individualizes medication therapy based on patient variables including, but not limited to, renal function, age, weight, laboratory values and pharmacokinetic parameters.
Manages drug therapy and utilization across the continuum of care through communication with physicians, nurses, other healthcare providers and patients to intervene on orders that are unclear, incomplete, inappropriate, or could cause poor drug outcomes and recommends alternative therapy.

Compounds and dispenses medications and injectable medications recognizing that compounding and/or dispensing errors may result in significant patient morbidity and mortality. Competency must be demonstrated before compounding or checking compounded products. Adheres to USP 797 / USP 800 Guidelines.
Participates in hospital emergency codes responses. Provides and prepares emergency medications via emergency carts, cardiac kits and EMS/Ambulance kits as necessary based on hospital practices.
Coordinates pharmaceutical services for specified hospital units. Problem-solves, communicates with, and serves as a liaison between these units and the pharmacy department.
Actively participates in staff development programs, continuing education, lectures, conferences, and preparation of newsletters, drug class reviews, and new policies and procedures as hospital practice allows.
As hospital pharmacy department practice allows, participates in teaching programs affiliated with the Department of Pharmacy and training programs for nursing, physicians, pharmacists and other allied health professionals. Precepts Doctor of Pharmacy students on patient care activities. Meets with students regularly to review drug therapy and clinical skills. Serves as a preceptor to the pharmacy residents if trained at hospital site.
Performs duties in compliance with all federal, state and local regulatory requirements.
Maintains control and accountability for disposition of controlled substances, hazardous medications and investigational drug regulations. Properly maintains all legally required records. Maintains a working knowledge of applicable Federal, State, and Local laws and regulations, the Trinity Health Organizational Integrity Program, including the Standards of Conduct, Code of Ethics, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professionally behaviors.

Position Summary
The Radiologic Technologist performs a full range of diagnostic imaging examinations in accordance with established departmental guidelines. Vanderbilt Clarksville Intensive Care Unit (ICU), is a 21 bed mixed medical/surgical adult ICU specialized in providing close monitoring and treatment for patients with a wide-range of diagnosis. We care for patients being treated for Cardiovascular conditions, Acute Respiratory Failure, Acute Respiratory Distress Syndrome (ARDS), Multisystem Organ System Failure, Sepsis, Drug Overdoses, Gastrointestinal Bleeds, Diabetic Ketoacidosis, Hypertensive Crisis, Acute/Chronic Renal Failure, STEMI, cardiogenic shock and post heart catheterization.
Our staff is committed to provide exceptional care to our patients. Our environment is challenging, team oriented, patient and family centered. We work collaboratively to provide opportunities for nurses to grow in critical thinking, professionalism, and participate in ongoing education.
Our team consists of Intensivists, advanced practice nurses, critical care nurses, and respiratory therapists. Daily multidisciplinary rounds include the intensivist, advanced practice nurse, ICU nurse, case manager, respiratory therapy, pharmacy, infectious disease, and a nurse leader. SWAT team consists of intensivist, advance practice nurse and charge nurse available as bedside consult. ICU charge nurse responds to codes, and RRT’s. This position provides pharmaceutical care to patients, serves as a resource and drug information specialist to medical, pharmacy, nursing staff and allied healthcare providers. Performs comprehensive clinical assessments and collaborates with the integrated healthcare team to provide quality patient care. Must demonstrate the knowledge and skills necessary to provide care appropriate to the age and disease state of the patients being served. Must possess the ability to assess data reflective of the patient’s status, interpret the appropriate information needed to identify each patient’s requirements and provide pharmaceutical care based on those needs. Accountable for on-going evaluation and documentation of all components of pharmaceutical care delivered to patients. Oversees the activities of the pharmacy technicians, coordinating workflow and may, when appropriate perform the final check of medications prior to being dispensed. Conducts educational activities, applies principles of performance improvement, and may participate in Pharmacy and Therapeutics Committee activities or Collaborative Drug Therapy Agreements as appropriate. Adheres to local, state, and federal laws regarding the profession of pharmacy practice.
CORE FUNCTIONS
1. Critically evaluates medication therapy and makes recommendations to improve patient medication regimens. Ensures appropriate and cost effective drug therapy. Areas of focus may include but are not limited to evaluating adherence, evaluating and addressing drug-drug interactions, drug-food interactions, contraindicated medications, inappropriate therapeutic regimens, adverse drug reactions, pharmacokinetics, disease state management and rational effective medication regimens. Accurately and completely documents patient assessment data and clinical interventions.
2. Functions as a therapeutic consultant in areas that may include rational and appropriate use of medications, medication adherence, clinical pharmacology, pharmacokinetics, disease state management and nutrition.
3. May participate in Pharmacy and Therapeutic Committee (P&T) activities, evaluate and recommend medications for formulary inclusion, and prepares Formulary evaluations for P&T consideration, as appropriate. May participate in the Quality Council activities including quarterly meetings to discuss quality concerns to improve patient safety and workflow. Quality Council members work with the Clinical Oversight body for approval on quality related process improvement projects.
4. Applies the principles of continuous quality improvement consistent with job expectations. Incorporates quality improvement principles into other activities and projects (i.e. data collection, documentation). Develops criteria for drug use evaluations, medication use indicators and disease-based evaluations. Provides oversight and assists with the development and implementation of evidence-based medication guidelines/protocols. Designs and implements medication monitoring guidelines and programs for both staff and clinical pharmacists.
5. The Clinical Pharmacist assesses the patient at the initiation of drug therapy, develops a patient care plan and drug monitoring regimen and monitors the patient according to the care plan throughout the course of therapy. Verifies medication orders in the computer system for appropriateness and accuracy and dispenses medications in an accurate and timely manner in appropriate quantities based on stability, compatibility of the drug and specific needs of the patient. Documents in the patient clinical record all recommendations and interventions communicated to the physician.
6. Coordinates the drug preparation work of pharmacy technician and checks the accuracy of that work according to literature guidelines.
7. Communicates with the patient, physician, nursing staff, pharmacists and other members of the health care team regarding medication treatment, patient status, response to therapy and clinical objectives to ensure clinical outcome is met. Monitors patient response and documents interventions as appropriate.
8. The Clinical Pharmacist will also be required to take after hours calls as indicated.
9. Participates in all aspects of medication use, including (1) selection, procurement and storage, (2) ordering or prescribing, (3) dispensing, (4) administering and (5) monitoring of all medications utilized. Internal customers may include Patients, Nurses, Licensed Independent Practitioners, Nursing Directors, Department Directors, Administrators and other health care team members for the purpose of integrating services, improving patient care and ensuring effective communication systems. External customers may include local, state, and national pharmacy organizations, Joint Commission, State Board of Pharmacy, colleagues and peers, educational institutions and pharmaceutical representatives.
MINIMUM QUALIFICATIONS
A degree in Pharmacy from an accredited College of Pharmacy.
Requires current and unrestricted pharmacy licensure in the applicable state of practice within 90 days of hire date.
Incumbents in Arizona are also required to provide the AZ Board of Pharmacy Wallet Card at time of hire.
May be required to possess a valid driver’s license and be eligible for coverage under the company’s auto insurance policy.
Must possess good organizational, problem solving and computer skills. Must possess good written and verbal communication skills for interfacing with all levels of staff, Physicians and other contacts. Must have knowledge of medication management software, particularly pharmacy system software, Microsoft applications and automated dispensing systems.
PREFERRED QUALIFICATIONS
Completion of an accredited pharmacy residency program or previous clinical pharmacy experience is preferred.

This position provides staffing support across a variety of 10-hour shifts to meet operational needs. The schedule includes a mix of daytime, evening, weekend, and holiday coverage on a rotating basis. Prepares and dispenses medications, including sterile or non-sterile compounded medications, in an accurate and timely manner in compliance with company policies and procedures and established regulatory requirements. Coordinates medication preparation and distribution and ensures accuracy prior to dispensing medications.
6. Oversees the day-to-day operations of the pharmacy, which may include management of technology, inventory management, and accurate record keeping. Supervises and directs the work of pharmacy technicians, interns, and students.

Job Description:

- Reviews patients' medication order for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug- drug interaction. Makes appropriate evidence-based interventions to optimize medication use. Ensures safe, appropriate, cost-effective therapies and accurately dispenses medications.
- Oversees and/or participates in assuring appropriate medications are prepared and delivered timely to care areas and are stored both securely and appropriate for both stability and effective workflow.
- Provides counseling to patients and promotes quality medication education and adherence. Ensures patient is well informed, treated with respect, and receives the highest level of professional service.
- Completes all regulatory and compliance training and ensures documentation of regulatory requirements is clear and complete. Is knowledgeable and compliant with state and federal laws, regulations, pharmacy policies, procedures, and standard operating procedures.
- Supervises and directs activities of interns, technicians, and support personnel as required. Provides medication related clinical consultation and clarification to other health professionals.

Position Purpose
We are hiring Pharmacists to support operations and our expanded emergency pharmacy program! We are expanding our shift coverage to include a decentralized pharmacist. We are searching for per diem help to cover this role as well as centralized distribution. This position is for pharmacists with experience in hospital pharmacy who are able to work directly with patients for medication histories, counseling, and responding to emergencies within the hospital.
As a Pharmacist, you will:
Appropriately assesses patient data to identify medication-related problems through profile reviews, pharmacy consults and interdisciplinary rounding.
Participates in medical emergencies and other formal alerts such as trauma, stroke, sepsis and STEMI as required.
Reviews and interprets providers orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, and comorbidities. Checks for drug incompatibilities and allergies, and takes appropriate action when necessary. Updates medication history for all admitted patients.
As necessary, consult with providers, nurses and other healthcare professionals involved in patient care regarding pharmacotherapy optimization with intent to achieve optimal patient outcomes across the continuum of care.
Provides accurate and timely responses to other healthcare professionals and patients regarding drug information questions and facility medication use policies to assure safe and appropriate use of medications.
Documents patient care activities including therapy plan, monitoring, and follow-up in electronic medical record as per departmental guidelines.
Minimum Qualifications:
Pharmacy degree (Pharm.D.) from an ACPE approved college of pharmacy required.
Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years of hospital experience is required.
Registered pharmacist in Pennsylvania required.
Preferred: Experience with Pyxis MedStation, EPIC
BLS and ACLS certification before completion of orientation (will be provided if cannot provide upon hire)
