ACE Nursing Model of Care: Achieving Collaborative Excellence
Geisinger is excited to be piloting a new model of care on our Advanced Acute Care Units at our Danville, Bloomsburg, and Shamokin campuses. The ACE Nursing Model of Care utilizes more Patient Care Technician staff to take responsibility for and perform the duties that do not require a nursing license.
This new model of care will allow Patient Care Technicians more opportunity for career advancement, autonomy in their role, and greater involvement in the delivery of patient care.
Primary duties may include:
- Being the primary facilitator of ADLs: hygiene, toileting, mobility, meals
- Collecting vital signs & daily weights
- Conducting Point of Care testing
- Turning & repositioning patients
- Performing equipment checks & exchanges
- Dressing care
- Removal of Foley Catheter and Peripheral IV
- Nasogastric tube removal and performing nasal swabs

May perform a wide variety of advanced patient care duties under the supervision of a Registered Nurse and maintains the patient care and unit environment consistent with the policies, procedures, and practices. Meets the qualifications to provide care for patients in the specific age range on their assigned clinical area.
- Provides enhanced patient care under the direction of the licensed nurse, which may include but is not limited to advanced dressing care, suctioning, removal of Foley Catheter and peripheral IV.
- May perform venipunctures and skin punctures to obtain blood specimens according to Clinical Laboratory Standards Institute guidelines.
- May perform daily, hands-on aspects of the storage, preparation and delivery of breast milk, infant and pediatric formulas.
- Documents in the appropriate sections to create a complete and accurate medical record.
- Identifies, observes, and documents the physical needs of the patient according to established standards. Reports any changes in patient condition to licensed personnel in a timely manner.
- Obtains and maintains the integrity of specimens in accordance with policy and procedure, according to physician order.
- Provides for or assists patients with activities of daily living and fulfills designated and delegated responsibilities for ongoing continuum of care. Includes patient hygiene and changing linens.
- Provides one to one patient safety watch as assigned following established policies and procedures.
- Accurately processes specimens and provides timely transport of the specimen to the laboratory.
- Performs point of care patient testing and point of care instrument maintenance and quality control.
- Assists in problem solving, trouble shooting and taking corrective measures in care and maintenance of equipment and supplies.
- Serves as a preceptor and mentor for new staff and students in a positive manner.
- Performs clerical duties for admissions, transfers, discharges, and deaths according to standards and facilitates patient throughput activities. Transcribes and processes orders according to unit standard.
- Releases and confirms ordered tests to appropriate departments.
- Coordinate's transportation of patients to multiple tests.
- Participates in obtaining and communicating electronic information as pertinent to the nursing unit. Schedules tests, faxes consult, etc.
- Assists in ordering supplies. Stationery items as well as floor stock, patient needs, individual nursing supplies.
- Communicates with food services to ensure appropriate patient diets.
- Operates, problem solves, and takes corrective measures for clerical equipment. To include scheduling maintenance of computers, fax machines, phones, etc.
- Staff have access to non-controlled medications storage and supply areas and can transport non-controlled medications as necessary to meet the needs of the clinic or unit.
- Assists with transporting non-controlled medications to and from Pharmacy and within the unit as needed.
- Assists with transporting blood products to and from Blood Bank as needed.
- Discharge follow up phone calls / coordinating virtual discharge appointment team.
- Support / Assist staff with 5 min Drills and/or Quality Rounds - participate in regulatory readiness as needed.

Position Details:
BP3 Advanced Acute Care Medical Transplant Unit
Schedule: 4, 8, and 12 hour shifts on days, evenings, or nights. A minimum of 24 hours of availability in a 4-week schedule are required to maintain per diem status.
Required Orientation: At the start of employment, a consecutive two-week classroom orientation is required with a schedule of Monday - Friday, 7:00am - 3:30pm.
No experience needed. Comprehensive on the job training provided.

Job Description:
The Clinical Pharmacist provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes for patients. The Clinical Pharmacist actively participates in multidisciplinary patient-care rounds, medication order evaluation and verification, drug distribution, technician supervision, and clinical services including medication histories, pharmacokinetic dosing, drug information requests, and patient counseling to ensure the best medication therapy outcomes. When applicable, the Clinical Pharmacist will participate in development of new programs, services, practices, and facilitate scholarly and professional development activities.
Major job responsibilities include: medication order evaluation (to include but not limited to appropriateness and effectiveness) and verification, drug distribution support, technician supervision, clinical intervention, formulary management, and resource for nursing and physician medication related inquiries. The Clinical Pharmacist will participate in the design, implementation, and monitoring associated with the provision of safe, effective and economical pharmacotherapy for all patients. They will collaborate with pharmacy leadership in the development and implementation of clinical pharmacy services and ensure compliance with policies and procedures at UofL Health.
The position requires the ability to communicate and collaborate effectively with patients, physicians, nurses, pharmacists, and allied health professionals from all clinical disciplines regarding activities related to the improvement of medication use. Ensures that all activities are consistent with overall hospital objectives. The position demonstrates competence to provide pharmaceutical care for neonatal (birth-discharge), pediatric (birth to 11 years), adolescent (12-18), adult (19-65), and geriatric (66-over) patients.
Essential Functions:
Delivers comprehensive pharmaceutical care to patients
- Works collaboratively with other clinicians and health-care providers through multidisciplinary patient care rounds
- Reviews medication orders for accuracy, appropriateness and efficacy
- Documents all pharmacy interventions in patient electronic medical record and integrated clinical decision support system
- Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions
- Provides pharmacokinetic consultations and other dosing recommendations as required
- Prevents, detects, monitors, documents, and reports adverse drug reactions and medications errors
- Participates in the drug therapy management of medical emergencies (code blue, code stroke, etc.)
- Performs both admission history and discharge medication review and/or counseling in order to facilitate the admission/discharge/transfer of patients
- Provides operational support to Clinical Staff Pharmacists as determined by departmental need
- Responds to drug information requests and optimize pharmacotherapy regimens utilizing current evidence-based literature
Promotes and demonstrates rational, cost-effective drug therapy
- Participates in formulary management by implementing therapeutic interchanges and enforcing the non-formulary ordering process
- Assists in the procurement of medication when necessary, including verification of home medication supply, when applicable
- Facilitates the implementation of clinical and operational initiatives
- Documents clinical consultations and cost avoidance as part of the departmental clinical documentation program
- Prepares formulary review and drug use evaluations of medications used in the area of clinical expertise for presentation to the Pharmacy and Therapeutics Committee
- Assists pharmacy leadership in the determination, communication, and education of alternative therapies during medication shortages
Other Functions:

Primary Duties and Responsibilities:
1) Clinical integration and pharmacist-to-pharmacist support
• Serves as a consistent “bridge” between BIDMC/MAH-inpatient pharmacists and DFCI inpatient-oncology providers, strengthening working relationships and streamlining communication
• Serves as an on-site oncology-pharmacy clinical resource for BIDMC/MAH teams for complex oncology-medication questions related to shared DFCI patients, providing real-time consultation support to reinforce safe oncology-medication practices
• Supports orientation, onboarding, and ongoing training of newly hired inpatient-oncology pharmacists at BIDMC or MAH, in collaboration with local pharmacy leadership, by providing DFCI-specific oncology-practice context, shared patient workflows, and key contacts/escalation pathways for DFCI-oncology patients
• Identifies recurring knowledge gaps or high-risk scenarios and coordinates targeted education with BIDMC/MAH-pharmacy leadership
2) Transitions-of-care communication for shared oncology patients from inpatient to DFCI ambulatory
• Establishes and maintains a reliable medication- and treatment-plan-handoff workflow for select high-risk DFCI patients discharged from BIDMC/MAH back to DFCI ambulatory care
• Communicates time-sensitive changes to DFCI ambulatory pharmacy teams, such as:
i. chemotherapy-plan changes, dose modifications, delays/holds, toxicity events impacting future therapy
ii. new or discontinued supportive care meds that affect ongoing outpatient management
iii. monitoring needs and follow-up medication plans tied to oncology therapy
• Anticipates downstream outpatient-access issues created by inpatient changes and alerts DFCI ambulatory pharmacy/clinic teams when action is needed (e.g., continuity of a specialty medication)
3) Inpatient chemotherapy-process support
• Serves as the pharmacy liaison for the inpatient chemotherapy-approval/authorization process for shared DFCI-oncology patients
• Provides regimen-specific clarification support for shared DFCI patients
• Escalates patient-specific concerns to the appropriate DFCI-disease group/ambulatory team to align inpatient actions with the outpatient-treatment plan
4) Liaison for inpatient-medication policies
• Serves as the DFCI-pharmacy liaison to BIDMC/MAH for inpatient medication-use policies with which DFCI intends to align
• Assists with policy-review cycles, and routes questions/updates through the appropriate governance channels at each institution (P&T, medication safety, etc.)
5) Quality/safety surveillance for cross-site issues

Job Duties

The Inpatient BMT/Cellular Therapies Clinical Team Lead Pharmacist at Levine Children’s Hospital will cover the inpatient bone marrow transplant team and provide medication expertise. Job Description:
May perform a wide variety of advanced patient care duties under the supervision of a Registered Nurse and maintains the patient care and unit environment consistent with the policies, procedures, and practices. - Provides enhanced patient care under the direction of the licensed nurse, which may include but is not limited to advanced dressing care, suctioning, removal of Foley Catheter and peripheral IV.

- May perform daily, hands-on aspects of the storage, preparation and delivery of breast milk, infant and pediatric formulas.

May perform a wide variety of advanced patient care duties under the supervision of a Registered Nurse and maintains the patient care and unit environment consistent with the policies, procedures, and practices. CORE FUNCTIONS
1. Fills prescriptions in an accurate and timely manner. Reviews and verifies or enters orders into the medication management information system. Assures that orders present no significant problems in regard to drug interactions, incompatibilities, therapeutic duplications or patient safety. Performs immunization administration upon patient request.
2. Supervises and directs work of pharmacy technicians. Coordinates medication preparation and distribution work of the pharmacy technicians and ensures accuracy prior to dispensing medications for distribution.
3. Provides counseling to patients, families, or caregivers regarding their medications, to support appropriate and effective drug use. Provides accurate, appropriate and timely information to physicians, nurses, patients and others regarding medication therapy, drug product guidelines, law and regulations, medication formulary and other topics related to medication use and control.
4. Supervises the day-to-day operations of the pharmacy that may include inventory maintenance, interactions with insurance companies, and accurate and appropriate record keeping.
5. Consults with prescribing practitioners to help ensure accurate and appropriate medication use. Reviews medication orders and evaluates their appropriateness and completeness. Contacts the prescriber with suggestions for improvement and resolves problems with the medication orders as needed.
6. Oversees all aspects of medication use, including (1) selection, procurement and storage, (2) dispensing, (3) monitoring of all medications utilized. External customers may include Patients, Nurses, Licensed Independent Practitioners, Physicians, other health care staff, local, state, and national pharmacy organizations, Joint commission, state Board of Pharmacy, colleagues and peers, educational institutions and pharmaceutical representatives.
MINIMUM QUALIFICATIONS
Requires pharmacy licensure in the state of practice and a degree in Pharmacy from an accredited College of Pharmacy. Incumbents in Arizona are also required to provide the AZ Board of Pharmacy Wallet Card at time of hire.
Pharmacy licensure. Immunization certification and/or licensure (if state requires) in state of practice required within 6 months of hire.
BSS Registry Team members are required to have a Pharmacy license and Immunization Certification at time of hire/transfer.
Must possess current BLS card.

The inpatient pharmacy at Trinity Health Ann Arbor is open 24 hours a day, 7 days a week, servicing inpatients, the emergency department, operating rooms and multiple procedural areas.
Clean room operations are accredited by Joint Commission for adherence to the highest standards of USP 797 and 800. Trinity Health Ann Arbor is also the lead location for the national Trinity Health Pharmacy Education and Advancement of Technicians (THE-PEAT) training program to educate pharmacy students to become PTCB, licensed Pharmacy Technicians.
Position Purpose
A Clinical Pharmacist provides medication preparation, distribution services and is prospectively involved with provision of pharmaceutical care and the promotion of high-quality, cost-effective, patient-focused care for populations across the age continuum (from neonatal to geriatric patients depending on hospital populations served) in conjunction with other members of the healthcare team according to professional standards and statutory requirements. The clinical pharmacist demonstrates pharmacotherapy expertise, participates in interdisciplinary education programs, and participates in problem recognition and resolution related to medication usage, actively participates in CQI initiative development and completion and provides clinical education to pharmacy department staff, students and pharmacy residents if part of the department.

Participates in hospital emergency codes responses. Provides and prepares emergency medications via emergency carts, cardiac kits and EMS/Ambulance kits as necessary based on hospital practices.
Coordinates pharmaceutical services for specified hospital units. Problem-solves, communicates with, and serves as a liaison between these units and the pharmacy department.
Actively participates in staff development programs, continuing education, lectures, conferences, and preparation of newsletters, drug class reviews, and new policies and procedures as hospital practice allows.
As hospital pharmacy department practice allows, participates in teaching programs affiliated with the Department of Pharmacy and training programs for nursing, physicians, pharmacists and other allied health professionals. Precepts Doctor of Pharmacy students on patient care activities. Meets with students regularly to review drug therapy and clinical skills. Serves as a preceptor to the pharmacy residents if trained at hospital site.
Performs duties in compliance with all federal, state and local regulatory requirements.
Maintains control and accountability for disposition of controlled substances, hazardous medications and investigational drug regulations. Properly maintains all legally required records. Job Description:
The Specialty Pharmacist provides high-touch pharmaceutical care to patients with chronic disease states and deals exclusively with medications and pharmaceuticals that are high in costs, require special handling, are subject to limited or restricted distribution, require ongoing assessment, treat rare disease, or require active monitoring of side effects. The Specialty Pharmacist has an increased focus on patient management, medication adherence, and actively collaborates with other members of the health care team. The Specialty Pharmacist supports URAC/ACHC Accreditation Standards and has the ability to utilize metrics in analyzing optimization of patient care and adherence rates. When applicable, the Specialty Pharmacist will participate in development of new programs, services, practices, and facilitate scholarly and professional development activities.
Responsibilities include
- Intake—Oversees the patient intake process by receiving referrals, screening patient demographic information, ensuring accurate benefits investigation and determination of payor coverage for specialty drugs, and exploring opportunities for patient financial assistance for qualified candidates.
- Clinical Management—Manages patient care by coordinating with an integrated team of health care providers, assessing clinical data, and optimizing therapy to ensure appropriateness and safety of medication use through evidence-based medicine principles. Promotes and monitors adherence and persistency.
Fulfillment—Coordinates the processing, distribution, and delivery of referrals while maintaining product integrity, managing inventory of specialty medications, following up with patients to ensure that they received medications, and billing for services rendered
Supports specialty pharmacy services and specialty clinic processes
- Oversees day-to day operations in the pharmacy as they pertain to workflow, production quotas, deadlines and accuracy of prescription processing
- Ensures the pharmacy is compliant with all accreditation standards and local, state and federal rules and regulations regarding the practice of pharmacy
- Supports the Medication Access Coordinators to ensure accuracy of dispensing-related functions, adherence to delivery schedules, and utilization of safety programs
