Position Title
Position Summary / Career Interest:
The Clinical Pharmacist provides comprehensive pharmaceutical care for patients. This consists of ensuring drug therapy that results in safe, effective, and cost conscious use of pharmaceuticals. Must demonstrate excellent interdisciplinary communication and exhibit strong educational and precepting abilities.Rotation of shift times and locations between our Main campus in Kansas City, KS, and our Southlake campus in Lenexa, KS, and every 6th weekend.Responsibilities and Essential Job Functions
- Actively participates in organizational and departmental performance improvement efforts to strengthen existing systems, processes, policies and procedures by introducing new ideas that lead to improved pharmaceutical care in clinical areas and in the pharmacy department as a whole.
- Actively participates on the multidisciplinary team by consistently attending patient care rounds and providing patient – focused pharmaceutical care by assessing and monitoring drug therapy to ensure safe and cost – conscious treatment regimens.
- Actively supports department staffing needs by flexing to other areas or alternative scheduling in order to assist in coverage of supplemental shifts when primary team is in need.
- Assures timely medication order processing by participating in order entry, order verification and reviewing medication orders for timely resolution of problem orders.
- Completes patient-specific medication profile reviews resulting in execution of protocol based drug therapy including, but not limited to pharmacokinetic dosing and IV to PO conversions with appropriate documentation. Adjusts doses as appropriate for patient specific parameters such as renal or hepatic dysfunction.
- Continually reports medication incidents and adverse drug reactions in Patient Safety Net (PSN) and participates in medication incident review, peer review, or other activities to establish safe medication systems.
- Participates in medication reconciliation at the time of admission, transfer, and/or discharge and provides patient education when appropriate.
- Shares pharmaceutical expertise by coordinating and providing educational presentations or in-services for pharmacy and non-pharmacy staff.
- Supports pharmacy residency programs, clerkship students and interns by precepting organized rotations, providing timely and constructive feedback, participating in training and orientation, providing mentorship, and collaborating on presentations and projects.
- Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience
- Bachelors Degree in Pharmacy from an accredited college or university.
Preferred Education and Experience
- Completion of an accredited pharmacy post graduate training program (PGY-1 and/or PGY-2 residency program).
Required Licensure and Certification
- Licensed Pharmacist - State Board of Pharmacy in state of primary practice site.
- Advanced Cardiac Life Support (ACLS) - American Heart Association (AHA) certification as necessary.
- Pediatric Advanced Life Support (PALS) and/or Neonatal Resuscitation Program (NRP) certification as necessary.
- Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
Preferred Licensure and Certification
- Board of Pharmaceutical Specialties certification in chosen area of specialty.

Description:
- Interprets and reviews medication orders for correct drug, dosage, route, frequency, allergies, therapeutic duplication, interactions, contraindications, and potential adverse events for all ages served.
- Ensure drugs are available to patients at the time they are needed to avoid delays in therapy and promote positive patient outcome.
- Provides comprehensive decentralized or pharmacy clinical services including, but not limited to, anticoagulation management/monitoring, pharmacokinetic dosing, renal dosing, and nutritional support services.
- Works with inter-professional patient care teams to help patients with transitions to care, including but not limited to helping prepare patients for discharge.
Requirements:
- Graduate of accredited College of Pharmacy.
- Kentucky Board of Pharmacy license in good standing as applicable to practice site.
- 1-3 years related work experience or completion of ASHP accredited pharmacy residency program preferred.
- Basic Life Support (BLS) certification is required. If not already certified, must obtain BLS within 30 days of their hire date.

This is for a NICU Pharmacist at Primary Children's SL campus. This is a Full-Time 40 hour a week positions. The shifts will include day, evening, weekend, and holiday shifts.
Essential Functions
- Reviews patients' medication order for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug- drug interaction. Makes appropriate evidence-based interventions to optimize medication use. Ensures safe, appropriate, cost-effective therapies and accurately dispenses medications.
- Oversees and/or participates in assuring appropriate medications are prepared and delivered timely to care areas and are stored both securely and appropriate for both stability and effective workflow.
- Provides counseling to patients and promotes quality medication education and adherence. Ensures patient is well informed, treated with respect, and receives the highest level of professional service.
- Completes all regulatory and compliance training and ensures documentation of regulatory requirements is clear and complete. Is knowledgeable and compliant with state and federal laws, regulations, pharmacy policies, procedures, and standard operating procedures.
- Supervises and directs activities of interns, technicians, and support personnel as required. Provides medication related clinical consultation and clarification to other health professionals.
- Takes the initiative to solve problems including, but not limited to, equipment, maintenance, drug availability, staffing issues, and extraordinary patient care concerns.
- Identifies potential areas for improvement including processes that could be streamlined or revised to improve patient satisfaction. Identifies potential safety hazards that may create problems for patients and/or staff and takes appropriate actions. Participates in departmental meetings, contributing ideas or feedback
- Supports clinic or hospital and system directives, including clinical, compliance, business operations, standardization, and customer service.

All pharmacists are responsible for drug preparation and distribution for medications; evaluating, inputting and verification of medication orders; assisting providers and other health care professionals by serving as a drug information resource; monitoring and managing medication regimens per protocol or per consult; and managing pharmacy inventory.
Essential Duties:
- Provides verbal and written consultations concerning medication therapy on a daily basis.
- Provides education to patients, physicians, nurses, and other healthcare team members as requested.
- Initiates, documents, and participates in department activities that promote positive patient outcomes. These activities may include, but are not limited to: MUE, ADR reporting, resolving drug therapy questions, therapeutic drug monitoring, identifying drug interactions and allergies, direct contact with prescribers to clarify major problem orders, and non-formulary drug requests.
- Responds to medical emergencies in department and throughout hospital as needed including Stroke Alerts and Code Blues.
- Assist in procedural sedations as well as rapid sequence intubations to ensure safe and accurate delivery of high risk medications.
- Ensures antibiotic stewardship activities and issues are communicated with the ED practitioners, regarding opportunities to optimize antimicrobial utilization and help reduce inappropriate antibiotic use and antimicrobial resistance.
- Assist nursing in the utilization of programmable pumps to promote safe drug administration in the ED. • ACLS required.
Requirements:
- Bachelor's degree

- Kentucky a Pharmacist License
- Must obtain ACLS within 6 months
- Must obtain Board Certification within 1 year
- Preferred Doctor of Pharmacy (D.P.)
Graduated of accredited College of Pharmacy, one of the following is required:
- Board Certification in Infectious Diseases within 3 years of hire

POSITION SUMMARY

POSITION SUMMARY
This position is responsible for assisting clinicians and nursing staff in providing medical care, as well as, implementing and evaluating direct patient care. This position utilizes specialized knowledge, judgment and skills to provide an excellent patient experience.

POSITION SUMMARY
This position is responsible for providing personalized coordination, clarification and communication of all financial aspects of care continuum, including insurance and authorization verification, registration, financial counseling and claims research for Oncology. This position partners with the clinical care team to determine financial impact for the patient and serves as the primary contact for any financial questions related to a patient’s care across the entire continuum of their treatment, ensuring a seamless experience for the patient and their family.
CORE FUNCTIONS
1. Performs pre-registration/registration processes. Partners with the clinical care team to determine initial authorizations needed based on the predicted care treatment plan. Obtains patient insurance benefit information for all aspects of the treatment, including, but not limited to, inpatient and outpatient services, prescription drugs, and travel and housing, if necessary. Assesses need for alternative coverage sources.
2. Verifies insurance coverage and obtains authorizations and notifications throughout the patient’s treatment. Obtains all necessary signatures and documentation required by the patient’s insurance plan. Accurately and completely documents all information into the patient records system to ensure maximum reimbursement. Monitors and updates information regarding insurance data, authorizations, preferred providers and changes in patient’s treatment plan. Partners with the clinical care team and insurance provider to ensure continued coverage of patient’s care and maximum reimbursement and minimized financial impact to the patient.
3. Provides financial counseling to patients and their families and serves as the primary resource throughout the patient’s treatment. Discusses benefits and other financial issues with patients and/or family members during initial referral and during continuation of care. Advises patients on insurance and billing issues and options. Explains company financial policies and provides information as to available resources and avenues for alternative payment arrangements. Assists patients, families and team members in addressing insurance coverage gaps via alternative funding options.
4. Provides financial advocacy, assistance and support to patients and families, as needed. Assists patients who are un-insured to access other funding resources and completes required documents. Maintains current working knowledge of Medicare, Medicaid and other program benefits and criteria, particularly as they pertain to long-term care and low-income patients. May serve as a liaison between the facility and community in making community resources available to the patient and family.
5. Acts as a liaison between patient/PFS department/payer to enhance account receivables performance, resolve outstanding issues and/or patient concerns, and to maximize service excellence.
6. Calculates patient liability according to verification of insurance benefits, collects deposits and co-payments.
7. May provide leadership and training to other members of the financial team and serves as a resource for internal and external customers.
8. Works independently under general supervision, leads and follows structured work routines. Works in a fast paced, multi task environment with high volume and immediacy needs requiring independent decision making and sound judgment to prioritize work and ensure appropriateness and timeliness of each patient’s care. This position requires the ability to retain large amounts of changing payor information/knowledge crucial to attaining reimbursement for the services provided. This position is an integral part of the care team, as they serve as the primary contact for all financial aspects of the patient’s care, both for internal and external customers. MINIMUM QUALIFICATIONS
Requires knowledge as typically obtained through an Associate’s Degree, with a focus in social work, healthcare administration or finance.
Requires knowledge of medical terminology and an understanding of all common insurance and payor types, authorization requirements and alternative financial resources as typically obtained through a minimum of three years of diversified experience in a hospital Patient Registration/Financial Services setting. Must have highly developed interpersonal, communications and human relations skills. Must also possess accurate and efficient keyboarding skills, strong organizational and time management skills and flexibility in responding to multiple demands.

May perform a wide variety of advanced patient care duties under the supervision of a Registered Nurse and maintains the patient care and unit environment consistent with the policies, procedures, and practices. POSITION SUMMARY
This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. Job Summary:

Administer immunizations and provide vaccine education to eligible patients, helping improve community health outcomes while expanding pharmacist-led preventive care services.
Collaborate closely with physicians, nurses, case managers, and other healthcare professionals to optimize patient outcomes through medication therapy management, antimicrobial stewardship, pharmacokinetic dosing, adverse drug event prevention, and evidence-based therapeutic recommendations.

Performs a variety of duties involving the transportation of patients throughout the hospital. Assists nursing staff in the physical, lateral transfer of patients before and after patient transport. Collects and delivers items to and from areas including the Labs, Pharmacy, Radiology, Medical Records, S.P.D., Mail Room and patient care areas. Cleans, sanitizes and delivers beds and transport equipment; maintains job activity logs as required.I. Major Responsibilities:
1. Escorts ambulatory patients. Transports patient by wheelchair, stretcher or bed. Prior to transport, verifies identification of all patients assigned by checking 2 identifiers and signing off on Safe Patient Transport Forms; notifies clinician if discrepancy on forms. To ensure patient comfort and safety during transport process, monitors and responds to basic patient needs before, during and after transport. Provides appropriate handoff communication when patient is delivered to receiving unit.
2. Assists nursing staff and/or lift tech in moving and positioning of patients using various lift devices in preparation of patient for transport and returning patient to bed. Includes lateral transfer of patients on precaution status that requires Patient Transporter to use personal protection equipment and to follow infection control process per precaution isolation-type.
3. Delivers stretchers, beds, cribs and wheelchairs as needed. Assists in deep-cleaning of all transport equipment on a biannual basis. Sanitizes transport vehicles and ancillary equipment between patient use.
4. Transports patient specimens to and from the Lab, Pathology, and blood products to/from the Blood Bank in accordance with established infection control guidelines. Transports contaminated equipment to Sterile Processing within same infection control guidelines.
5. Delivers other materials such as medical records, pharmaceuticals, X-ray films, sterile items, and miscellaneous reports and items as required.

Systems Administration provides design, implementation, maintenance, and support services for server level enterprise applications and medical devices. System Administrators (SysAdmins) are responsible for maintaining end-to-end knowledge of, assessing the impact of organizational change on, and identifying the opportunities and threats surrounding the systems they support. - Serves as a Tier-II support resource, engaging other team resources as necessary and as defined by support processes.

Responsible for the provision of pharmaceutical care at West Virginia University Hospitals through involvement in direct patient care, operational, and educational activities.MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Must possess current license as required by state board where services will be provided:
WV: Pharmacist license through the West Virginia State Board of Pharmacy
2. Obtain certification in Basic Life Support within 60 days of hire date
PREFERRED QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Pharm.D. from an ACPE accredited school of pharmacy
EXPERIENCE:
1.Candidates should have experience in at least ONE of the following areas:
Completion of a PGY-2 Residency in an applicable clinical field
Completion of a PGY-1 Residency
Three years of relevant pharmacist experience

1. Reviews each order to verify and confirm accuracy and appropriateness.
2. Dispenses medications from the provider’s order.
3. Ensures that policies and procedures related to drug distribution are followed.
4. Responsible for drug use control in assigned service areas.
5. Identifies problems related to aspects of drug use control and offers constructive ideas for resolution.
6. Assumes responsibility for and makes decisions related to the provision of pharmaceutical services in assigned service area.
7. Detects, reports, and helps to minimize or manage adverse drug reactions and medication errors.
8. Participates in the management of acute and chronic disease states, medical emergencies, and adverse drug reactions.
9. Communicates relevant findings and recommendations and documents them in the clinical information system.
10. Ensures that necessary records are appropriately maintained according to legal requirements and hospital policies and procedures.
11. Supervises technicians in drug preparation and distribution duties.
12. Provides objective information on the performance of co-workers (both within and outside of the pharmacy department) to facilitate performance management.
13. Participates in the orientation and education of pharmacy students, pharmacy residents, and/or new department employees.
14. Participates in educational and scholarly activities related to drug therapy to enhance personal knowledge as well as that of other health professionals at West Virginia University Hospitals.
15. Participates in planning and implementing changes in the pharmacy and improves productivity through participation in staff meetings and projects.
16. Routinely completes and submits written assignments in a timely manner, continuing education documentation, and other items as assigned.
17. Successfully completes annual clinical competency evaluations.

Additional Job Description:
Shift Hours:
- 7 days on/7 days off: 9:30 pm - 9 am
- Weeks rotate Monday night - Monday morning
Daily Tasks:
- Operational product verification for sterile compounded products and non-sterile compounded products
- Product verifications from pharmacy automation, such as Boxpicker
- Clinical queue verification support
- Support response to patient codes
- Float to different night roles (N-Med/Onc and N-Med/ICU) as needed

Under the leadership of the Chief Medical Officer, the Venice Family Clinic physician evaluates, treats, and makes appropriate diagnostic and specialty referrals for our patients. The physician is an integral part of the clinic staff, providing leadership and expertise on medical and patient care issues. Because the Venice Family Clinic has a large volunteer program, the staff physician has a unique role in orienting and providing technical support to the volunteer medical providers. This responsibility includes providing a positive role model, ensuring that volunteers understand Venice Family Clinic procedures and providing continuity of care for patients. The physician works cooperatively all Clinic departments including nursing, pharmacy, clinic coordinators, and Mental Health staff. The position requires flexibility in days and hours for clinical care, as some evening are included in the weekly schedule.
The University of California, Los Angeles is required to provide an estimate of the salary range for this role. This range takes into account the wide range of factors that are considered in making compensation decisions. Salary offers are determined based on candidate qualifications and experience. Salary range: $235,000.00-$250,000.00 Annual
