PHARMACIST - Per Diem (PRN)
VytlOne (formerly known as Maxor National Pharmacy Services) is seeking a kind, patient and caring Per Diem (PRN) Staff Pharmacist for our Vermont locations. We are looking for a Pharmacist to join our dedicated team of pharmacy professionals at this community healthcare center based, in house pharmacy. In this integrated model, the Maxor pharmacy team will work closely with providers and clinic staff collaboratively, providing team-based medicine with a strong pharmacy component to provide optimal care for patients. Prior retail pharmacy experience is required, as well as the ability to work in a fast paced, busy environment. This pharmacy is managed by Maxor National Pharmacy Services, LLC

Responsibilities:
ESSENTIAL FUNCTIONS (These duties must be performed with or without reasonable accommodation. This document in no way states or implies that these are the only duties to be performed by the employee occupying this position):
Interpret and fill drug orders.
Provide patient information and counseling.
In the absence of store Pharmacist-In-Charge/Pharmacy Manager, may be required to act as the alternate store leader, responsible for managing ongoing functions of the pharmacy.
Contact and confer with physicians regarding questions or irregularities on prescriptions.
Dispense prescriptions, compound, and prepare I.V. admixtures (if applicable).
Monitor drug therapy for contraindications, interactions, allergies and appropriateness.
Maintain patients’ medication records.
Maintain inventory and records of controlled drugs and other drugs as required by law.
Supervise pharmacy technicians in routine performance of their duties.
Provide drug information to professional staff and patients as necessary.
Participate in inventory control programs such as ordering, inventorying and monitoring correct pricing and quantities.
Attend and participate in programs, committees, meetings, and functions as required by the facility or pharmacy.
Participate in educational programs for professional staff as necessary.
Participate in pharmacy orientation programs, pharmacy staff meetings and in-service educational programs.
Participate in activities resolving unsafe and unsanitary practices.
Exemplify excellence in customer communications and service.
Work cooperatively with other staff members to enhance the team concept.
Participate in the Performance Improvement Program.
Maintains regular attendance in accordance with established policies.
Promptly report any allegations of impropriety to the Compliance Department.
Comply with Maxor’s Ethical Business Conduct policy and Maxor’s Compliance Program.
Remain free from exclusion under the OIG and SAMS Medicare/Medicaid lists.
Complete required training and ensure the departmental staff training is completed as required.
Must be able to cope with the mental and emotional stress of the position.
Ensure staff is informed of product upgrades or enhancements in a timely manner.
May assist management with hiring and training competent staff.
On a biweekly basis, verify departmental timesheets for departmental employees ensuring the timesheets are accurate based on the work-schedule and necessary corrections are done prior to Payroll submission.
Develop and mentor departmental staff by providing career coaching; motivate departmental staff, individually and as a group; understand and support diversity; and establish an effective team.
May assist with conducting initial and annual performance evaluations.
Promote teamwork; discipline staff when warranted.
Qualifications:
Education:
Bachelor’s degree, or Pharm. D., from an approved school of pharmacy and registration by the state board of licensure.
Licensed to practice pharmacy in Vermont
One-year experience as a practicing pharmacist is preferred.
Knowledge, Skills and Abilities:
Knowledge of drugs and their indications, contraindications, dosing, side effects, and proper administration.
Ability to communicate technical information to non-technical personnel.
Knowledge of polices and regulations pertaining to the disbursement of pharmaceutical drugs.
Ability to maintain quality, safety, and/or infection control standards.
Ability to supervise and train assigned staff.
Knowledge of related accreditation and certification requirements (if applicable).
Skill in the use of personal computers and related software applications.

About the Role
We are seeking a dedicated and detail-oriented Registered Pharmacist (PRN) to join a fun and friendly pharmacy team. In this role, you will oversee all aspects of drug dispensing, ensure the accuracy and completeness of technician work prior to medication delivery, and support the supervision of pharmacy staff to ensure full compliance with company policies and procedures. This is an excellent opportunity for a licensed pharmacist looking for flexible, as-needed hours in a collaborative and patient-focused environment.
Key Responsibilities
Maintain full compliance with all state and federal regulatory standards applicable to pharmacy practice.
Demonstrate a thorough understanding of pharmacy-related laws and share relevant information with staff and colleagues as needed.
Review all new prescriptions to verify appropriate dosing and diagnosis alignment.
Review new prescriptions for prescribing physician appropriateness.
Notify physicians of any concerns related to prescribing practices or specific prescriptions.
Clarify orders with physicians or nurses as needed, and complete verbal orders accordingly.
Accurately dispense and prepare medications for delivery.
Support pharmacy consultation calls and maintain proper documentation.
Provide pharmaceutical counseling to patients, guardians, physicians, nurses, physician office staff, and internal employees as needed.
Assist with the daily duties of the pharmacy coordinator as necessary.
Verify proper storage of products and confirm all technician-prepared prescriptions within established timeframes.
Perform other related duties as assigned.
Supervisory Responsibilities
Assist in supervising and reviewing the work of pharmacy coordinators.
Oversee work assignments for pharmacy coordinators and technicians to ensure efficiency and quality.
Qualifications
Strong verbal and written communication skills, with the ability to organize and supervise pharmacy support staff and communicate clearly with nurses, physicians, and patients.
Solid pharmaceutical and clinical knowledge to appropriately dispense and monitor drug therapy.
Ability to work effectively under pressure and prioritize tasks during high-volume periods.

Melissa Memorial Hospital is a 15-bed critical access hospital, located in Holyoke, Colorado. While being “rural” we are equipped for any situation. It is a model rural hospital with 1 operating room, surgical procedure room, 4 bay emergency room, and radiology including 64-slice CT scanner and mammography, MRI, ECHO, and Nuclear Medicine, along with other ancillary services. The hospital is co-located with a family practice clinic and retail pharmacy services. - Obtain & record in the electronic medical record an allergy history for each patient. In doing so, clarify & record the nature of the allergic reaction when necessary.
- Consult with a pharmacist in all cases of unresolved uncertainties related to a patient’s medication & allergy history.

Serves as a liaison between the customer and the pharmacist. Effectively uses pharmacy computer systems to investigate and respond to missing medication requests. Consistently demonstrates an ability to assess a situation, consider alternatives, and choose the appropriate course of action. Demonstrates proficiency in preparing compounded intravenous medications, total parenteral nutrition products, and chemotherapeutic agents within compliance with USP <797>/<800>. Ensures that the sterile compounding environment and all equipment are cleaned and maintained according to policy. May educate and train peer Pharmacy Technicians, Technician Externs, Technician Trainees, and Volunteers. Must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients.
CORE FUNCTIONS
1. Identifies medications and other items for reorder as necessary to maintain a continuous inventory of medications and supplies. Receives, checks-in, and restocks medications ordered through the wholesaler and other vendors verifying all quantities against invoices. Assesses practical ways to deal with shorted items and short-dated or unusable items.
2. Manages users within the automated dispensing technology database as per policy. Investigates discrepancies and troubleshoots malfunctions in automated dispensing technology.
3. Exhibits excellent interpersonal skills and is customer service oriented in all contacts with patients, visitors and staff. Responds to standard requests from internal customers. Interacts primarily with department personnel, cross-department personnel, internal customers, patients and physicians.
4. May use automated dispensing technology or requisition to restock medications to nursing and ancillary areas after checking by a pharmacist. When allowed by law and policy, and upon completion of proper training/competency assessment, may restock automated dispensing technology using barcode scanning verification devices. Restocks medications to all areas outside automated dispensing technology which may include but not limited to trays, code carts and emergency boxes.
5. Compounds non-sterile medication products. Understands and appropriately follows all product worksheets/recipes for documentation and check processes. Understands and adheres to all applicable USP compounding guidelines. Demonstrates knowledge of and consistently performs calibrations of equipment and accurately measures components for compounding. Repackages bulk medications into unit of use packaging. Completes accurate documentation of medications compounded and repackaged per policy.
6. Maintains a safe and clean working environment by complying with all policies, laws, and regulations. Performs quality control activities such as monthly nursing unit inspections and pharmacy section checks in a timely manner. Completes documentation of refrigerator temperatures, and equipment maintenance activities as required.
7. Ensures the proper use of garbing and compounding techniques in accordance with policies and has knowledge of basic principles within USP <797> and USP<800>, laws, policies, and procedures that pertain to compounding. Performs compounding and labeling of intravenous compounds and hazardous agents to include chemotherapy using proper sterile and aseptic technique as appropriate to the site of care. Understands the different product lines in sterile compounding (patient specific and non-patient specific batch) and the differences between the various populations (Adult, Pediatric, Neonate). Completes all required environmental cleaning procedures. Documents all required tasks and other information per policy. Understands the safe handling of hazardous drugs and demonstrated the correct receipt, storage, labelling and transport process.
8. Handles narcotic medications following all federal and legal requirements as set through policy and procedure. Understands all regulatory requirements for receiving, compounding, repackaging, and distribution for controlled substances. Consistently adheres to all storage and inventory movement documentation requirements for controlled substances.
Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. - Procures and distributes medication orders per physician request, pharmacist request, or prearranged work assignment according to established policies, procedures, and protocols.

- Competent in understanding and usage of computers and software programs, including Microsoft Office Suite and Google. Ability to learn new applications relevant to the position.
- Basic Life Support (BLS) Certification as appropriate to practice site.
Qualifications
- High School or equivalent is required
- Current pharmacy technician certification and/or license applicable to state of practice.
- Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy and the distribution of approved drugs, controlled substances, and investigational agents.
- Any lapse or failure to meet these requirements may result in disciplinary action, up to termination, at the hiring manager's discretion.
