Saint Alphonsus Medical Group (SAMG) is a physician led and accountable 800+ provider multispecialty group practice that has an excellent opportunity for an APP, to be part of the SAMG Comprehensive Pain & Spine Service. The Service is seeking an APP to practice primarily within the Boise Hospital serving the local area. The qualified candidate will be responsible for all aspects of a traditional Comprehensive Pain & Spine clinic, including medication management, clinic interventions, referral, and physician procedure work up as well as follow up/post op visits. The well-qualified candidate will also commit to and demonstrate a multi-disciplinary team approach to patient care and high levels of compassion for this vulnerable population.
Support: 24/7 CRNA Anesthesia; Level II Trauma Center in Boise; ED; Saint Alphonsus FM/UC/Ortho/Occ Med/PT clinics on-campus in Boise; community family medicine clinics.
Requirements:
- An Unrestricted Physician Assistant or Nurse Practitioner medical license in Idaho and Oregon; valid controlled substance registration with Idaho and Oregon; Board of Pharmacy and DEA.
- A strong medical background and a desire to develop a practice to support a growing multi-specialty medical group.
- Two years' experience is preferred

We’re Looking For:
- Completion of an accredited or approved program in nursing

- ACLS required within 6 months of hire

Med/Surge Garden is a 24-bed medical/surgical unit with mixed acuity, specializing in the care of stroke and oncology patients and Med Surg I is a 24-bed acute care unit specializes in medical-surgical, orthopedic, surgical, and trauma care, serving primarily surgical and orthopedic patients. Nurse-to-patient ratios for both Med Surg units are typically 5:1, and nursing assistant ratios are 6-7:1.

Position Summary:
This is a role for someone who wants their analytical work to directly shape what gets built and what gets prioritized. Embedded within Geisinger AI product function, you will turn complex clinical and operational data into the evidence that drives portfolio decisions, validates AI performance in production, and informs how the organization communicates value to leadership. You will work closely with product managers, data scientists, and clinical partners across a broad range of AI initiatives spanning care delivery, revenue cycle, operations, and more.

Key Responsibilities:
- Partner with product managers during problem sizing to quantify opportunity areas, establish baselines, and define what success looks like before any solution is built
- Conduct exploratory data analysis during model development, profiling data quality, understanding distributions, identifying anomalies, and helping data scientists understand what the data is actually telling them
- Build and maintain the feature datasets and data definitions that model development depends on, ensuring the analytical foundation is sound before anyone builds on it
- Design and own the measurement frameworks that track AI performance in production, going beyond adoption to capture clinical and operational impact
- Dig into Epic, Clarity, Caboodle, and other enterprise data sources to answer hard questions about how AI initiatives are performing and where they need to improve
- Identify and resolve data quality issues at the source, working with clinical and operational teams to address problems that would otherwise degrade model performance silently
- Turn complex analytical findings into clear narratives and visualizations that help product and strategy leaders make faster, better decisions
- Establish and maintain data standards and documentation that make analytical work reusable and trustworthy across the portfolio

- Oversees and/or participates in assuring appropriate medications are prepared and delivered timely to care areas and are stored both securely and appropriate for both stability and effective workflow.
- Provides counseling to patients and promotes quality medication education and adherence. Ensures patient is well informed, treated with respect, and receives the highest level of professional service.

The RN Transition and Triage Care Manager offers comprehensive, time-limited services to patients and their families, ensuring continuity of care as they move across healthcare settings and clinicians. TYPICAL EXPERIENCE:
- 1 year of recent relevant experience.
- Minimum of two (2) years of professional experience in a managed care organization, healthcare eligibility, enrollment, or coordination of benefits role required or in lieu of experience, an Associate’s Degree.
- Experience with Medicare and/or Medi-Cal eligibility, redetermination, and PACE processes strongly preferred.
- Familiarity with DHCS and CMS systems (834 files, DTTRs, MARx) is a plus.
- Minimum of one (1) year of documented experience working with a frail or elderly population.
Duties include, but are not limited to:
- Eligibility Monitoring and Verification:
- Monitor and track daily participant eligibility and enrollment status as reported by the Department of Health Care Services (DHCS) and the Centers for Medicare & Medicaid Services (CMS).
- Review and reconcile 820/834 eligibility and payment files from DHCS and Daily Transaction Reply Reports (DTRRs) from CMS.
- Track Medicare entitlement dates and work closely with the Intake team to ensure timely and accurate enrollment into the PACE program.
- Medi-Cal Redetermination Assistance
- Track upcoming Medi-Cal annual redeterminations (financial, categorical, and residency) at least 120 days in advance using DHCS 834 reports, MEDS data, and internal records.
- Conduct outreach (phone, mail, in-person, text) and document consent for assistance using authorized representative forms (e.g., MC382/MC383).
- Occasionally assist the Patient Advocate and participants/representatives with gathering verifications, completing forms, and submitting renewals to the county (via portal, mail, or e-fax) during the process and 90-day cure period.
- Follow up on status, assist with appeals or reapplications if needed, and notify IDT of any potential coverage gaps.
- Maintain data files in Excel for tracking and reporting.
- Reinstatement and Retroactive Enrollment Support
- Identify cases of accidental enrollment in another Medi-Cal or Medicare plan and Medi-Cal-only participants turning 65.
- Occasionally perform outreach, obtain consent, and gather required documentation (signed Enrollment Agreement, IDT assessments, eligibility verifications).
- Coordinate disenrollment from other plans and prepare/submit retroactive requests to the CMS Retroactive Processing Contractor (RPC) within 90 days (or escalate older cases).
- Submit DHCS Enrollment/Disenrollment Request Logs per policy letters.
- Align dates for age-related Medicare entitlement transitions and ensure seamless PACE coverage.
- Maintain the data files in Excel and report outcomes to leadership.
- Enrollment, Disenrollment, and Cross-Departmental Support:
- Update and maintain enrollment/eligibility databases, tracking spreadsheets, and participant records.
- Assist all departments in confirming and verifying participant eligibility for internal use and external inquiries (providers, CMS, DHCS).
- Collaborate with Intake, Social Work, Claims, and Pharmacy teams to resolve eligibility, redetermination, reinstatement, and COB barriers. Communicate resolutions clearly and supportively to participants and families.
- Compliance and Team Collaboration:
- Maintain current knowledge of PACE eligibility, enrollment, redetermination, reinstatement, retroactive processing, and COB requirements under CMS (42 CFR Part 460) and DHCS regulations
- Occasional travel to participants’ homes, PACE centers, or county offices may be required for outreach, redetermination assistance, eligibility coordination, or other job-related duties. As a Registered Nurse at MercyOne, you will provide and direct safe, effective, and culturally competent care for patients with acute medical and surgical conditions. Through evidence-based practice, utilizes critical thinking to assess, plan, implement, and evaluate outcome-based care. The Acute Care RN manages a diverse patient population with varying levels of acuity, including post-operative, medical, telemetry, and neurologic patients. Supervises and coordinates care provided by LPNs, Patient Care Assistants, and other nursing team members. Facilitates communication with physicians, patients and families, and interdisciplinary team members. Adheres to MercyOne’s performance expectations and upholds the Mission, Vision, and Values of MercyOne.
Essential Functions:
1. Conducts an initial assessment to gather data.
- Makes an initial assessment of new admissions within 30 minutes of the patient’s arrival to the unit.
- Completes all areas of the admission assessment database within 24 hours of admission.
2. Identifies patient problems and develops them into an outcome/goal-based plan of care.
- Initiates the patient problem list and incorporates relevant medical history into the plan of care.
- Analyzes and interprets the assessment data to identify desired patient outcomes/goals.
- Determines nursing interventions to achieve those outcomes/goals.
- Prioritizes patient care needs/ activities.
- Identifies and makes appropriate referrals (i.e., Patient/Family Services, Nutritional Services, Spiritual Care, Diabetic Educator, etc.), and education needs.
- Collaborates with other health care disciplines to plan interventions/ patient outcomes/goals.
3. Implements the outcome/goal- based plan of care.
- Assigns and coordinates patient care incorporating patient acuity and the skills, knowledge and abilities of the team members.
- Organizes and prioritizes care for the patients within the team.
- Delegates tasks appropriately to other team members.
- Implements the plan of care including procedures, treatments, physicians orders, nursing orders, education plan and medications.

5. Communicates relevant clinical information to the team to optimize patient outcomes/goals.
- Communicates relevant clinical information to physicians regarding the patient’s condition.
- Reports progress toward outcomes/goals and/or changes in condition to other health team members.
- Maintains ongoing communication with interdisciplinary team members throughout the shift, at shift change, and transfer of care.
- Communicates with other team leaders and members throughout the shift to reassess and prioritize the needs of the unit.
- Documents completely, comprehensively and legibly and in accordance with policy.
6. Demonstrates clinical competence.
- Utilizes Patient Care Policies and Procedure as references to guide nursing practice.
- Assures medication safety by following the “5 Rights” of medication administration.
- Prioritizes and implements intervention according to patient assessment.
- Demonstrates competency in computer systems and applications required for role.
- Receives, transcribes and processes and implements physician orders per policy including dating, timing and verifying accuracy.

We’re Looking For:
- Completion of an accredited or approved program in nursing
- Candidates must have at least 6 months of RN experience, this position is not for new graduates
- Registered Nurse license issued by the state in which the teammate practices
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association
- ACLS required within 6 months of hire

Position Summary / Career Interest:
The Accreditation Coordinator plays a critical role in maintaining the organization's compliance with healthcare accrediting bodies such as The Joint Commission (TJC) and regulatory agencies such as the Centers for Medicare & Medicaid Services (CMS). This position ensures continuous compliance and readiness for surveys by monitoring standards, coordinating accreditation activities, and facilitating internal audits. The coordinator participates in the development and implementation of corrective action plans, collaborates with clinical and administrative teams to address deficiencies, and promotes a culture of quality and safety throughout the hospital. Strong organizational, communication, and analytical skills are essential to drive accreditation excellence and adherence to standards.Responsibilities and Essential Job Functions- Develops and maintains accreditation and regulatory readiness activities to support safety and ongoing regulatory compliance.
- Collects, analyzes and reports data in support of continuous readiness activities.
- Performs in-person patient and departmental tracers and medical record review audits to assist in determining compliance with accreditation and regulatory requirements.
- Disseminates the results of audits and surveillance as needed and as data is available. Dissemination occurs via electronic communication, presentations at organization-wide venues, and directly with hospital leaders.
- Interprets standards in relation to policies and practice.
- Maintains knowledge and serves as a resource for interpretation of accreditation and disease-specific care certification standards as applicable. Provides formal and informal education on regulatory requirements as needed.
- Utilizes quality improvement techniques and tools, leads/assists in the development and implementation of detailed action plans to achieve compliance.

- Releases and confirms ordered tests to appropriate departments.
