Specialty Pharmacy Clinical TeamsJob Summary:
Provides leadership in optimal product preparation and dosing of medications, and provision of patient specific drug information to include patient specific dosing. Aides in the prevention and reporting of medication errors and adverse drug reactions according to policy. Supports quality relationships with health care professionals, families, and patients; and the pharmacist is actively involved in being further educated and providing education in all appropriate ways..
Position Highlights
- Hours are Monday - Friday typically 8:00am - 4:30pm.
- Currently no required weekends or holidays.
- Maintains the quality, safety, and consistency of clinical pharmacy services in assigned areas.
- The day-to-day assignments will focus on ensuring patients obtain medication access via completion of Benefit Investigation, PA/Appeal Completion, Enrollment in Patient Assistance Programs, and addressing any barriers reported as well as ensure compliance and efficacy by completing timely patient assessments and chart review.
Key Responsibilities
- Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications.
- Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
- Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy.
- Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.

Technical Capabilities
- Medication Management (Intermediate): Demonstrates proficiency in medication management in practical applications of a difficult nature. Possesses sufficient knowledge, training, and experience to be capable of successfully conducting Medication Management without requiring support and instruction from others. Has the ability to determine the clinical appropriateness of each medication taken by the patient, determine the appropriateness of the dose, dosage form, indication, contraindications, potential adverse effects and potential problems with concomitant medications. Able to monitor for therapeutic duplication or other unnecessary medications, interpret, monitor and assessing patient's laboratory results as they relate to medication therapy and provide drug information to other healthcare professionals, patients and their families.
- Pharmaceutical Drug Preparation & Dispensing (Novice): Possesses sufficient fundamental proficiency to successfully review, prepare and dispense prescribed medications. This includes being proficient in the general principles of aseptic technique utilized to compound sterile preparations (CSPs) to minimize the risk of contamination (where applicable).
- Mentoring & Coaching (Novice): Constructively trains and mentors others by providing positive direction and feedback. Has provided constructive suggestions to improve a situation. Knows how to remove roadblocks to success and avoids judgments at all times. Makes a concerted effort to pass on experiences and job information that can be learned easily.
- Pharmaceutical Computer Literacy (Novice): Has demonstrated a proficient understanding of computers or has successfully completed training for applications that are used to support medication use processes. Is capable of using the fundamental features of pharmaceutical computer applications to support patient care issues of moderate difficulty.
- Pharmaceutical Quality Assurance & Compliance (Advanced): Possesses specialized knowledge of the appropriate rules and regulations and applies them in difficult, stressful and complex situations. Demonstrates expertise and advanced knowledge of process improvement, and quality assurance techniques. Understands the principles upon which the rules and regulations are based. Demonstrates a commitment to continuous quality improvement and maintains compliance with a federal, state and local regulations as well as VUMC policies and procedures. Able to explain rules and regulations that are ambiguous or unclear.

SUMMARY: Reports to Manager, Retail Pharmacy Services. Utilizes experience-based knowledge to perform more advanced technical functions in preparing, formulating and packaging medication and pharmaceutical supplies. Assists the Pharmacy Technician Group leader to coordinate activities in keeping with the scope of services provided, and with consideration to the age of the patients/customers being served. Interacts professionally as needed with patients/customers, practitioners and their support personnel to obtain accurate information, including insurance information, needed to process patient/customer prescriptions, and to process accurate payment transactions. Accesses, inputs and retrieves necessary information using multiple pharmacy system applications. Enters pertinent patient/customer data into appropriate pharmacy systems in order to ensure accurate charging for services and processing of insurance billing/payment as well as patient/customer co-pay information. Collects and processes payment for patient/customer transactions. Assists patients/customers in resolving identified problems. Refers patients/customer questions concerning medications and drug interactions to thestrike /strikepharmacist on duty. Determines patient needs and as appropriate, facilitates referral to case management, if required. Promotes and facilitates available pharmacy services including delivery of prescriptions and/or medical equipment to discharged patient’s bedside. Rotates to all applicable work assignments and work shifts as business need dictates and contingent upon individual demonstrated competency of necessary skills and knowledge for the respective area. Assumes responsibility for assigned areas of the pharmacy including inventory management, medication purchasing, medication compounding and repackaging, dispensing, and sales.strike /strike Prepares, formulates and packages medications including hazardous substances, controlled substances, and other pharmaceutical supplies under the immediate supervision of a pharmacist. WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS: Must have the physical ability to lift, carry and maneuver medication delivery equipment and pharmaceutical related inventories, requiring the ability to grasp instruments and use finger dexterity.Work may require extended periods of time spent standing and walking. Work requires the visual acuity and manual dexterity necessary to operate a personal computer, to accurately input data into various applications and systems, and to navigate screens on computer monitors. Major Responsibilities
- Assists licensed pharmacist in technical duties related to preparation and distribution of medications
- Accurately selects, prepares, and labels medications for patients including hazardous drugs and controlled substances
- Assists with medication distribution tasks
- Screens phone calls and routes as needed using proper telephone and voicemail etiquette

This job delivers medications to designated areas under close supervision and according to detailed instructions, policies and procedures. The Certified Pharmacy Technician II provides daily supportive services for the Department of Pharmacy according to hospital policies and procedures and in compliance with Kentucky pharmacy laws and other regulatory agencies. The technician actively supports and participates in the department’s continuous quality improvement initiatives and competency assessments and demonstrates a commitment to customer service and patient safety. The technician responds to requests in a prompt and efficient manner. Major job responsibilities include: distributes medications, including controlled substances, to a secure place for storage or administration, restocks automated dispensing cabinets (ADC), compounds sterile and non-sterile products, communicates with hospital staff, completes inspections, and reports unsafe practices to leadership.
Essential Functions:
Provides supportive services to the inpatient pharmacy
- Assists with filling, processing, and dispensing of medications by labeling medication, including bulk supplies, with proper ancillary labeling and bagging procedures, with final check performed by a pharmacist
- Answers phones according to established phone etiquette and triages calls to the proper personnel
- Charges and/or credits patients for medications used via the computer system
- Accurately and efficiently unit doses medications including cleaning the compounding area, restocking and replacing supplies, documenting in the compounding record, and ensuring doses are correctly labeled and prepared for pharmacist verification
- Prepares medications for loading into the ADC and unloads medications from the ADC utilizing bar code scanning
- Fills inventory requests for nursing unit’s ADCs, floor stock, and fills patient specific drawers by placing the proper bins according to census profile
- Supports the drug inventory process including ordering, rotation, and restocking of drug dispensing areas with drugs and supplies needed to process patient medications orders
- Ensures the maintenance of a clean and orderly work area, including a sterile environment
- Completes nursing unit and inventory inspections, as assigned. Restocks kits and crash cart trays, as required
- Fills controlled substance orders, reconciles all discrepancies and closes all loops on a daily basis
- Monitors medication activity for potential drug diversion and reports any discrepancies to the pharmacist or pharmacy leadership
- Delivers medications, including controlled substances, to nursing units’ ADC as scheduled and for emergent deliveries as necessary
- Assists with ensuring medications are delivered efficiently and accurately to patient care unit areas, including checking the tube system with each delivery, retrieving and transporting medications to the appropriate location

POSITION SUMMARY
This position will work under the supervision of the pharmacist performing routine duties in packaging, distribution, non-sterile compounding, and record-keeping of pharmaceutical supplies. Serves as a liaison between the customer and the pharmacist. Becomes proficient in preparing compounded intravenous medications, total parenteral nutrition products, and chemotherapeutic agents within compliance with USP regulations. CORE FUNCTIONS
1. Identifies medications and other items for reorder as necessary to maintain a continuous inventory of medications and supplies. Receives, checks-in, and restocks medications ordered through the wholesaler and other vendors verifying all quantities against invoices. Assesses practical ways to deal with shorted items and short-dated or unusable items.
2. Manages users within the automated dispensing technology database as per policy. Investigates discrepancies and troubleshoots malfunctions in automated dispensing technology.
3. Exhibits excellent interpersonal skills and is customer service oriented in all contacts with patients, visitors and staff. Responds to standard requests from internal customers. Interacts primarily with department personnel, cross-department personnel, internal customers, patients and physicians.
4. May use automated dispensing technology or requisition to restock medications to nursing and ancillary areas after checking by a pharmacist. When allowed by law and policy, and upon completion of proper training/competency assessment, may restock automated dispensing technology using barcode scanning verification devices. Restocks medications to all areas outside automated dispensing technology which may include but not limited to trays, code carts and emergency boxes.
5. Compounds non-sterile medication products. Understands and appropriately follows all product worksheets/recipes for documentation and check processes. Understands and adheres to all applicable USP compounding guidelines. Demonstrates knowledge of and consistently performs calibrations of equipment and accurately measures components for compounding. Repackages bulk medications into unit of use packaging. Completes accurate documentation of medications compounded and repackaged per policy.
6. Maintains a safe and clean working environment by complying with all policies, laws, and regulations. Performs quality control activities such as monthly nursing unit inspections and pharmacy section checks in a timely manner. Completes documentation of refrigerator temperatures, and equipment maintenance activities as required.
7. Handles narcotic medications following all federal and legal requirements as set through policy and procedure. Understands all regulatory requirements for receiving, compounding, repackaging, and distribution for controlled substances. Consistently adheres to all storage and inventory movement documentation requirements for controlled substances.
8. Becomes proficient in performing compounding and labeling of intravenous non-hazardous compounds using proper sterile and aseptic technique as appropriate to the site of care. Understands the different product lines in sterile populations (adult, pediatric, and neonate). Completes all required environmental cleaning procedures. This position will prepare, dispense, and compound medication under the direct supervision of a licensed pharmacist. Provide prescription support services for patients to ensure safe, appropriate, effective and efficient delivery of medication therapy. Provide excellent customer service skills, high attention to detail, strong problem solving, documentation and computer skills. Work closely with the multidisciplinary healthcare team to support pharmaceutical care activities, complete pharmacy billing activities, record keeping, reporting, inventory management, and arrange appropriate medication delivery.
CORE FUNCTIONS
1.Under the direct supervision of a pharmacist, review and interpret prescriptions and input into the computer for verification by the pharmacist. Communicate with patients and initiate problem resolution on payer issues.
2.Prepare, dispense, and compound appropriate amount of medication under supervision of a pharmacist. Deliver medications accurately to the patient.
3.Answer phones, accept prescriptions, and communicate with patients in waiting area.
4. Assist with inventory, maintain a clean work environment, and assist with any other administrative duties as assigned by supervising pharmacists or senior technicians.
5. Follow policies, procedures, and regulatory requirements and perform other work assignments.
MINIMUM QUALIFICATION
Requires Pharmacy Technician licensure and active Pharmacy Technician certification OR Pharmacy Intern licensure in the state of practice.
Incumbents in Arizona are also required to provide the AZ Board of Pharmacy Wallet Card at time of hire.
Minimal experience, typically gained through 0-2 years relevant work experience. Ability to attend and successfully complete 160 hours of on the job training required. Strong customer service focus, superior problem-solving abilities. Ability to complete moderate to difficult mathematical calculations using a hand-held calculator is required.

POSITION PURPOSE
Provides daily pharmacy technical and professional services to inpatients, outpatients and other Health Center departments. Assists in all aspects of drug distribution that do not require the professional judgment of a pharmacist. Interns function in accordance with standard, written procedures and guidelines under the supervision of a pharmacist.MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Must possess current license or trainee license as required by state board where services will be provided:
MD: Pharmacy Intern or Pharmacy Technician license through Maryland Board of Pharmacy
OH: Pharmacy Intern or Pharmacy Technician license through Ohio Board of Pharmacy
WV: Pharmacy Intern License (Registration) or Pharmacy Technician license through West Virginia Board of Pharmacy
PA: Pharmacy Intern license through Pennsylvania Board of Pharmacy
2. Must be enrolled in or accepted to an accredited School of Pharmacy OR graduate of an approved college of pharmacy or graduate of a foreign pharmacy who has obtained Foreign Pharmacy Graduate Examination Committee certificate for purposes of getting required intern hours OR qualified graduate who is awaiting examination for licensure OR individual participating in a residency or fellowship program.
CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. They are not intended to be constructed as an all-inclusive list of all responsibilities and duties. Other duties may be assigned.
1. Responsible for reading physician’s orders, preparing medications, delivering medications, and ordering and delivering drug supplies for various patient‑care units.
2. Reads physician's orders and prepares medication for dispensing, including unique dosage forms for pediatric and geriatric patient populations.
3. Has an understanding of standard pharmaceutical/medical terms, abbreviations and symbols commonly used in the prescribing and dispensing of medications.
4. Interprets cart fill lists and uses this information to replenish patient's medications in medication carts and delivers contents to patient care areas according to schedule.
5. Accurately prepares and delivers controlled substances to patient units and maintains all necessary records.
6. Takes inventory and analyzes patient‑care area medication supplies, replacing outdated or deteriorated medications; replaces depleted inventory.
7. Assists in the preparation of sterile parenteral products under the supervision of a pharmacist.
8. Accurately prepares intravenous admixtures, including dilutions for pediatric patients, parenteral chemotherapy, parenteral nutrition, other injectable preparations, and sterile irrigating solutions using aseptic technique.
9. Accurately enters information related to parenteral admixtures, parenteral chemotherapy, parenteral nutrition, and other injectable products in Pharmacy IV and TPN Computer Systems.
10. Labels parenteral products appropriately.
11. Performs general operational functions of a pharmacy intern.
12. Performs arithmetic calculations for usual dosage determinations required in medication and solution preparation.
13. Performs initial check for pediatric doses for appropriateness based on weight and gestational age.
14. Performs in accordance with departmental standards established for speed and accuracy.
15. Prepares and labels prepackaged medications after determining usage patterns and prepackaging requirements. Also maintains records related to this.
16. Maintains inventory and supplies in assigned pharmacy service areas.
17. Answers telephone and channels calls to appropriate individual.
18. Ensures that the pharmacy service areas and drug storage areas in the institution are kept clean and orderly.
19. Participates in planning and implementing change and maintaining and improving productivity through attendance and participation in staff meetings, committees, task forces and projects.
20. Assists in the education of pharmacy students, pharmacy residents, new pharmacists, new pharmacy technicians and pharmacy interns.
21. Prepares, processes and reprocess patient’s prescriptions and adjudications
22. Follows up with third parties on rejected claims and coordinates efforts to fix the problem
23. Assist with medication management therapy and pharmaceutical adherence programs
24. Works and coordinates with unit floors in discharge patients’ prescription and costs.

1. Requires pushing/pulling of carts weighing up to one hundred pounds and lifting of boxes containing twenty-five or more pounds of sterile fluids.

3. Possible exposure to communicable diseases, hazardous materials and pharmacological agents with little likelihood of harm if established health precautions are followed.
