Job Summary

We are seeking a Registered Nurse (RN) to join our Mother Baby / Postpartum team at our Katy Magnet Hospital! This is a full-time, night shift (7pm - 7am) position.
Minimum Qualifications
Education: Graduate of an accredited School of Professional Nursing; Bachelor of Nursing preferred
Licenses/Certifications:
Current State of Texas license or temporary/compact license to practice professional nursing
Certified in Basic Life Support
Additional certifications may be required based on discipline and/or nursing unit requirements
Experience / Knowledge / Skills:
RN I < 1 year
RN II = 1-3 years
RN III = 3-10 years
RN IV = 10+ years
Effective oral and written communication skills
Principal Accountabilities
Delivers safe and appropriate care to patients, according to the nursing skills inventory and the age and needs of the patient/population served.
Coordinates the planning for the delivery of nursing care with an interdisciplinary team and administers appropriate clinical decisions in the delivery of patient care.
Collaborates with, directs, and may delegate to the healthcare team in the attainment of positive outcomes based on data, research, evidence, and professional standards of care.
Coordinates treatment plans; develops/implements discharge and/or transfer plans.
Participates in a systemic, interdisciplinary, and ongoing evaluation of programs, process improvement and desired client-centered outcomes.
Considers the diverse needs of each patient through the entire continuum of care, providing and ensuring the highest level of service according to established service standards, optimizing the patient experience.
Completes other duties as assigned.
Adheres to all regulatory and Texas Board of Nursing standards.
Performs other duties as assigned, within scope of practice and clinical competency.

Job Summary
The purpose of the Transfer Center RN position is to coordinate all activities and communication involved in transferring patients from one facility to another, as specified by our customers. The position is also responsible for completing the clinical reviews to determine appropriateness of transfer, as applicable.Job Description
Minimum Qualifications
Education: Bachelors of Science in Nursing (BSN), preferred
Licenses/Certifications: Current and valid license to practice as a Registered Nurse in the state of Texas
Experience / Knowledge / Skills:
- Five (5) or more years of clinical experience with two (2) years in a critical care environment or Emergency Department.
- Aware of state and federal regulatory requirements regarding patient transfers.
- Strong analytical, data management and PC skills—must be able to type minimum 25 words/min.
- Excellent interpersonal communication and negotiation skills.
- Strong organizational and time management skills—able to prioritize multiple tasks.
Principal Accountabilities
- Receives transfer requests from referring facilities and coordinates patient transfer to receiving facility using excellent customer service skills with all callers. Facilitates all communication between referring and receiving hospitals/physicians in a professional and effective manner
- Enters information into the database, as it occurs, for creation of real time data with a high degree of accuracy. Documents all required information pertaining to the patient transfer process accurately and thoroughly.
Secures Memorandum of Transfers (MOT’s) in a timely manner on every patient from the referring facility; that are compliant with state regulatory requirements for patient transfers
- Applies approved clinical criteria to evaluate appropriateness of transfer and level of care. Documents findings based on department standards
- Partners with Life Flight and specialty transport teams to coordinate flight and ground transportation as needed; Collaborates with Patient Business Services to protect the financial integrity of the organization.
- Maintains knowledge base of processes and systems by attending meetings and reviewing training materials and identifies, addresses, and works to resolve system problems impacting transfers. Escalates issues as appropriate to management.
- Assists with quality assurance (QA), identifying areas where process improvement is needed and helps drive change (i.e. day to day workflow, education, process improvements, customer service, etc.).
- Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
- Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.

Job Summary
The Clinical Documentation Integrity (CDI) Quality Reviewer responsibilities include comprehensive secondary clinical chart reviews to identify potential missed opportunities for documentation clarification, act as a liaison between coding and CDI to resolve Diagnosis-Related Group (DRG) or other code discrepancies, collaborate with CDI educator to educate CDI team based on opportunities identified in second level reviews and work directly with clinicians and providers to improve the overall quality and completeness of documentation through the query process and/or provider education. PRINCIPAL ACCOUNTABILITIES
Completes comprehensive, clinical secondary reviews of targeted patient populations to include cases with DRG and/or code discrepancies; mortality reviews to ensure documentation supports risk of mortality; hospital acquired conditions (HACs), patient safety indicators (PSIs) or other top priority diagnoses as identified for potential missed opportunities to clarify documentation or clinically validate a diagnosis.
Analyzes and interprets clinical data to identify gaps, inconsistencies, and/or opportunities for improvement in the clinical documentation and queries the provider using concurrent query process following ACDIS/AHIMA Guidelines for a Compliant Query Practice.
Acts as a liaison between the Coding Department and Clinical Documentation Integrity to reconcile discrepancies in code and/or DRG assignment.
Communicates findings of secondary reviews to Manager.
Collaborates with CDI Educator/Auditor when educational needs are identified from second level reviews.
Researches, investigates and remains up to date on both clinical and coding guidelines in quarterly Coding Clinics as they relate to physician documentation improvement needed, in an ICD-10 coding environment.
Assists in overall quality, timeliness and completeness of the quality health record to ensure appropriate data, provider communication, and quality outcomes. Serves as a resource for appropriate clinical documentation.
Documents and tracks second level reviews and results; shares this information with Manager.
Collaborates with physicians and/or other clinicians to enhance understanding of the CDI program goals; ensures the medical record can be coded accurately in order to accurately reflect patient severity of illness and risk of mortality.
