Summary:
The provision of these job responsibilities are mandated by the Centers for Medicare & Medicaid (CMS) for Medicare-approved transplant centers. As a member of the transplant team, the Solid Organ Transplant (SOT) Social Worker utilizes specialized clinical knowledge and advanced clinical skills to help in the areas of assessment, treatment of medical, patient and caregiver navigation, mental health and behavioral disorders, and discharge planning.
This position supports all programs within the departments, including Adult Congenital Heart Disease, Advanced Heart Failure, Adult Heart Transplant, and VAD.

Responsibilities:
Ensures attendance at multidisciplinary rounds or huddles and reports on psychosocial status and its implication for transitions of care.Participates in the Transplant Selection Committee; presents the mental health, social status, and functional abilities of candidates.Conducts a detailed psychosocial assessment to determine the candidacy for solid organ transplant or living donation.Participates in discharge planning including coordinating patient transfer to other facilities.Monitors progress toward the goals of the social work plan of care and revises it in response to changes in patient needs and condition.Organizes and facilitates care conferences with patients, families, representatives, and members of the multidisciplinary care team.

Summary:
As a member of the HR Shared Services team , the HR Shared Service Specialist will serve as the first point of contact and deliver front line support and to the MHS Human Resources team as well as to employees and leaders throughout the organization. Summary:
As a member of the HR Shared Services team , the HR Shared Service Specialist will serve as the first point of contact and deliver front line support and to the MHS Human Resources team as well as to employees and leaders throughout the organization. The Patient Financial Services Representative (PFSR) serves as the first point of contact in greeting patients and guarantors in the hospital, ambulatory or medical office setting. Summary:
The Navigator is responsible for guiding patients through the complexities of the healthcare system. Summary:
Under the supervision of a RN, the Monitor Technician monitors, recognizes, and interprets cardiac rhythms while identifying deviations in patients' baseline, and changes in SP02. The Monitor Technician notifies the registered nurse of changes to a patient's status and initiates the appropriate emergency response by the critical care team in the event of a lethal arrhythmia, including irregular heart rhythms and cardiac arrest.
Responsibilities:
Identifies arrhythmias, deviation in each patient's baseline, signs of patient deterioration, and intervenes to ensure patient safety.Performs continual and simultaneous surveillance for the operation of complex cardiac monitoring systems, including ECG rhythm screens, computerized dysrhythmia detection system and rhythm printers.Documents the electrocardiograph tracings of patients on telemetry monitors.Attaches electrode to patients' bodies to record electrical impulses from the heart to perform continuous ECG Monitoring.Records information in patient's log and maintains accurate, neat account of patient's cardiac monitoring status during episode of care. Identifies and records patient's rhythm and changes in patient rhythm and condition. Updates patient's records with all readings taken from the heart monitor or EKGs.Identifies and corrects for artifacts.Activates emergency response or rapid response for lethal arrhythmia or cardiac arrest.Reports abnormal or change in patient's telemetry monitoring to nursing staff or immediate supervisor or designated chain of command.

Summary:
The Patient Financial Services Representative (PFSR) serves as the first point of contact in greeting patients and guarantors in the hospital, ambulatory or medical office setting. The PFSR engages with the patient or guarantor to obtain pertinent information and answer any questions in an effort to ensure that all required demographic, financial, and insurance eligibility information is gathered and verified. Ensures all required notices and consent forms are signed accordingly.
Responsibilities:
Explains polices including all regulatory and financial consent forms; secures all required signatures.May perform patient discharge functions including, but not limited to, review of after visit summary (AVS), future appointment scheduling, and referrals.Interviews patients and guarantors at the workstation or bedside to obtain all necessary information, including a copy of the patient or guarantor identification card.May confirm physician and prescription orders ensuring accuracy.May schedule walk-in appointments for services offered.Collects patient out-of-pocket responsibility per collection guidelines. Provides patient estimates as requested. Prepares and balances a daily deposit of all payment collections.Provides exceptional customer service and ensures all questions and concerns are addressed in a timely and courteous manner. May guide the patient to appropriate destination for services.Obtains pre-certification and authorization.Verifies insurance benefits including obtaining insurance card(s) and confirms coverage is active. Determines correct insurance filing order, if multiple insurance coverages are effective for that service.
