Nurse Practitioner

The Nurse Practitioner collaborates closely with physicians, nurses, caregivers, families, hospice, home health, rehabilitation, and other healthcare professionals to ensure coordinated and continuous care. Responsibilities may include performing physical assessments, ordering and interpreting diagnostic tests, managing medications, addressing acute illnesses and chronic disease, providing dementia and geriatric care, and helping patients and families understand treatment options and goals of care.
By providing medical care directly within residential and long-term care settings, the Nurse Practitioner helps improve accessibility, continuity of care, comfort, and overall quality of life while reducing unnecessary hospital visits and supporting patients through changes in their health and functional needs.
Required
Graduate of an accredited Nurse Practitioner program
Current APRN licensure and board certification
Prescriptive authority in the state of Kansas
Experience in primary care, geriatrics, or chronic disease management
Strong assessment, communication, and care coordination skills
Ability to work collaboratively within an interdisciplinary team
CPR certification
Reliable transportation and ability to travel for home/community visits
Ability to work in clinic, home, and community-based settings
Preferred
Experience working with older adult populations and complex medical conditions
Familiarity with electronic health records (EHR)
Strong patient-centered and preventive care approach
Ability to work independently and manage changing participant needs
Experience collaborating with families and caregivers
Ability to recognize early changes in condition to help prevent hospitalizations

Hospice Program
Compassionate, nonprofit end-of-life care service designed to support individuals with life-limiting illnesses and their families by focusing on comfort, dignity, and quality of life rather than curative treatment. The hospice team provides personalized pain and symptom management, emotional and spiritual support, and practical assistance wherever the patient calls home — whether that’s in their own residence, a nursing facility, or at the organization’s dedicated inpatient hospice house for more intensive needs. The program also extends bereavement support for families and works to help patients make the most of the time they have left by addressing physical, social, and emotional needs with sensitivity and respect.
Hospice Admissions Nurse
Plays a key role in helping patients and families begin hospice care with understanding and support. The nurse completes the initial assessment by reviewing the patient’s medical history, evaluating symptoms, and determining eligibility for hospice services. They explain the hospice philosophy, services, and care plan to the patient and family, answer questions, and obtain necessary consents. The RN admissions nurse also collaborates with physicians and the hospice care team to develop an individualized plan of care, ensures medications and equipment are arranged, and helps families feel prepared and supported during the transition to hospice care.

Nurse Practitioner
The Nurse Practitioner provides advanced clinical care to patients with serious or life-limiting illnesses, with an emphasis on comfort, symptom management, and quality of life.
The Nurse Practitioner evaluates patients, diagnoses and treats medical conditions, manages complex symptoms, prescribes medications, and develops or updates individualized care plans. They work closely with patients and families to understand goals of care, explain treatment options, and provide guidance throughout the hospice or palliative care journey. The Nurse Practitioner also collaborates with physicians and the interdisciplinary team—including nurses, social workers, chaplains, and other care professionals—to ensure care is coordinated and aligned with each patient’s wishes.
Required
Current, unrestricted Nurse Practitioner/APRN license in the state of Kansas
Current national board certification as a Nurse Practitioner through an approved certifying body
Graduate degree from an accredited Nurse Practitioner program
Current DEA registration and ability to prescribe medications
Current BLS certification
Experience providing direct patient care as a Nurse Practitioner
Strong assessment, clinical decision-making, and communication skills
Ability to work collaboratively as part of an interdisciplinary care team
Ability to provide compassionate, patient-centered care to individuals with serious or life-limiting illnesses
Ability to work independently and manage multiple patient needs while maintaining accurate clinical documentation
Preferred
Experience in hospice, palliative care, geriatrics, oncology, or another setting caring for patients with serious or life-limiting illnesses
Hospice and Palliative Medicine (HPM) certification or willingness to obtain it
Experience with symptom management, pain management, and end-of-life care
Experience discussing goals of care and advance care planning with patients and families
Experience working within an interdisciplinary team
Familiarity with Medicare hospice regulations and hospice eligibility requirements

Hospice House
A peaceful, 12 bed, inpatient facility dedicated to providing compassionate, end of life care when a home setting is no longer feasible. Here, patients receive around the clock, skilled hospice care from a multidisciplinary team—nurses, social workers, chaplains, and other specialists—focused on managing pain and symptoms with dignity and respect. The environment is designed to feel like home, offering comfort not just to the patients but also to their loved ones, providing space for visitation, emotional support, and moments of peace in the final chapter of life.
RN Hospice House
Provides hands on, comfort focused nursing care to patients receiving end of life services in an inpatient hospice setting. The role centers on assessing and managing symptoms (especially pain), administering medications, and ensuring patients remain as comfortable and dignified as possible. The nurse also updates care plans, documents all care provided, and works closely with the interdisciplinary hospice team (physicians, aides, social workers, chaplains, and volunteers). In addition, they support and educate families, helping them understand what to expect and offering emotional reassurance during a difficult time. Overall, the position combines clinical skill with compassionate, holistic care focused on quality of life rather than curative treatment.

RN Case Manager
The RN Case Manager plays a critical role in coordinating and delivering high quality, patient centered end of life care. They are responsible for assessing patients’ medical needs, developing individualized care plans, and ensuring that patients receive appropriate symptom management, medications, and therapies to maintain comfort and dignity. The RN Case Manager also serves as the primary clinical liaison between patients, families, and the interdisciplinary hospice team, which may include physicians, social workers, chaplains, and volunteers. Additionally, they provide education and emotional support to families, help navigate complex medical decisions, and ensure compliance with regulatory standards, all while advocating for the patient’s wishes and enhancing quality of life during hospice care.
