Job Summary:
Laramie Health and Rehabilitation is seeking a MDS Coordinator Nurse to conduct and coordinate the development and completion of the Resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Area Assessments (CAAs) and Care Plan in accordance with State and Federal requirements. Participates as part of the Nursing Center Interdisciplinary Team (IDT) for Medicare management as assigned.
Essential Functions:
Coordinate the IDT in timely completion of the assessments.
Utilize MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
Communicate ARD selection for specific MDSs to IDT.
Complete Care Area Assessments (CAAs) as required.
Complete care plan items relevant to current MDS and attend care plan meetings as assigned.
Monitor the overall documentation in the medical record to validate that it supports MDS coding.
Participate in Medicare utilization management as assigned.
Education, Licensure, and Experience:
Required: Current LPN or RN Licensure in the State of Employment
Required: Computer aptitude with the EMR for this position
Preferred: Two years’ experience working in a long-term care
Preferred: Experience with Residence Assessment Instrument (RAI) process and MDS certification
Preferred: Point Click Care (PCC) experience

Job Summary:
Laramie Health and Rehabilitation is seeking a Occupational Therapist to join our exciting and fast paced team! Our housekeepers provide daily housekeeping services to maintain the Center's safety, sanitation, and attractive condition.
Essential Functions:
Comply with state practice act and/or American Occupational Therapy Association (AOTA) guidelines.
Develop and implement treatment plans that focus on functional and measurable outcomes with physician order. Monitor residents’ responses to treatment and alter the treatment plan as appropriate with physician approval.
Participate in continuous quality improvement activities.
Identify and advise Rehabilitation Manager (RM) of any Center, staff, or operations issues that need to be addressed.
Communicate resident/staff issues affecting assigned caseload.
Develop/implement Occupational Therapy and/or Dysphagia services in conjunction with the RM and Center staff.
Participate in discharge planning.
Demonstrate supervisory activities in patient charts and service logs as applicable.
Participate in RAFT (Rehab Advocating Function Today) activities to identify residents in need of rehab services.
Provide OT screenings and evaluations to determine residents’ functional level before initiation of treatment.
Maintain productivity at level determined by the RM.
Supervise OT assistants and aides if applicable.
Self-monitor productivity level daily.
Participate in resident/family meetings as well as Center meetings when appropriate.
Follow administrative policies and procedures related to emergency procedures, infection control, and safety.
Maintain consistent and predictable hours of service in buildings as assigned.
Provide clear, concise documentation in medical record and on appropriate forms as it relates to reimbursement guidelines and Center policy and procedure.
Document and sign each treatment encounter in medical record.
Complete progress notes per Center policy.
Participate in Quality Assurance Review and denials process as requested by RM.
Qualifications:
Bachelor or Master of Science in Occupational Therapy from accredited program.
Successful completion of National Certification Exam for Occupational Therapy Registered (OTR).
Current licensure in State of practice where applicable.

Job Summary
Laramie Health and Rehabilitation is seeking a dedicated Physical Therapist (PT) or Physical Therapy Assistant (PTA) to join our exciting and fast-paced rehabilitation team. Team members provide high-quality rehabilitative care focused on improving residents’ functional abilities, independence, and overall quality of life in a compassionate and supportive environment.
Essential Functions
Comply with State practice acts and/or American Physical Therapy Association (APTA) guidelines.
Provide physical therapy evaluations, screenings, and treatments to determine and improve residents’ functional levels.
Develop, implement, and carry out treatment plans established by the Physical Therapist with measurable, functional outcomes and physician orders.
Monitor residents’ responses to treatment and modify care plans as appropriate with physician approval.
Provide direct resident care and therapeutic interventions.
Maintain clear, concise, and timely documentation in medical records in accordance with reimbursement guidelines and facility policy.
Document and sign treatment encounters, supervision activities, and progress notes as required.
Participate in discharge planning and resident/family care meetings.
Collaborate with the Rehabilitation Manager (RM) and interdisciplinary team regarding resident care and operational concerns.
Participate in RAFT (Rehab Advocating Function Today) activities to identify residents in need of rehabilitation services.
Maintain productivity standards established by the Rehabilitation Manager.
Participate in quality improvement and Quality Assurance Review activities.
Follow policies and procedures related to infection control, emergency preparedness, and workplace safety.
Maintain consistent and predictable service hours as assigned.
Supervise PT assistants and aides when applicable.
Review and communicate caseload information regularly with supervising therapists and staff.
Qualifications
Physical Therapist (PT)
Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.
Successful completion of the National Certification Examination for Physical Therapists.
Current state licensure where applicable.
Physical Therapy Assistant (PTA)
Associate degree from an accredited Physical Therapy Assistant program.
Current state licensure where applicable.
