Nurse Practitioner (NP) | Community-Based Primary Care | Southern MN
Twin Cities Physicians View all jobs
Posted 1 month(s) ago
Reference: 2312316
*Company:* Twin Cities Physicians
*Location:* Southern Minnesota | Owatonna, Rochester, Albert Lea, and Surrounding Communities
*Compensation:* *$120,000 - $160,000+ Total Compensation* (Base Salary + Productivity Incentive Program) + Mileage Reimbursement
*Benefits:* Comprehensive Benefits Package, PTO, Paid Holidays, CME Allowance, Malpractice Coverage, DEA Reimbursement, Clinical Resources, and More
*Schedule:* Full-Time or Part-Time | Community-Based Rounding
*To Stand Out*
Email Tom Lecy at *[email protected]* with your resume and tell us:
* About your clinical background.
* Which patient population you most enjoy serving.
* Why *community-based senior medicine* appeals to you.
*Your Practice Doesn't Have to Be Confined to Four Walls.*
You finish seeing one of your longtime patients, update the nursing team on her plan of care, grab your things, and head to your next community.
There's no waiting room full of patients watching the clock.
No spending the entire day in the same exam room.
Instead, you're out in the community practicing medicine where your patients actually live.
At Twin Cities Physicians, we're looking for a Nurse Practitioner who wants to build relationships with older adults throughout *Owatonna, Rochester, Albert Lea, and surrounding Southern Minnesota communities*.
You'll have the independence to manage your patients, the variety of practicing across multiple communities, and the backing of an experienced *physician-led clinical team* when you need it.
For an NP who enjoys *autonomy, geriatrics, and getting out into the community*, this can be an incredibly rewarding way to practice.
*Trade the Traditional Clinic for Community-Based Medicine*
Your patients shouldn't always have to come to you.
Sometimes better healthcare means *going to them*.
You'll provide primary care directly within *assisted living, memory care, independent living, and skilled nursing communities*, caring for older adults with a wide range of clinical needs.
You'll manage chronic conditions, address acute concerns, follow patients after hospitalizations, and work closely with the nurses and caregivers who know them best.
And because you'll follow many of these patients over time, you'll have something that's becoming harder to find in healthcare:
*Continuity.*
You'll know your patients. They'll know you.
*Why Nurse Practitioners Choose Twin Cities Physicians*
* *$120,000 - $160,000+ total compensation.*
* Productivity incentive program with additional earning potential.
* *Mileage reimbursement.*
* Full-Time or Part-Time opportunities.
* Practice at the *top of your license*.
* High degree of *clinical autonomy*.
* Variety without being confined to the same clinic every day.
* Strong physician collaboration and clinical support.
* Opportunity to build meaningful relationships with *older adults and senior living teams*.
* Comprehensive benefits package.
* Paid Time Off and *Paid Holidays*.
* CME Allowance.
* Malpractice Coverage.
* DEA Reimbursement.
* Clinical resources and administrative support.
*Your Office Is the Community*
One day may take you into a memory care community in Owatonna.
Another may include patients around Rochester.
Another may bring you farther south toward Albert Lea.
Wherever you're rounding, your purpose remains the same: *bring proactive medical care directly to seniors who benefit from having a consistent provider where they live*.
You'll work alongside nurses, caregivers, therapists, pharmacists, families, and facility leadership to understand the complete picture surrounding each patient.
That can mean catching a subtle change before it becomes an emergency, helping stabilize a chronic condition, or making sure a hospital discharge plan actually works once the patient returns home.
Your work helps *strengthen continuity of care, reduce unnecessary hospitalizations, and keep older adults healthier where they call home*.
*What You'll Be Doing*
* Provide comprehensive primary care throughout *senior living and long-term care communities*.
* Travel throughout *Owatonna, Rochester, Albert Lea, and surrounding Southern Minnesota communities*.
* Diagnose and manage *acute and chronic medical conditions*.
* Perform comprehensive assessments, preventative care, and routine follow-up visits.
* Develop individualized treatment plans.
* Manage chronic disease and monitor changes in condition.
* Coordinate care following hospitalizations, rehabilitation stays, and specialist visits.
* Collaborate with *physicians, nurses, therapists, pharmacists, caregivers, and senior living teams*.
* Educate patients and families regarding diagnoses, medications, and treatment plans.
* Help improve outcomes through *proactive and preventative medicine*.
* Maintain accurate and timely clinical documentation.
*Clinical Backgrounds That Translate Well*
Experience in any of the following areas is highly valued:
* Primary Care.
* Family Practice.
* Internal Medicine.
* *Geriatrics.*
* Long-Term Care.
* Skilled Nursing.
* Post-Acute Care.
* Hospital Medicine.
* Home Health.
* Hospice & Palliative Care.
You don't need to come directly from senior living. We're especially interested in NPs who genuinely *enjoy caring for older adults* and want the opportunity to build longer-term patient relationships.
*What We're Looking For*
*Required Qualifications*
* Active Minnesota Nurse Practitioner license.
* Excellent assessment, diagnostic, and *clinical decision-making skills*.
* Ability to practice independently while collaborating with physicians.
* Strong communication and care coordination skills.
* *Passion for serving older adults.*
* Comfortable managing medically complex patients.
* Reliable transportation and willingness to travel throughout *Southern Minnesota*.
*Preferred Qualifications*
* Experience caring for *medically complex older adults*.
* Chronic disease management expertise.
* Community-based or senior care experience.
* Assisted living, memory care, or skilled nursing experience.
* Advanced wound care experience.
* *Wound Care Certification (WCC)* is a plus.
*More Freedom. More Variety. More Connection.*
Some providers thrive inside a traditional clinic.
Others want something different.
They want to *get out into the community*, manage their own patient population, work with different clinical teams, and build relationships with patients beyond a single appointment.
They want independence without practicing alone.
Most importantly, they want to feel connected to the people they're caring for.
If that sounds like the way you want to practice medicine, *Southern Minnesota needs providers like you*.
*About Twin Cities Physicians*
Twin Cities Physicians is a growing, *physician-led medical group* providing proactive primary care services to thousands of seniors throughout Minnesota.
Our providers bring healthcare directly into senior living communities, helping improve *access, continuity of care, quality of life, and patient outcomes while reducing unnecessary hospitalizations*.
If you're ready to *leave the traditional clinic behind, get into the community, and build a practice around the older adults who need you*, we'd love to meet you.
Pay: $120,000.00 - $160,000.00 per year
Benefits:
* 401(k)
* 401(k) matching
* Dental insurance
* Flexible spending account
* Health insurance
* Health savings account
* Life insurance
* Paid time off
* Vision insurance

Nashville, TN
Are you over 30...and STILL love Sponge-Bob?
At Volunteer Orthodontics, where patients rave on Google and their parents bring us baked goods, you won’t just find a high-performing office—you’ll find a team that believes in purpose-driven dentistry and celebrates wins like family. We’re deeply rooted in the community, proudly serving special needs patients and partnering with local organizations that keep us grounded in what matters most. Whether you're a new grad eager to learn, a seasoned associate ready for mentorship or leadership, or someone eyeing a future path to ownership, this is the kind of place that supports your goals without micromanaging your style. We believe in clinical autonomy, genuine collaboration, and a culture where growth is real and laughter is required.
Why Join our office?
No committee meetings or 47-step approval chains here—just great dentistry, loyal patients, and freedom to do what you do best. Add mentorship, a fun-loving team, and a tech-savvy setup, and you’ve got a pretty sweet gig.
Fully digital workflow—no clipboards, no paperwork, just a seamless experience for you and your patients
Enjoy the Clinical autonomy to “practice as you please” without corporate red tape
Thrive in a family-oriented, fun, and supportive culture
Receive mentorship from seasoned clinicians
Be part of a team that values joy in the workday and connection with patients
Serve a loyal patient base, including individuals with special needs and unique care needs
Compensation & Benefits:
You bring the skills—we’ll fill the schedule. With our proven playbook and support system, our doctors average $500K in annual compensation, (yes, really), and we’ll help you get there and beyond. Drill, fill, and thrive with a fully staffed team, admin support that has your back, and a schedule designed to grow your career and protect your sanity.
Defined path to ownership/partnership—if you want it, we’ll help you earn it. Ready to build your legacy? We’ll show you how—and cheer you on while you do it.
Highly competitive comp plan with unlimited earning potential—Bring your skills, fill your schedule, and watch the "unlimited" part happen faster than you think.
Flexible schedule built for real work-life balance—Dentistry is what you do, not your entire identity. Go enjoy your weekends—and maybe even some weekdays.
Health, Dental & Vision Insurance and so much more! Because taking care of patients starts with taking care of you.
401(k) Program to invest in your future—Your future self called—they said, "Thanks for thinking ahead."
CE & hands-on training opportunities (think CBCT, intraoral scanners, advanced tech), because getting better at what you love should feel exciting—not like homework.
Full admin support so you can focus on patients, not paperwork—Say goodbye to chasing insurance claims and hello to more time doing dentistry.
A high-performance culture—built on excellence, teamwork, and real support where people push each other to grow and have a little fun doing it.
Your Responsibilities:
Yes, you’ll be diagnosing, drilling, and dazzling patients with your clinical skills—but let’s be real, we’re looking for more than just a steady hand and a sharp eye. You’re the kind of doctor who can explain a treatment plan without sounding like a textbook, build trust that keeps patients coming back, and lead a team like it’s second nature (with a few lame dad jokes along the way). If you’re all about high standards, continuous growth, and believe that leadership means serving first—welcome to the crusade.
Bonus points if RavingFans and EntreLeadership are already dog-eared on your bookshelf.
Diagnose and provide excellent patient care and treatment
We’re talking top-notch care that’s more "wow" and less "just okay"—because your reputation deserves to be as sharp as your skills.
Educate patients on oral health and treatment options
You’ll turn confusing dental jargon into "aha!" moments—and maybe even make flossing sound fun (or at least convincing).
Develop and maintain trusting, long-term patient relationships
From first visits to lifelong care, you’ll be the dentist patients trust—and yes, they might just bake you cookies.
Partner with and lead a supportive, experienced team
Think of it like being the captain of a winning squad—everyone’s here to help you (and each other) succeed.
Maintain high clinical standards and a positive work environment
Deliver the kind of quality you’d want for your own family—and create a vibe where people enjoy coming to work (no forced smiles required).
Qualifications:
You’ve completed a Pediatric Dental Residency, have your DMD or DDS degree, an active license with no drama attached, and a chairside manner that puts people of all ages at ease before they even sit down. You’re a confident communicator, a natural leader, and a team player who treats patients like people—not procedures. If you’ve got clinical chops, a collaborative spirit, and compassion baked into everything you do, we already like your style.
DMD or DDS from an accredited dental school
Active [state specific] dental license (in good standing, no disciplinary actions)
Strong communication and leadership skills
Passion for teamwork and treating patients with compassion
Your dream job called—it said to bring your handpiece and hit apply.
