Responsibilities:
1. Conduct comprehensive evaluations of patients to determine their physical therapy needs and develop individualized treatment plans.
2. Provide evidence-based outpatient physical therapy services to patients with various musculoskeletal and neurological conditions.
3. Implement therapeutic exercises, manual therapy techniques, and modalities to improve patients’ mobility, strength, and function.
4. Monitor patients’ progress closely and adjust treatment plans as needed to ensure optimal outcomes.
5. Educate patients and their families on therapeutic exercises and home exercise programs to facilitate continued progress outside of therapy sessions.
6. Collaborate with interdisciplinary team members, including physicians, nurses, and other therapists, to coordinate patient care and achieve treatment goals.
7. Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans in accordance with facility policies and regulatory requirements.
Qualifications:
1. Graduate of an accredited physical therapy program.
2. Current state licensure as a physical therapist.
3. Minimum of 2 years of experience working as a physical therapist, preferably in an outpatient or skilled nursing facility setting. (Preferred but not required)
4. Strong clinical skills in evaluating and treating patients with orthopedic and neurological conditions.
5. Excellent communication and interpersonal skills.
6. Ability to work independently as well as collaboratively within a multidisciplinary team.
7. Commitment to providing compassionate and patient-centered care.

The BCBA will be responsible for overseeing ABA programming for pediatric clients, supervising RBTs, collaborating with families and interdisciplinary teams, and ensuring clinical excellence across services.

JOB SUMMARY
General Description: Responsible for providing a wide range of mental health services to families with children that have emotional disturbances requiring intensive intervention using family and group treatment methods.
Responsibilities and Duties: Responsible for the evaluation and diagnoses of recipients. Provide services for families with children, within the home and the community, that have emotional disturbances requiring intensive intervention. Provide crisis intervention at the appropriate settings. Provide individual, mental, family and group therapy based on the person centered
plan and best practice standards. Other responsibilities include case management, consultation, and education. Provide consultation and coordination of services with other community agencies such as the Family Court and The Department of Human Services. Requires the ability to work with multiple agencies to facilitate community treatment plans and to work without direct supervision. Occupational Therapy Assistant will assist in providing appropriate aspects of quality patient care, under the supervision of licensed therapists. OTA will document all services provided to ensure accurate and timely billing of therapy services. The OTA assures that patient care follows the OT Plan of Care, the organization’s guidelines, meets professional standards, community needs and is in compliance with Federal and State guidelines.
Job Duties:
* Provides Therapy services established by the Occupational Therapist (OTR) in the patient Plan of Care.
* Provides therapy services under the supervision of an OTR as determined by the Registered Therapist and in compliance with State and Federal guidelines.
* Consults with OTR regarding changes in treatment or condition in a timely manner.
* Appropriately and effectively completes documentation including daily notes and assisting the OTR to create progress notes, discharge summaries, or monthly reports as needed.
* Submits billing, payroll, and expense documentation in a timely manner.
* Communicates with patients, families, referring physicians and other members of the team.
* Attends continuing education seminars/workshops to keep abreast of new clinical skills, programs or techniques.
* Participates in care planning meetings, rehab meetings and family training in order to communicate patient progress as requested by OTR, Director of Rehab or Rehab Resource Consultant.
* Follows organizational and facility policies and procedures to ensure employee and patient safety.
* Ensures equipment is in good working order prior to providing care to patients. Follows procedures for reporting damaged equipment.
* Maintains a clean environment.
* Attends required meetings, in-services and functions as designated by the Director of Rehab and /or the Rehab Resource Consultant.
* Coordinates treatment goals with other team members.
* Maintains valid licensure, certification or equivalent as requested by appropriate state agency.
Requirements
*Education Required:*
* Minimum of Associate of Arts in Occupational Therapy Assistant
* *Licensure/Certification: *Licensure as required by State guideline
* Certification as required by State
* CPR Certification
*Experience:*
* 1 year Occupational Therapy Assistant experience preferred.
* 1-2 years experience in a residential/ assisted living or skilled nursing facility preferred.
* Graduates in Occupational Therapy Assistant Program will be considered.

Pay: $40.00 - $55.00 per hour
