Requirements

Position Summary
Mountain View Health & Rehabilitation is looking for a *compassionate, motivated, and team-oriented Physical Therapist* to join our growing rehabilitation team.
Our therapy department is a cornerstone of the care we provide. We believe therapy should be more than completing exercises—it should help residents regain confidence, maintain independence, and get back to doing the things that matter most to them.
The ideal candidate is someone who brings clinical expertise, positive energy, creativity, and genuine compassion to their work. You will have the opportunity to work closely with residents, families, nursing, therapy, and other members of the interdisciplinary team to provide individualized, resident-centered care.
Essential Duties & Responsibilities
* Evaluate residents to determine physical therapy needs and develop individualized treatment plans.
* Provide skilled physical therapy services in accordance with physician orders, care plans, and applicable regulations.
* Develop treatment goals based on each resident's medical condition, functional abilities, and personal goals.
* Provide therapeutic exercises, functional mobility training, gait training, balance activities, strengthening, and other appropriate interventions.
* Assist residents with improving mobility, strength, coordination, balance, endurance, and overall functional independence.
* Monitor residents' progress and modify treatment plans as appropriate.
* Document evaluations, treatments, progress, and discharge information accurately and timely.
* Communicate changes in residents' condition or functional status to the appropriate members of the care team.
* Collaborate with nursing, occupational therapy, speech therapy, social services, dietary, and other disciplines to provide comprehensive care.
* Participate in care plan meetings and interdisciplinary team discussions as appropriate.
* Educate residents and families regarding treatment plans, safety, mobility, exercises, and strategies for continued progress.
* Recommend and assist with appropriate adaptive equipment, positioning, mobility devices, and safety interventions.
* Promote fall prevention and safe mobility throughout the facility.
* Assist with discharge planning and provide recommendations for continued therapy or services when appropriate.
* Maintain compliance with facility policies, professional standards, and applicable federal and state regulations.
* Maintain a clean, safe, and organized therapy environment.
* Participate in quality improvement initiatives and facility programs as requested.
* Perform other duties as assigned.

Lincoln Medical Center is seeking an X-Ray/CT Tech (Radiology Technologist) to join our Radiology department in a full-time position. The ideal candidate would have at least 1 year of experience.
The basic purpose of this position is to perform interpretive, patient care and technical functions to insure proper radiologic diagnosis of patients. Responsibilities include various tasks concerning film/report management, secretarial duties, patient transportation, and darkroom technique, and the transmittal of films via the digitizer.
_*Essential Functions:*_
* Accepts patients and assesses their condition to determine proper handling and special requirements.
* Explains and prepares patient for all radiographic and fluoroscopic procedures.
* Relies on extensive knowledge of anatomy, physiology, and effects of disease processes to properly and safely position the patient for maximum diagnostic yield.
* Operate highly complex radiographic equipment within the radiology department, operating suites and at the patient's bedside, selecting technical factors to ensure high-quality radiographs using the A.L.A.R.A. (as low as reasonable achievable) standards.
* Maintains radiographic equipment and x-ray rooms in proper working conditions.
* Performs various clerical duties, but not limited to: verification of information and exams performed, update and change exam descriptions to reflect actual exams performed when verifying, ensure correct billing of patient accounts.
* Assist with examination of higher complexity such as arteriography, myelography, etc.

* Graduate of an accredited Radiologic Technology program
* ARRT registered

Job Description:
OneWell Health Care is seeking a compassionate Physical Therapist (PT) to provide personalized therapy to individuals with Intellectual and Developmental Disabilities (IDD) in home and community-based settings in Pennsylvania.
You will evaluate each individual's physical needs, develop goal-based treatment plans, and collaborate with caregivers and other professionals to improve mobility, strength, balance, safety, and overall independence.

What We're Looking For: :
- Doctorate or Master's degree in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in the state of practice.
- Current CPR certification (preferred or required in accordance with organizational policy).
- Strong clinical, documentation, organizational, and communication skills.
- Proficiency in functional mobility and gait assessments, balance and postural control interventions, adaptive and assistive equipment recommendations, evidence-based physical therapy techniques, and electronic health record (EHR) systems.
- Valid driver's license, a clean driving record, reliable transportation, and willingness to travel to home and community-based service locations. Flexibility to support varying schedules and assigned service areas is required.
- Experience supporting individuals with Intellectual and Developmental Disabilities (IDD) and providing physical therapy services in home or community-based settings is preferred.
- English proficiency is required. Bilingual proficiency (including Spanish) is a plus.
