*Expectations of Partnership:*
As a Mindful Therapy Group partner, you’ll be expected to:
* Invest in starting and growing your private practice, including maintaining a business license, professional liability insurance, and a professional online presence. (Not needed upon applying)
* Participate in regular business setup and coaching sessions for guidance, accountability, and support as your practice grows.
* Maintain consistent client availability to support continuity of care.
* Complete evaluations, assessments, treatment plans, chart notes, charge slips, and other required documentation in a timely manner.
* Practice within your licensure scope, uphold clinical and ethical standards, safeguard client confidentiality, and maintain active licensure in any state where services are provided.

Key Responsibilities
Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.
Collaborate with the interdisciplinary care team to optimize patient outcomes.
Provide education and support to patients and families/caregivers on therapy programs.
Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
Identify patient and family/caregiver needs for additional services and make appropriate referrals.
Confirm daily visit schedules to ensure seamless coordination with other staff.
Communicate patient progress and changes with physicians and care teams.
Document all visits and progress notes on the same day of service.
Requirements:
Qualifications
Must have a current PT license (state-specific).
Graduate of an accredited physical therapy program.
Valid BLS certification.
Minimum one year of PT experience (home health experience preferred).
Excellent communication, organizational, and time management skills.
Must have a valid driver’s license and access to a reliable and insured vehicle.
Ability to visit homes that may not be wheelchair accessible.
Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
Proficiency with electronic medical records is a plus.

This is not a private contractor arrangement. It is a dynamic, W-2 group practice based on three pillars: financial viability, ethical integrity and risk management, and clinical efficacy. You will gain from and contribute to a real team. We use a private practice commission model, offering a percentage of earned revenue. Nurse Practitioner
The Nurse Practitioner provides comprehensive, patient-centered primary care to older adults and individuals residing in rehabilitation centers, skilled nursing facilities, assisted living communities, and independent living settings. The role focuses on assessing and managing acute and chronic health conditions, monitoring changes in patients’ health, developing individualized treatment plans, and supporting patients throughout recovery and long-term care.
The Nurse Practitioner collaborates closely with physicians, nurses, caregivers, families, hospice, home health, rehabilitation, and other healthcare professionals to ensure coordinated and continuous care. Responsibilities may include performing physical assessments, ordering and interpreting diagnostic tests, managing medications, addressing acute illnesses and chronic disease, providing dementia and geriatric care, and helping patients and families understand treatment options and goals of care.
By providing medical care directly within residential and long-term care settings, the Nurse Practitioner helps improve accessibility, continuity of care, comfort, and overall quality of life while reducing unnecessary hospital visits and supporting patients through changes in their health and functional needs.
Required
Graduate of an accredited Nurse Practitioner program
Current APRN licensure and board certification
Prescriptive authority in the state of Kansas
Experience in primary care, geriatrics, or chronic disease management
Strong assessment, communication, and care coordination skills
Ability to work collaboratively within an interdisciplinary team
CPR certification
Reliable transportation and ability to travel for home/community visits
Ability to work in clinic, home, and community-based settings
Preferred
Experience working with older adult populations and complex medical conditions
Familiarity with electronic health records (EHR)
Strong patient-centered and preventive care approach
Ability to work independently and manage changing participant needs
Experience collaborating with families and caregivers
Ability to recognize early changes in condition to help prevent hospitalizations
