Summary of Responsibilities:
Provide physician orders and plan of care under the direction and supervision of the Director of Clinical Services.
Work in conjunction with all other home care personnel in patient care planning.
Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
Take initial patient history and make initial evaluation.
Consult with physicians regarding change in treatment.
Instruct patients and family in home programs and activities of daily living.
Participates in Quality Assessment and Performance Improvement activities as assigned.
Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
Appropriately utilizes ICD-10 codes.
Requirements:
Graduate from a physical therapist curriculum approved by the American Physical Therapy Association.
Currently licensed as a physical therapist in the state of Washington.
Two (2) years' experience as a physical therapist, preferred.
Home Health experience, preferred.
Detail oriented, self motivated and capable of efficient time management.
Critical thinker capable of working collaboratively with a team.
Valid WA State driver's license.

*About the Position*
We're looking for a highly skilled Physical Therapist to join our home health team in Marin County. This is a part time position. You'll provide exceptional care to patients in their homes, helping them regain their strength and independence. You'll work closely with physicians, nurses, and other healthcare professionals to develop and carry out individualized treatment plans that meet the unique needs of each patient. The goal is to help every patient reach their maximum potential and improve their quality of life.
*Coverage Area:* Marin County, CA. Candidates must be able to visit patients in their homes throughout the Marin County area.
*Compensation:* $55.00 - $60.00 per hour based on experience
*What You'll Do*
* Conduct patient evaluations and develop individualized treatment plans
* Provide hands-on therapy to patients in their homes
* Educate patients and their families on proper techniques and exercises

* At least 1 year of home health experience

Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
Develops, implements and evaluates the plan of care in partnership with the client, representative, caregiver(s) and physician(s)
Provides direct occupational therapy services in accordance with the plan of care established by the physician
Educates and instructs client, family members or other client representatives, in rehabilitation care and activities to necessary to promote the client's health, safety and independent living
Accurately documents observations, interventions and evaluations pertaining to client care management and services provided
Select activities that will help individuals learn work and life-management skills within limits of their mental or physical capabilities
Recommend changes in patients' work or living environments, consistent with their needs and capabilities
Engage in research activities and participation in formal educational training activities related to therapy.
Perform other duties as assigned
Minimum Qualifications (Knowledge, Skills, and Abilities)
Degree from an accredited Occupational Therapist Assistant Program (approved by the AOTA)
Current and valid COTA License for Virginia (required)
Recent work experience in home health (preferred)
Driver’s License, Car Insurance, Car Registration
Strong clinical assessment, organizational and communication skills
Ability to handle multiple assignments simultaneously
Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation
Bilingual (Spanish) preferred
Excellent oral and written communication skills
Excellent customer relationship skills

Responsibilities:
What You'll Do
Provide skilled occupational therapy services in accordance with physician orders and individualized plans of care.
• Evaluate patients and develop personalized treatment programs to improve function, safety, and independence.
• Assess patients' ability to perform activities of daily living (ADLs) and recommend adaptive techniques or equipment as needed.
• Document patient evaluations, treatments, and progress in a timely and accurate manner.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure optimal patient outcomes.
• Educate patients and caregivers on home safety, adaptive strategies, and ongoing therapy goals.
• Supervise Certified Occupational Therapy Assistants (COTAs) as appropriate.

Qualifications:
Qualifications
Graduate of an accredited Occupational Therapy program.
• Current Occupational Therapist license in Pennsylvania.
• Current CPR certification (or ability to obtain upon hire).
• Valid driver's license, reliable transportation, and proof of auto insurance.
• Strong communication, organizational, and documentation skills.
• Home health experience preferred, but not required.

Key Responsibilities
Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.
Collaborate with the interdisciplinary care team to optimize patient outcomes.
Provide education and support to patients and families/caregivers on therapy programs.
Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.

Requirements:
Qualifications
Must have a current PT license (state-specific).
Graduate of an accredited physical therapy program.
Valid BLS certification.
Minimum one year of PT experience (home health experience preferred).
Excellent communication, organizational, and time management skills.
Must have a valid driver’s license and access to a reliable and insured vehicle.
Ability to visit homes that may not be wheelchair accessible.
Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
Proficiency with electronic medical records is a plus.
