*Company:* Sunflower Communities | Elmore Assisted Living
*Location:* Elmore, Minnesota
*Schedule:* *Full-Time or Part-Time | Evening Shift | Every Other Weekend + Shared Rotating On-Call*
*Compensation:* *$35.00 - $50.00/hour + $4,000 Sign-On Bonus + $1.00/hour Weekend Differential*
*Want to stand out from other applicants? Email Nikky Vogelgesang directly at [email protected]* with your resume, a brief overview of your nursing background, and whether you're interested in *full-time or part-time evening hours*.
*When the Day Changes, You’re the Nurse Who Knows What Happens Next*
Evenings have their own rhythm in senior living.
Families call with questions. Medications and treatments need attention. A caregiver notices that a resident doesn't seem quite right. Someone returns from an appointment or hospital stay. A resident with dementia is having a more difficult evening than usual.
And when a clinical question comes up, *the team needs an RN who can assess the situation and help determine what happens next.*
At Elmore Assisted Living, we're hiring an *Evening Registered Nurse* to provide that clinical presence for our Assisted Living and Memory Care community.
This is a role for a nurse who likes being trusted, using their judgment, and helping an entire team provide better care.
*Be the Clinical Anchor for the Evening*
Your value here isn't measured by how quickly you can move from one task to another.
It's your ability to *assess, prioritize, communicate, and provide direction* when residents or caregivers need nursing support.
Some situations require an assessment. Others require a provider call, family conversation, medication follow-up, documentation, or simply an experienced nurse saying, *“Here's what we're going to do.”*
That's where your RN experience matters.
*Why Consider Elmore?*
* *$35.00 - $50.00/hour* based on experience
* *$4,000 sign-on bonus*
* *+$1.00/hour weekend differential*
* *Full-Time or Part-Time* opportunities
* Evening shift
* *Every-other-weekend rotation*
* Shared rotating on-call responsibilities
* Work in *Assisted Living and Memory Care*
* Use your assessment skills and clinical judgment
* Provide clinical guidance to an established caregiving team
* Build relationships with residents and families
* Join a *family-owned Minnesota senior living organization*
*What You'll Do*
As an RN at Elmore Assisted Living, you'll:
* Perform nursing assessments and respond to *changes in condition*
* Administer and oversee medications and treatments
* Develop and update individualized service plans
* Provide *clinical guidance to caregivers and medication staff*
* Communicate with residents and families
* Coordinate with physicians, pharmacies, hospitals, home health, hospice, therapy, and other healthcare partners
* Support admissions and transitions of care
* Maintain accurate clinical documentation
* Support *Minnesota Assisted Living regulatory compliance*
* Participate in shared rotating on-call responsibilities
*Bring the Experience You've Already Built*
You'll need an active *Minnesota Registered Nurse (RN) license*, strong assessment skills, sound clinical judgment, and the confidence to provide clinical direction when your team needs support.
Experience in *Assisted Living, Memory Care, Long-Term Care, Skilled Nursing, Home Health, Hospice, Geriatrics, or other settings serving older adults* can translate especially well.
We're looking for someone who can hear *“Can you come take a look at this resident?”* and know what questions to ask next.
*A Border Community Means We're Recruiting From Both Sides*
Elmore sits near the *Minnesota-Iowa border*, so we're interested in connecting with nurses throughout southern Minnesota and northern Iowa who can reasonably commute to the community.
On the *Minnesota side*, that includes communities such as *Blue Earth, Winnebago, Fairmont, East Chain, Frost, Bricelyn, and surrounding areas*.
On the *Iowa side*, we're also interested in nurses around *Buffalo Center, Lakota, Ledyard, Swea City, Bancroft, Lake Mills, Forest City, Estherville, and surrounding northern Iowa communities*.
If you're an Iowa-based RN interested in the position, you'll need to be able to meet the *Minnesota RN licensure requirement* for the role.
*Your Shift May Be Evening. Your Impact Isn't Secondary.*
An experienced evening RN can change how confidently an entire community operates after the traditional workday ends.
You're there to assess the change. Answer the clinical question. Guide the caregiver. Communicate with the family. Contact the provider when necessary. And make sure the resident receives the appropriate next level of care.
If you're looking for *$35.00 - $50.00/hour, a $4,000 sign-on bonus, full-time or part-time options, and an RN position where people genuinely rely on your clinical judgment*, we'd like to meet you.
*Want to stand out from other applicants? Email Nikky Vogelgesang directly at [email protected]* with your resume, a brief overview of your nursing background, and whether you're interested in *full-time or part-time evening hours*.
Pay: $35.00 - $50.00 per hour
Application Question(s):
* Email Nikky Vogelgesang at [email protected] with your resume and tell us:

Midland Care Connection is a not-for-profit, community-based organization in Topeka, Kansas, dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.
Hospice Program
Compassionate, nonprofit end-of-life care service designed to support individuals with life-limiting illnesses and their families by focusing on comfort, dignity, and quality of life rather than curative treatment. The hospice team provides personalized pain and symptom management, emotional and spiritual support, and practical assistance wherever the patient calls home — whether that’s in their own residence, a nursing facility, or at the organization’s dedicated inpatient hospice house for more intensive needs. The program also extends bereavement support for families and works to help patients make the most of the time they have left by addressing physical, social, and emotional needs with sensitivity and respect.
Hospice Admissions Nurse
Plays a key role in helping patients and families begin hospice care with understanding and support. The nurse completes the initial assessment by reviewing the patient’s medical history, evaluating symptoms, and determining eligibility for hospice services. They explain the hospice philosophy, services, and care plan to the patient and family, answer questions, and obtain necessary consents. The RN admissions nurse also collaborates with physicians and the hospice care team to develop an individualized plan of care, ensures medications and equipment are arranged, and helps families feel prepared and supported during the transition to hospice care.

This is a new model of care — where you're not just treating symptoms, you're owning outcomes. You work with the same residents every day, track their progress, and build real relationships. No passing through like in a hospital. No hospital grind. Just respiratory care done right, with the time and autonomy to actually do it.
If you've ever wanted to see your impact over weeks and months instead of a single shift — this is it.
What you'll do
Provide hands-on respiratory care — nebs, assessments, airway clearance, and more
Track resident progress and adjust care plans over time
Partner directly with nursing and physicians to improve outcomes
Be the respiratory expert in the building
What you need

Graduate of an AMA-approved respiratory therapy program
Current BLS (or within 30 days of hire)
ACLS if working ventilator units (or within 6 months of hire)
Strong clinical judgment, communication, and the ability to work independently
Why Banyan
Consistent patients — you actually see improvement over time
Autonomy — you're the respiratory lead in the building
Support — we handle the backend (billing, scheduling, compliance) so you can focus on care
Growth — help build something new in post-acute respiratory care
Better pay + referral bonuses — bring your favorite coworkers with you
The role, practically
Setting: Travel between nearby partner facilities
Schedule: Depends on FT/PRN status; 7 am - 7 pm shifts
Physical: Frequent walking, standing, reaching and handling of respiratory equipment throughout the day
The bottom line This is relationship-based care with real outcomes. If you want to change how respiratory care is delivered in post-acute settings — apply.
