Main Job Responsibilities:
Serve as a coordinator between facility employees and departments and Eden Senior Care management.
Provide oversight of the Nursing Department
Planning services provided to residents and supervising their care

Qualifications:
License registered nurse (RN) in the State of Wisconsin;
Proven experience in a skilled nursing home environment with at least one year of successful experience as a Unit Manager, Assistant Director of Nursing, OR Director of Nursing (DON);
Knowledge of federal and state survey processes;
Work history in the long-term care industry;
Wound Care Certification (preferred, but not required);
Commitment to team-oriented outcomes and quality care; and

* JOB SUMMARY: Provides competent health care to health centers’ patients according to the policies/procedures of CAA and within the scope of physician’s licensure and experience
* SPECIFIC DUTIES:
ESSENTIAL FUNCTIONS
1. Provides primary care in accordance with established health center protocols within the physician’s scope of practice and Ohio Scope of Practice guidelines
2. Completes assessments, medication regime, and monitors progress of clients in Medication Assisted Treatment Program
3. Routinely screens for cognitive impairment, common mental health problems and behaviors that compromise health, using evidence-based and developmentally appropriate tools
4. Supports patient centered care by electronically organizing and documenting problem list, care plan, patient goals, and team communication
5. Participates in quality improvement activities as assigned by the Medical Director and health
center’s supervisory staff
6. Complete and submit in a timely manner all patient charges
7. Serve as clinical preceptor for interns, residents and medical staff as assigned by the Medical
Director or COO
8. Collaborates with patients and family members to make decisions about treatment and attaining
health goals
9. Provides direction for other health center staff regarding medical treatment and decision-making
10. Participates in community outreach programs as assigned by Medical Director or COO
11. Follows CAA policies/procedures including current HIPAA regulations
12. Travel required for trainings, CME opportunities, satellite sites, etc.
13. Participates in recruitment/retention activities
14. Maintains compliance with all laws, regulations, and policies as defined by Federal, State
and any other regulatory body
15. Other duties as assigned
REQUIRED JOB QUALIFICATIONS:
* Current license to practice medicine in Ohio
* Board eligible or Board Certified by an appropriate American Medical Specialty Board
* X-Waiver – Certified MD for MAT Program
* Current DEA registration
* Participation in committees, collaboratives, health fairs and other assigned programs
* Secure/maintain hospital privileges
* CMEs as required to maintain licensure
ADDITIONAL REQUIREMENTS AND CONDITIONS OF EMPLOYMENT:
* 1 year of experience in ambulatory rural health center is preferred
* MAT program experience preferred
* Effective interpersonal communication skills
* Background check including Ohio Bureau of Criminal Investigation, Medicare exclusion list, verification of valid driver’s license, employment medical exam, including a TB test, adherence to all CAA policies/procedures and requirements of all funding sources. CAA is a drug/alcohol-free and smoke-free workplace and post-offer drug and alcohol testing is required.
* Ability to work in several locations with diverse staff
* Ability to sustain CAA’s required provider productivity rate
PHYSICAL REQUIREMENTS:
* Ability to lift/carry up to 50 pounds
* Ability to push, pull, carry, lift, bend, twist, reach, sit, stand for lengthy periods of time; ability to walk for extended periods of time
* Ability to work in a fast-paced and often high-stress environment as well as the ability to adapt quickly to workload changes

* Above-average oral, reading and writing skills to accurately operate a computer, perform necessary documentation and effectively communicate with staff, patients, clients, customers, business partners, other CAA department visitors

Chatterbox is recruiting a physical therapist to help reestablish PT services alongside our speech-language pathology and occupational therapy team.
This opportunity offers a role in developing care that connects movement and mobility with children’s everyday participation.

Partner with leadership to develop PT workflows and use available resources to support individualized care, purposeful play, and caregiver priorities.

Chatterbox is led by Beth Fleming, M.S., CCC-SLP, CEO and Clinical & Executive Director, alongside her husband, Tim Fleming, Finance Director and licensed Orthotist/Prosthetist.
Beth has practiced as an SLP since 2009, founded Chatterbox in 2011, and remains involved in pediatric patient care. Tim continues to provide cranial helmet remolding services alongside Dr. Kumar in Savannah.
Together, they bring clinical and operational perspectives to practice leadership. You’ll collaborate with SLP and OT colleagues in a clinic that also provides feeding services.

We pay for actual direct-care hours and administrative work, including documentation, planning, caregiver communication, care coordination, and other assigned responsibilities.

New graduate: $23.46/hour - $27.60/hour (Applies to Direct Clinical Care)
Experienced: $27.06/hour - $31.86/hour (Applies to Direct Clinical Care)
Administrative work: $18/hour (Applies to Documentation, Planning, Intedisciplinary Collaboration, etc)
Designated onboarding training: $15/hour

POSITION OVERVIEW: The Speech Language Pathologistevaluates, plans, and provides treatment and rehabilitation as prescribed by a referring physician. Works with family and team members regarding patient’s abilities and goals.

The Registered Nurse in the Medical/Surgical Department is accountable for the coordination of nursing care, patient education, and discharge planning.
Essential Functions
Use assessment skills and techniques to determine patient needs and condition upon admission and on an ongoing basis, responding appropriately.

Hourly Rounding on patients.

They have a long term periodontist that is retiring, so they have a busy referral base already developed. They average over 1,600 new patient *referrals *each year. They are a fee for service office.
The office is a beautiful one in a great location. It is *completely state of the art*, with digital work flows, CBCT, guided surgery, intraoral scanning, photogrammetry and PRF. They have an *experienced and well trained team*. There is wonderful collaboration and or *mentoring *as well. This is a *100% FFS *office.
They are offering a *425K guaranteed salary or 40% of net production*, whichever is highers. Health insurance, and continuing education stipend. There is *buy-in potential *as well, if interested but certainly not required.

*Procedures:*
* LANAP and traditional osseous surgery
* Implant placement — single, multiple, and full-arch cases
* Bone grafting and site development for implant placement
* Soft-tissue grafting, including connective tissue grafts, free gingival grafts, tunneling, and allograft procedures
* Functional and aesthetic crown lengthening
* Extractions
* Scaling and root planing
* IV sedation
*Periodontist Requirements:*
* Doctor of dental surgery (DDS) or doctor of medicine in dentistry (DMD).
* Periodontic residency completed or in the process of doing so.
* State license to practice dentistry.
